Andrew Bonett, MD

After medical school, Dr. Bonett moved to South Carolina and completed residency in General Surgery at Greenville Health System, now known as Prisma Health. As chief resident, he received the Resident Academic Achievement Award which is given to the top resident of the program. Mark D. Walsh, MD, FACS

In addition to his contributions to patient care and resident education, he has also served as an invited reviewer and has provided expert commentaries for multiple surgical journals, including the highly regarded Aesthetic Surgery Journal.

Although it’s difficult to say without an in-person exam, it looks like your nipple position is good, your skin looks healthy, and ID expect you don’t need a lift at all. I also think a smaller implant will fit better in the pocket, as it seems much of you implant volume is in the upper pole.

Worst case scenario? nipples are a bit low after the procedure and you can do a lift later, possibly under local anesthetic even.

You have something we call an “animation deformity”, which is when you can move your implant and breast quite significantly with your pectoralis muscle, wrinkling the skin too.

You’d need a surgical revision to tighten the pocket and release the muscle more. Possibly fat grafting too.

I’d discuss this issue with your plastic surgeon.

It’s difficult to say with just the one photo, but it appears that just implants would be ok. A lot will depend on the look you’re going for (more natural vs. more rounded or “artificial”)

If you’re looking for a DD, you likely don’t need a lift.

It appears you’d best benefit from a lift. If you don’t, you’ll risk getting a waterfall deformity (breast tissue sits below the level of the implant).

One “workaround” in your circumstance would be to use oversized implants, possibly a classic dual plane. But understand that this will leave you with large, low positioned breasts. Also they’d likely be much larger than you like.

Plastic surgery is often a trade-off between scars and shape/form.

Why does my under the muscle silicone breast implant stick out this way?

From the pictures and your description it’d appear you need a revision. The implant size seems to be quite asymmetric. You also have a waterfall deformity, which is when the breast tissue sits below the level of the implant. This may be due to the tuberous breast, or that the implants have not descended to the correct position, or both.

I’d agreed that smaller implants would help. You also would need a release of the lower portion of your pocket, or even potentially a breast lift.
