WHAT IMPLANT SIZE?
Selcting the correct implant size is not as easy as it may seem. You should express your desires to your surgeon. Pictures are always helpful. The surgeon will measure your breasts, evaluate your skin quality, and determine an implant size range based on his phsical evaluation. Together you will come up with a size that is best suited for you. A team approach in my opinion is the best.
Bed rest after breast augmentation
While there’s no science that would dictate what the correct answer this question is, each plastic surgeon has their own protocols. The vast majority of my patients, with the technique that I use, can go back to all but the most physical of jobs in 48 hours.
It’s not uncommon for my patients to have a breast augmentation on Friday and be back at work on Monday. Many of my patients will go out to dinner the night after surgery. Certainly ask your chosen plastic surgeon what their recommendations are.
Try to find a board-certified plastic surgeon in your region who has experience in cosmetic breast surgery.
Uneven breasts after augmentation
Although you do show slight asymmetry, you are too early to predict how the implants will settle. The question is: were you symmetrical to start with? The advantage of saline implants is that you can adjust the volume intraoperatively if there is some asymmetry.
At this time I advise that you be patient andd wait about 6-8 months for full healing to take place.
Most likely you need a lift
You will probably need a lift, however the only accurate way to determine that is with a examination.
You may choose to have the augmentation performed at the same time or you can do it at a later time, the safer way is in two separate procedures, the advantages of doing it together is only one recovery time.
There is a remote possibilty you can have only a dual plane augmentation without the lift, however you, most likely, will have lower place nipple/areola complex. Definitively you need to see a Member of the American Society for Aesthetic Plastic Surgery .
Saline Implants
When placed under the muscle saline implants can look and feel natural. If you do not have much breast tissue covering the implant you may eventualy experience some rippling. Silicone implants are just as safe as saline and tend to feel more natural and have a decreased risk for rippling.
Crease incision is preference
25CC will not be a dramatic difference. I use the IMF approach almost exclusively. Using a periareolar approach has it’s own set of issues. With a large implant, it would require a longer incision along the circumference of the areola.
The IMF incision allows the best control of the muscle and where the lower shelf will be.
Breast implant incision
This is a question I asked one of the world authorities in Breast Augmentation in Stockholm, who does around 20 – 40 Breast Augmentations per month (most months), when I trained with him. He has tried all the approaches, and has come back to the infra mammary approach for all the advantages associated with it that are listed in this thread.
Nipples pointing down after Breast Aug, is this normal?
Yes , you have chosen implants that are on the larger size and you started out with very little tissue, it will take time for your breasts to relax and then the implants will fall into position and your breast/nipples will once again be pointing where they need to be, I encourage you to visit with your PS and he can instruct you on ways to encourage the fall of your implants.
Submuscular vs subglandular implants
There are many things to consider when deciding whether to go under or over the muscle. First is the amount of breast tissue and body fat. The more you have the more likely an implant over the muscle will look OK.
Another consideration is life style. Do you do a lot of weight lifting and strenuous upper body activity? If so you may want to consider going over.
Are you doing silicone or saline? Are you concerned about capsular contracture or do you have a family history of breast cancer? If so then going under the muscle would be better.
(mammography is generally easier to perform with unders). I wouldn’t hesitate to go back to your PS and ask questions, or if you don’t feel they are not addressing the issues, seek a second opinion.
Implant size
Breast aesthetics is very individual and subjective. It is important that you define the size and shape YOU want. Most women come into my office knowing they want to be bigger but they haven’t thought beyond that.
You need to think of several things: size, cleavage, “side boob”, a rounder or smoother appearance on the top portion of your breasts, and nipple position. All of these aspects of your breasts can be changed with a particular implant but your natural breast size/shape also dictates what can be achieved as well.
It is difficult to clearly describe your goals verbally so it is best to then communicate your goals to your surgeon with front view, topless photos showing all the features you like and desire. The idea is to find a photo of a pair of breasts that you would love to have on your chest.
Pictures allow your surgeon to precisely know what you are thinking and what you like. Then, a discussion ensues about wether the result is achievable given the natural breasts you are starting with and if so, measurements are done and implant sizers are used to pick the optimal implant both in terms of size, shape and profile.
I tell my patients to not micro-manage the process too much. The most important thing for you to do is figure out exactly what result you are looking for and then trust your surgeon to pick the appropriate implants to achieve your goals.
A Combination of Things Will Work Well!
First, visit with a dermatologist for help with your acne. Second, a combination of breast uplift and augmentation will give you the best look. Although your wide cleavage cannot be completely corrected with a breast implant, improvement is possible.
Fat grafting will also help. Most patients prefer silicone implants and my preference for you would be a textured implant placed under the muscle since a lift is being done. As to the specifics of the lift and the implant type, you really need to have a face-to-face meeting with a plastic surgeon to examine you and discuss the multiple options.
Should the surgeon pr the patient choose implant size?
You are absolutley right, BUT it’s more complicated than that. Some patients change their mind after surgery about what size they want to be. Then they insist on a free re-do to change the size (always bigger) at the physician’s expense if he chose the implants the first time.
An occasional patient will go as far as to hire an attorney to force the surgeon to do the re-do for free! Now you can see the other side of the situation. At Lake Tahoe Plastic Surgery we recognize that patients have no clue how the implant they chose will look inside the body.
It always looks smaller in the body that it does in the bra. The tighter the skin, the more difference it makes. Since it’s our job to give the patient the look they want, it’s our job to adjust the size during surgery to give the patient what they expect.
We have the confidence that we can do that as long as the patient doesn’t change her mind after surgery. And there are those that do.
Breast Augmentation
The results vary from patient to patient. It depends on how patients natural breasts are starting off, if both breast are symmetrical, and type of body frame (height, weight, and Bra size) patient has.
Reducing an implant size by 100cc will make a noticable difference
As a rule of thumb, reducing or increasing the size of a breast implant by 100cc will make a noticeable difference. There is nothing wrong with reducing the size down to say 200cc, yet as the implants become as small as 150cc in size the base diameter and projection may not give the fill needed unless the natural breast and chest size is quite small.The real question you should ask is do you need an implant at all? If you hate the appearance of your breast with the 350cc implant and feel that it makes you look fat, it may be time to get rid of them altogether.
Be cautious about the lift as well unless the nipple is below the fold. The mastopexy scar may not be worth the lift, and you may want to consider if the lift is necessary after you have completed your family.
