Fat transfer breast augmentation can be a good alternative to breast implants.

Nonsurgical breast enhancement is nearly impossible. Some women find that their breasts increase in size after starting on oral birth control pills or gaining weight. Pills and creams offer little to no evidence of breast enhancement.

However, if you are seeking an alternative to breast implants, you may be a good candidate for fat transfer breast augmentation. This is a two-step procedure that involves harvesting fat cells from another part of your body (usually the hips) and transferring the cells to your breasts for a modest increase in bust size.

Fat transfer breast augmentation is best for women seeking a subtle size increase. If you’re interested in a more pronounced augmentation, you should consider the benefits of breast implants, as no alternative can currently produce the same results.

Implant volume and breast cup size

I don’t want to directly answer that question, rather I want to make a comment. High profile implants do not have a large overfill capacity. It seems to me that filling 250cc’s to 325cc is out of range and will lead to rippling and perhaps early implant failure.

Am I too old for breast implants

It would be my opinion, that at 53 as long as you are healthy you would still be a candidate for breast augmentation. From the look of your photos, you have a fair amount of breast tissue, and all implants should look quite natural as long as you keep the implant base width just smaller than your natural base width.
Try to find a board-certified plastic surgeon in your area to consult with.

Differences

All the implant companies have a “gummy bear” implant they are firmer than a regular gel implant. Yes the cost of the “gummy Bear’ implant is higher than a regular implant. They come in round and shaped implants.

The shaped implant have less fullness in the upper part of the breast. The round shaped implants have more fullness in the upper part of the breast. Yes you can request a specific brand of implant form your surgeon.

You can set up a consultation with a Board Certified Plastic Surgeon and decide what would be the best option for you.

You will require both a full breast lift or mastopexy and an augmentation at the same time. You have Grade 3 ptosis which requires anchor scar.
How to decide what style breast implant is best for me.

The dimensions of the implant are the most important. Your surgeon should measure your breast and chest width. Based on this, there are several implants that will fit your frame, including different styles such as high or moderate profile.

Try on the different styles in the sizes that fit your frame and look in a full length mirror, including from the side, to see which you like better. Also put on different types of tops and look.

In my experience, very few first time augmentation patients want high profile implants. Also, high profile implants will tend to stretch your tissues more over time and they project a lot. It is usually a much more “dramatic” look.

Spending the time pre-operatively to select the right implant for you is very important and your plastic surgeon and his/her office staff should be happy to help you with this.
Pain after a breast augmentation

You are absolutely right – pain is a very subjective and individualized “experience.” Many women compare the pain of breast augmentation to the sensation when their milk first came in – of feeing engorged. That being said, in my experience almost all patients are able to go back to a desk job no later than 6 days post-operatively.

To decrease post-operative pain I inject a long acting anesthetic to keep the patient as comfortable as possible. Additionally, in circumstances where patients wish to minimize their use of narcotics I will also prescribe both Celebrex (a cox-2 inhibitor – kind of like a super duper Advil) as well as Lyrica.

Lift with implants

You could do a circumareolar lift with implants and I think you could avoid the T incision altogether.
What breast implants do you think?

Unfortunately it is impossible to recommend a particular size implant without knowing your key breast measurements as well as the appearance you are trying to obtain. If the 535cc implants replicate your desired look, and your surgeon states that they fit your chest dimensions it sounds as if these are the right ones for you.

Is It Safe to Breastfeed After Getting Implants?

Only if she is able too. If incision around areolar than might not be able to breast feed.

You can use gel implants in U.S

You are referring to the gummy bear mplants that are not available in U. S. These implant are more cohesive than the implants available here. There is not much advantage to the gummy bear implants except less rippling.
The implants that avaiable now are safe and are cohesive (not as firm as gummy bear). The other issue is the travel to other countries for surgery and level of protection after surgery. Your implants have warrenty for ten years in u.

If there is concern post op, you have you local plastic surgeon to take care of you. I do not see a reason for you to travel oversee for surgery (this also out sources american jobs) .

For smooth round implants, some surgeons believe that massage can make the implant pocket more natural and help the implants to drop into a more natural position. For anatomic (teardrop) implants, massage is not advisable because it can alter the position of the implants.

Discuss massage with your surgeon for the best answer.
Breast implants placed under the muscle.

SEE VIDEO BELOW FOR GENERAL OVERVIEW. Even if you place the implants under the muscle it is not likely that your ENTIRE implant will be covered by muscle. IF you go under, you will achieve better draping of your implant with less likelihood of stripper boobs, and lower chance of “rock in the sock” deformity, decrease visibility of upper pole ripples, and diminished tendency for capsular contracture.

However, it does not entirely eliminate the possibility for lower and outer rippling particularly when bending over.

Saline or silicone over or under the muscle?

In my opinion, I prefer silicone implants under the muscle. You will always get better mammograms, decrease your chance of a capsular contracture, and have a firmer perkier look for a longer period of time.
Also, I prefer a round , smooth silicone shape. It is still possible for a shaped implant to turn and should this happen, you would need another surgery to correct the problem. a bit.
