Philadelphia Implants do not have to be replaced every ten years.

The good news is – implants do not have to be replaced every ten years. We expect implants to last longer than that although of course the exact life of each set of implants cannot be guaranteed.

however the implant companies believe so strongly in their products that if the implants do break the companies will replace the implants. Depending on the company and the type of warranty that might have been purchased at the time of the original surgery there may also be some compensation for the Operating room costs as well.

That said – how do you know if there is a problem with your implants? Some women experience a change in size or feel of the breast. However the best way to examine the implants is by MRI – currently the FDA has recommended a MRI every three years for a women with silicone implants .

Above or below pectoralis muscle for breast implants – Submuscular implants

Breast augmentation in Philadelphia should generally be made under the pectoralis muscle to decrease descent of the implant, capsular contracture, and palpability of the implant. There are only a few cases that necessitate subglandular breast implant placement.

Is 50 too old for Implants?

50 is absolutely not too old for implants! Many of my patients are between the ages of 40 and 60, and they are thrilled with the results! I recommend seeing a board certified plastic surgeon for a consultation.

Alternatives to breast implants in Philadelphia

Fat injection is an alternative technique. The problem is that it can cause micro calcification in the breast where it is injected. The radiologists say that they can tell the difference between that and the calcification caused by breast cancer.

I do fat injection for revision of breast reconstruction especially in cases where the area was radiated. There is some evidence that the stem cells in the fat could revers some of the radiation changes.

I don’t like to experiment on patients. Once it is proven that the fat injection for breast augmentation is safe I will offer it to my patients who desire a small augmentation.

The other alternative in cases where the breasts are droopy is to do a breast lift which can give the impression of a bigger breast.

Rippling uncommon with reasonable implants

Although I am also concerned about rippling, I do not think you have much to worry about. You have reasonable goals and have selected silicone implants in Philadelphia. It is unlikely that you will notice rippling based on the info you provided.

Patients who have very thin coverage (A cup) and want excessively large implants (D cup) are much more likely to have issues with rippling. The only piece of advice I have to offer is that I suggest you place the implants below the muscle to further decrease the chance of ripples.

You should be fine.

Lift with or without implants

I think you need a lift in order to give your breasts a better shape. My guess is that you will also want an implant, even if just a modest size, in order to give you some additional volume that you speak about.

I realize that there are additional scars but if the surgery is done well the scars should fade into the background. If your breasts look sexy you won’t mind the scars.

Too Old for Augmentation?

General health and wellness, rather than age, are most important when considering whether or not a patient is an appropriate candidate for breast augmentation or a face lift. I would encourage you to seek consultation with a board certified plastic surgeon to discuss your concerns and develop a plan that will allow you to achieve your desired cosmetic results.

Breast lift is needed

Your photos suggest there is too much droopiness (ptosis) and too much skin laxity for an implant alone to work for you. Some patients and surgeons try to get away with placing large implants in Philadelphia, but this only creates large, droopy breasts in the long run and can make the lift operation much more difficult when it is done eventually.

Breast implants after reduction

Philadelphia Breast implants after a breast reduction are not uncommon in patients who lost a lot of weight after surgery. You should see a plastic surgeon to evaluate your anatomy and give you options of different types of implants.

Implant profile

It appears that a high profile implant would be best. However, I have seen many patients who look great in a bra but not as good when examined critically without clothing.

Also, remember that a high profile implant (especially if size is larger) exerts pressure against the breast tissue more than a moderate profile implant. This can result in thinning of breast tissue with time. The photos you see are a reflection of an implanted breast at one point in time.

You have no idea how many revisions they underwent or will undergo in the future with oversized and highly projecting implants in Philadelphia.

I always recommend patients to have an implant that fits their breast parameters. You can certainly have a high profile implant if it falls within these parameters.

How Long Should I Wait to Buy New Bras After Breast Implants?

4 to 6 weeks out after breast augmentation surgery is a good time to buy a couple of new bras. Don’t try to replace all of your bras all at once as there will still be some changes over the next several weeks.

Smoking cessation and breast augmentation

As you have seen from the responses, there is much variability as to how long you need to quit smoking before surgery. Smoking is a concern for two reasons — one is related to its effect on wound healing (decreasing blood flow), and the other is related to its effects on your lungs (increasing mucus production and airway reactivity).

For surgeries with large skin flaps (such as facelifts, tummy tucks, and breast lifts), many surgeons will advise that you quit smoking 4-6 wks prior to surgery in Philadelphia.

