1. All of the saline filled implants have a specific fill volume range. for example a 325 cc Mentor Moderate Plus implant has a fill volume range of 325 to 380cc. It is advisable to stay within this fill range. You do not want to go below the minimum since this will increase the likelihood of more wrinkles and creases. If you go above the maximum you increase the risk of developing scalloping along the edges of the implant.

2. The more you fill the implant, going to or above the maximum, the rounder the implant becomes. It will also become firmer. This will change the shape and feel of the implant and the breast.

3. An increase of 50 to 60 cc of saline to a 325 cc implant will increase your cup size about 1/3 to 1/2 a cup size. As you see it will make you a bit larger but not a lot larger. Once you know the style and type of implant as well as the fill volume you can determine the amount of additional fluid that can be added. You can go over the maximum but this has its own set of problems.

4. If you desire greater than 1/2 cup size increase then you will need to go to a larger sized implant.

For instance, if you want more symmetry, you will have to recognize that because you have two different breasts now, you will likely have to undergo different types of procedures on the two breasts.

You may need a lift or reduction on the right, depending upon your goals, or an augmentation on both, but with different sized implants, again, depending upon your goals. Having a consultation with a surgeon experienced in dealing with this type of breast issue can help you understand what can be done, and the more you can tell him or her about your specific goals with regard to symmetry, final size of both breasts, shape, and the like, the better he or she can help you.

Your observation of asymmetry and high position is exactly why I hesitated back in the middle 80’s. Whenever I saw a presentation, they were cockeyed and often high. I thought there were inherent procedure difficulties that caused this.

For the athletic female patient who desires breast enhancement with a breast implant, breast implant size is the most important factor and not the breast implant plane (under or over the muscle). Most athletic women want to have a conservative size change to match their slender frame.

A large breast implant placed either over or under the muscle will make it cumbersome during physical activity. I would recommend a silicone breast implant placed under the muscle in a conservative size range. This will provide the enhancement you want with an easy recovery combined with a very natural appearing breast.

What is most natural looking: moderate profile or high profile silicone implants

High profile implant in NC can many times appear unnatural as normally a breast of that volume is not that narrow. It can make you look more fake and hat needs to be balanced by your breast width to start off with.

Can I get a perkier look with implants alone?

I don’t believe you would be “perky” with implants alone. Implants will add weight to your breast and eventually cause more droopiness without some type of lift. I’m sure you don’t want to hear that answer, based on your question, but I wouldn’t want you to be mislead into having a procedure that ultimately won’t accomplish what your after.

North Carolina Breast implant style is less important than skill and judgment of your surgeon

Each surgeon had their favorite and I’m certain you will get good results with both types of implants in North Carolina. You have short fold nipple distances so there is a chance you will have double bubbles as your fold is lowered but these tend to flatten with time.

I would choose whom I like better all other things being equal (fees, revision policy, availability post-op, etc). You’re going to get a good results regardless of which way you go.

Beware the peri-areolar lift for breast that are too long!

The periareolar mastopexy has a very limited application for someone who needs a lift smaller than 3 cm. The implant will also have very little impact on the “lift” of the breast. In order to achieve an aesthetic result, the 1st thing is to create an aesthetic breast and then to augment the breast to the size desired.
In the case of very droopy breast with large areola (as in the photo posed on this question) there is not question a periareolar lift will not provide adequate lift, nor will it create attractive shape.

The periareolar lift has a very high risk of both asymmetry and deformity of the breast as there is so much tension on the areola it causes striations extending from around the areola from the irregular tensions around the areola which causes the pigment to look irregular and the scar to widen differentially around the areola.

It may be difficult to repair the results and likelihood is that you will a second time to under go a full lift in order to correct the deformity and achieve the result you are looking for.
I would recommend reduction of the areola, wise pattern lift (anchor incision) with the augmentation only if you do not like the size of the breast in the bra or would like to assure more superior fullness in the breast mound.
