How to choose an implant?

You appear to be an excellent candidate for augmentation. I would agree with the stated operative plan of sub-muscular augmentation. Implant selection is challenging and should represent a marriage of goals/expectations and the unique chest dimensions of each patient.

Cup size can be misleading and is difficult to correlate with a particular volume. Sizers and 3D imaging can be helpful. Providing your surgeon with sample photos illustrating the upper and lower limits of the result you are seeking can also be of use.

Submuscular and subglandular are toatally different in their goals and effects

Submuscular placement of implants vs “in front of muscle breast implants” are totally different operations in their goals and effects.

Simply put subgladular implants give better cleavage and more natural behavior of breasts. Submuscular implants tend to move with movement of chest muscles, have muscle flex deformity, suboptimal cleavage but less long term sagging.

However, it is your doctor who gets to male final recommendations based upon your own unique body, needs and desires.

Breast augmentation size involves several factors

I think you have very realistic goals for achieving a good result from breast augmentation surgery.

Factors to be considered are
1) amount of breast tissue and elasticity of your skin
2) diameter of your breast
3) breast size you would like to achieve.

I belive that placing silicone implants below the muscle will achieve a very natural look for you. Your desire to be smaller, and proportional (not to large) to your body size, will also increase your chances of having an optimal outcome.
I think the decisions you have made are appropriate for your goals of achieving a natural appearing look from your surgery.

A persons genetic makeup determines whether they are prone to stretch marks.

The best indication as to how an incision will heal ( whether it will leave a “bad” or thick “keloid” scar) is how a patient has healed before. That is the scarring is dependent on the persons genetic makeup.

The same is true for stretch marks. Some women are prone to stretch marks – they have them even though they were never heavy or even had children. Other women have gained and lost weight or had large babies and do not have stretch marks.

Immediately after surgery stretch marks which were present before surgery can be more noticeable . Once the swelling has gone down in general the stretch marks return to their appearance before the surgery.


Tuberous Breast Deformity

You do not appear to have tuberous breasts. You do, on the other hand, appear to have a good bit of ptosis or sagging. In order to improve this, you would need a mastopexy or breast lift with or without an implant.

You should obtain a consultation with a plastic surgeon with whom you can discuss your various options.

Gummy Bear Implants – What are They?

Gummy bear implants are highly cohesive silicone gel implants. The gummy bear label is applied to silicone gel implants which have a cohesive or “sticky” silicone gel inside them.

This type of silicone keeps its shape longer and if the outer shell ruptures, the gel stays put. These are typically in “anatomic” implants meaning the implant is shaped like a tear-drop. Although this is a great implant for some people, it is not for everyone. Currently, these implants are not widely available in the US as they have not been cleared by the FDA.
There are surgeons who are participating in clinical studies for which you can enroll to be eligible to receive these implants.

Implants under muscle

Yes, it’s very straightforward to place implants under the muscle through a periareolar approach. This creates a small, half circle scar that lies along the border of the areola. Studies are showing, however, that an infra mammary (under the breast) approach may result in a lower risk of capsular contracture.

For this reason I’m recommending the peri areolar approach less and less.

Adjusting for unequal breasts

Most women have some asymmetry of the breast, some though more than others. There are multiple options to improve the symmetry and as you wish to be smaller a reduction of one or both breasts is a consideration.
If you like the smaller breast as it is, a short scar or vertical reduction on the larger breast will help you become more even, and if the smaller breast is too large as well a reduction of both will work out.

Lift or simple breast augmentation

I strongly suspect that a lift would be required, with or without implants. Your best option would be to have a formal assessment with a plastic surgeon. I don’t think that a simple breast augmentation will suffice.

Please discuss your concerns with your experienced surgeon. And follow his/her recommendations. In my practice, I have seen the benefit of early massage, and early range of motion. We like to see implant settling by the sixth to eighth week.
I begin measuring around the fourth week after surgery to track the descent, and quantify what the goal is for reaching symmetry. At three weeks your presentation is common, and with the proper set of dynamics you should see improved settling over the next few weeks.

Natural feeling breast implants

The newest generation of silicone breast implants feels soft, squeezable, and natural. Although saline implants feel a bit less soft and have a bit more rippling, they too can feel natural and soft, depending on the surgical technique used.

Much of how soft a breast implant feels depends on your body type. The more subcutaneous fat you have, the softer the implant will feel. If the implant is placed submuscularly, it is also likely to feel a bit softer and more natural.

If a patient has less tissue coverage (is thinner) or the implants are going to be placed above the muscle then silicone implants would probably be a better choice. The silicone implant will probably look and feel more natural.

You should be aware that although the the FDA has approved the use of saline implants, for cosmetic purposes, for patients of all ages; the FDA has only approved silicone implants, for cosmetic purposes, for patients 22 years of age or older.

Although, there is no scientific evidence that silicone implants pose a greater risk to patients under 22 years of age; due to this fact, some plastic surgeon only offer silicone implants to patients over the age of 22.