For a breast augmentation, whether a surgeon will insist that you quit smoking may vary; and I would suggest that you discuss this issue with your plastic surgeon.

I have spoken with several of my friends who are anesthesiologists. Most of them have told me that if a patient is going to quit smoking prior to surgery, that patient should quit at least 3 months prior to surgery to minimize pulmonary complications.

Once again, I would suggest that you discuss these issues with your surgeon.

Tubular breast deformity and asymmetry

Breast asymmetry is incredibly difficult to treat, and you should just keep in mind that your breasts will probably never be perfectly symmetric. One or both breasts may sag at different rates over the next few years, and it’s impossible to predict this.

The best thing to hope for is to have symmetric breasts in clothing, which have the same size and shape.

Breast Deformity

Puffy nipples are often associated with tuberous or constricted breasts, although photos would help to determine this. Tuberous or constricted breasts can be droopy even though the breasts are small, and tend to have a tight, flat lower portion of the breast instead of a more rounded contour.

It’s likely that an experienced board certified plastic surgeon will be able to help you, but I can’t give more specifics without the photos.

I want implants but not a lift; is that a possibility?

Your breast shape will not be pretty if you do implants alone without a lift. You have too much sag and your nipples sit too low on your breasts to elevate with only an implant.

Breat enhancement with and without mastopexy (lift)

Your photo suggests that you would certainly benefit from the addition of volume in the form of a breast augmentation. This will improve the balance between breast volume and skin envelope, but your nipples may still be a little low.

A lift (mastopexy) at the same time would be appropriate, but you might want to save money and save the extra scars for now and just have the augmentation: wait a few month and see how you feel about your new look and have a lift later if you want one.

This gives you some control over what happens and you may very well be happy with volume alone. If you are convinced you want one surgery only and feel that you will not be happy without the very best result then you would choose the lift at the same time.

Be sure to see a surgeon certified by the American Board of Plastic Surgery who has a fair amount of experience in this area. Review his/her portfolio of pre- and post-op pictures so you can see the types of results that are reasonable to expect.

Planning ahead for a wedding is very important as you need to ensure you have chosen the appropriate dress size. Therefore, you should move forward with the breast augmentation as soon as possible.

Volume, projection, diameter, and your personal preferences

The size you should be choosing by trying on sizers. This will give you the best estimate of what it is going to look like. Then when you have chosen what you think to be the best size for you, the surgeon can make suggestions based on your breast dimensions.

There are pros and cons for each approach:

1. Below the muscle (or dual plane):

Better implant visualization on mammogram
Better protection for the implant
Better used in very thin patients with little breast tissue
Better used in saline impants due to future rippling

2. Above the muscle:

More natural result specialy with a gel implant .

Having breast augmentation will not change your personal breast cancer risk

Many women seek cosmetic breast augmentation, and some of these women will also have a history of breast cancer in their families. I counsel ALL women to know their family history of both breast cancer and ovarian cancer prior to considering any surgery on their breasts.

When appropriate, I refer women with a significant family history of these types of cancer (particularly before menopause) to a breast cancer specialist or a genetic counselor for further workup. If women are older than age 40, I recommend they have a screening mammogram prior to any breast surgery – just for future reference.

However, having breast implants placed will NOT change the risk of developing breast cancer. In fact, two studies in the literature found that women with breast implants who developed a breast lump, found it slightly earlier than those who did not have breast implants.

This is not to say that breast implants protect against breast cancer – they merely perhaps enabled the lump to be felt earlier. Implant placement above or below the muscle also makes no difference.

Two different size implants

Your breast volume, and the envelope and shape are different. The amount of asymmetry you find acceptable is a personal decision, and the size of the implant to acheive the result you want. If the implant is quite large, a very significant percent of overall volume, then it is possible the same size will do on each side, If not though better attention to differing implant shape and size relative to the breast may give a better result.

Tear drop vs round implants

The choice of having a tear drop or a round implants depends on your preference and your surgeon experience. If you are looking for fullness on top of your breast or you have lost fullness you are better with a rounded implant.

I would say that you can have a “fake look” only with round implant but you can have a natural look with either a tear drop or a round implant.

Round implant does not mean round breast at the end. All depends by anatomical features and implant size and position of the implant.

You can have a very natural look with a small implant behind the muscle. Speak with your surgeon and find the best solution for you.

Silicone implants placed under the muscle provide the most natural feel and appearance. At your consultation you can try on different implants and see what best works for you.
