If you want to look natural, take your surgeon’s advice. Instead of choosing an excessively large implant, tell your surgeon to use the largest implant that will leave you looking natural.

Your surgeon shouldn’t make the final decision on implant size in advance. Once the breast pockets are made, your surgeon should insert disposable implant sizers (they only cost $45), and try different volumes.

From there, the surgeon should place the operating table into a sitting position so that he or she can really judge shape and symmetry and keep different size implants available. If you insist on an inappropriately large size, you may be turned away. This will actually improve the reputation and income of your surgeon in the long run, and it will definitely improve your surgeon’s peace of mind in the short run.

Best Breast Implant Size For You – 5 key elements

For breast implant sizing , it comes down to 5 key elements for the highest patient satisfaction and least revision rate in my experience.

They are listed in the order of importance for my practice:

Patient Choice – The overall goal is to help your patients find the best size for them that looks natural not fake. For most patients, this means a round smooth implant – saline or sillicone – below the pectoralis muscle. I listen carefully to what my patients want.
Surgeon’s Guidance – Our role is to help guide the patient to the correct implant size, shape and type based upon what she wants to look like with and without clothes. This is best done using a set of photos which the patient is requested to bring, or they can go to my website-drrohrich.com, or we provide a set of photos in the office to show all the different types of implant shapes and sizes. This along with the chest wall measurements allow me to give a patient the best advice on what will look best on them . Chest Wall Measurements – This is very important to help guide the patient to the correct size and shape as well as the amount of breast tissue and skin envelope the patient has as this will determine what size will look the best on her long term. This wil also help decide whether the patient would look better with a saline or silcone implant .
Pre- Op Photos – I find this very helpful to gain a true understanding of what the patient means when she says -” I want to be a C Cup “. As we all know, the Cup size is so variable that it is not useful. A set of photos which is from the patient is invaluable in helping both the surgeon and the patient be on the same page for the ultimate sizing for the patient
Intra-Op Sizing/Refinments – This is this final refinment needed intraop to detect any subtle dicrepancies to acheive the best result for the patient . I routinely place sizers and sit the patient upright intraop . Although, I has already used the correct size of implant in over 90% of patients intraop without this final technique, this does help further define a good result . (Rod J. The most important thing we as physicians can do for patients like yourself is match the implant size with your body’s proportions.

There are several measurements we take, in addition to looking at your breast tissue to determine what size would be ideal for you. The artificial look in the upper pole of the breast can be caused by multiple things: too large an implant, an implant on top of the muscle, or in some cases a shaped implant. Choosing the correct breast implant size

There is no easy way to choose the correct breast implant in Dominican Republic. The size of the implant should be customized to your body. This is the art and science of plastic surgery. A responsible plastic surgeon should take measurement of your breast and match it to your desires. Having said that, we can’t always get what we want. Imagine going to the shoe store and wanting to get a size 7, but your size is 9 or vice versa.

Dominican Republic Breast implants sizing is very similar. It has to fit you and your body. To accomplish all of the above, I ask my patients to bring photos of their desired breast. While in the office, I spent a lot of time listening and educating them on breast implant size issues.

I take the time to measure the breast dimension and tissue quality. The patients are then given breast implant sizer to try on with a bra. The correct size is usually between the recommended size based on measured dimension and the sizer the patient likes the most. This is a range. It also gives me an idea of my patient’s desires and what fits in her body.

I build relationships of trust and understanding with my patient. Knowing ideal implant based on tissue dimension, the patient desires, and the comfort range of sizes discussed in the office, I proceed to the operating room. The final decision is made while in surgery as I try on different sizes while the patient is asleep. The best way to get more individualized information is to consult with a plastic surgeon certified by the American Board of Plastic Surgery.

Members of the American Society of Plastic Surgeons are also board certified. A cosmetic surgeon is not necessarily a plastic surgeon.

Breast implant size – how to choose and be sure

It’s called the “mirror test”. At the initial consultation, I spend a lot of time with my patients in helping them select the implant size that is most likely to make them happy.

