Breast Augmentation: fitness and excercise and implant placement
Breast Augmentation is one of the most common aesthetic surgeries performed in the world. There are four major decisions that a patient should make in consultation with a board certified plastic surgeon to create the perfect surgical plan.
1. Silicone or Saline Implants.
2. Implant under or over the muscle.
3. Incision location.
4. Implant Size or Goal Cup Size.
In my experience, using a silicone implant under the muscle gives a woman the most natural, sensual, aesthetic result and longest duration of contour and balance.
I have performed this type of augmentation on various women including fitness competitors and athletes. Silicone implants under the muscle should not impede in anyway your active lifestyle or fitness goals, and will give you the most sensual, aesthetic, balanced, and sexy results.
Breast augmentation vs. breast lift
I believe you can achieve a nice result with implants alone. A lift does not appear to be indicated. You have done the correct thing to go on several consults. Choose the surgeon who makes you feel comfortable and who does breast augmentation routinely.
Exercise DOES NOT cause breast implant failure or rupture
Implant Leak / Shell failure is the cause of progressive weakening of the shell.
It is very similar to how bending a piece of thin metal back and forth eventually breaks it. This phenomenon is seen with ALL man made materials including steel which experiences fatigue and stress fractures.
Breast implants can last as long as twenty years or so but ALL breast implants eventually age and leak when their shell weakens and breaks. Since most leaks occur in the areas which are constantly folding in and out, TEXTURED (and Anatomical / tear drop shaped implants) implants (which are stuck to the tissues because of the Velcro-like shell and do not move) and UNDER FILLED implants – tend to leak earlier and more frequently than smooth fully filled breast implants.
If you do not wear a proper bra and have either textured or under-filled saline breast implants – you will be in-folding and out folding the implants much more frequently resulting in much earlier delation.
Otherwise, there is NO REASON why with proper breast support you could not exercise as much as you want to.
Treatment of tuberous breast
Single state correction of tuberous breast (snoopy breast) is a difficult one. It typically will involve a nipple lift and a reasonably sized implant. The potential for complications and a need for revision surgery is quite high.
Make sure to select a doctor who is very experienced in this type of case.
Do you think that I need a lift or will implants under muscle suffice? Scarring scares me!
You may be a good candidate for a breast augmentation without a lift. The larger the implant, the lest chance you would need a lift. I would see a board certified plastic surgeon so that they can exam you and give you further recommendations.
In my opinion the most important consideration regarding size is the patient’s desire. As long as the patient’s goal is surgically achievable and not unreasonale, then we should try to get them there.
Second thoughts about 775cc implants
if you are having second thoughts about your implants, your PS would likely want to be aware as soon as possible and help you choose implants that will achieve the desired result. In my practice the majority of patients choose implants in the 300-450cc range.
However, your PS has your breast dimensions so hopefully they will be able to help you select implants that are appropriate.
BRAVA is not a scam, but its results are limited.
BRAVA has been advertised as a nonsurgical method to enhance breasts, however, it has been described by users as labor intensive and cumbersome with minimal clinical outcomes.
The BRAVA device applies suction to the breasts to promote tissue expansion, and must be worn for 12 hours a day over several months before change is evident.
Breast implants will not compensate when need lift
The photos really help a lot. In my opinion, you definitely need a lift. It is a little hard to determine if you would benefit from a small implant.
It is possible that a lift alone will meet your goals. It is very important that the lift includes not just elevating the nipple but also lifting and repositioning the breast tissue itself. I usually use a technique that includes creating an internal “bra” to lift and maintain the elevated position of the breast tissue.
Profile for a narrow chest.
The basis of your question is what will give me the greatest volume relative to my chest size. The answer for most would be a higher profile breast implant. The second issue is what would suit your body, and that will depend on just what you wish your breast to look like.
Bigger and rounder might be your look, or a compromise might be in order. Take time with your surgeon to understand options and the trade off.

Can I have a pretty breast at 55?
Absolutely you can have BEAUTIFUL breasts at 55! Your breasts are actually quite nice, and will look great with implants. I disagree that in 10 years you will have alot of sagging- depends on what technique you have.
If you have implants placed behind the muscle, you will have a good support mechanism for the weight of your implants, and a fuller, more youthful breast. If you have sub-fascia or sub-glandular placement, you will still have a nice look (if you get silicone gel) but over time the weight of the implant may cause a lower breast/ more drooping over time.