I have them put on a non-padded sports bra and try on different implant sizers (air-filled implants) while looking in the mirror.

Once they try on a sizer that makes them say “that’s what I want to look like” then I know exactly what volume of implant to use. The patient can return to my office and re-do the test with my nurse as often as she likes before the surgery to confirm her choice or to choose a different size.

It is not certain where the you have to replace your saline breast implants every 10 years come from. The warranty used to be 10 years, but now the warranty for silicone is lifetime.

If you are not having a problem, there is no need to replace them. Don’t fix what isn’t broken. Having said that, it would be silly to claim that you will never need surgery on your breasts again after having implants.

How long do saline breast implants last ?

How long do implants last, is a great question and an area of confusion for many. It is important to understand that as stated by the breast implant manufacturers, and as seen in practice, saline breast implants are not meant to be “lifetime devices.”

A very crude estimate of implant failure is about 1% per year in the early years after implantation and that number will climb the longer the implant is in place.

Newer large scale studies have implant rupture or failure rate slightly lower than that for silicone and slightly higher than that for saline. These numbers really don’t help people much.I started practice in 1996, and have placed thousands of saline breast implants in Dominican Republic over the years.

I have seen perhaps a handful of deflations from my patients over the years, understanding another number of deflations being handled by other surgeons. I generally tell patients that they can expect saline implants to last 15 to 20 years, however they may may last for less time or longer than those numbers.

A saline implant deflation is generally very easy to detect with the breast losing its size and shape with obvious difference from the opposite breast, as the saline is harmlessly absorbed by the body. There is no need for MRI or mammogram in this instance and replacement should be performed. It may be wise to simply change both implants at this point if they were placed many years earlier.

Although no emergency exists at all to replace, it is wise to replace the ruptured implant at least within a couple of months, as the capsule and tissues will shrink now that the implant is no longer full in place.

Replacing a deflated implant after an extended period of time, will result in a tighter space and may have more tendancy for a period of asymmetry between the two breasts, especially if larger implants were used.

A bit of a different story for silicone implants. Those implants that were placed prior to the moratorium in the mid 1990’s were made of a more liquid silicone. When they rupture, there is a tendancy for a change in shape, quality and an increase incidence of firmness or capsular contracture and even leakage outside the capsule into local tissues and lymph nodes. Although large scale studies have shown no incidence of illness or disease, there is still often undesirable local tissue changes that can occur.

On the flip side of that, i have seen ruptured old silicone gel implants with no appreciable breast or soft tissue changes. Ruptured silicone gel implants can generally be determined by MRI and sometimes mammogram.

These studies are not fool proof as i have seen studies say the implant is ruptured when it is not, and i have seen studies report the implant is intact when it is not. As a general rule, for the liquid type silicone implants placed prior to 2000, I would say from my experience in removing and exchanging many, that they last at maximum from ten to fifteen years.

There are some surgeons that say if your not having any problems with these implants even after this time, leave them alone. Others say to replace these older silicone implants after this time. In my opinion, unless perhaps in an elderly woman with no problems, i would recommend exchanging these older silicone gel implants after that period of time.

The newer generation of Cohesive silicone-gel breast implants in Dominican Republic, first approved in the US in 2006 is made of a semi-solid gel that reduces the occurrences of silicone leakage or silicone gel bleed and of the migration of the silicone from the implant pocket to local tissues outside the implant pocket, or capsule. Of the thousands of these new cohesive silicone gel implants that i have placed, i have not had one come back with a rupture. Certainly, there could be instances of a rupture cared for by another surgeon. As we have only been implanting these newer cohesive gel implants since 2006, the jury is still out on just how long they will last, on average, for the general implanted population. Hopefully, these new and improved implants will last longer. As it is highly unlikey to be able to determine a ruptured cohesive silicone gel implant on physical exam, MRI studies are recommended for routine interval follow up.
