You're thinking is correct. The different gels that natrelle implants are available in can theoretically change the amount of upper pole fullness as shown on their website (link included). But this would likely be much more visible in either a subglandular (on top of the muscle) breast augmentation or a breast reconstruction after mastectomy. Implant sit against your chest wall in their own pocket. If the pocket is soft and doesn't have thick scar tissue (capsular contracture), then the implants can move on your chest wall just like your breast tissue can move as you change positions. This is normal and nothing to worry. A pregnancy will likely reverse a lot of the positive changes a tummy tuck can offer. A tummy tuck removes excess skin and if someone became pregnant after a tummy tuck the skin would likely stretch out again, causing another tummy tuck to be needed. Additionally, pregnancy would likely stretch out the abdominal muscles causing the "hourglass" results of a tummy tuck to go away. Judging by your pictures, you probably have excess skin going all the way around your torso, and a normal tummy tuck wouldn't address this. Mini tummy tucks are often used in very select patients who only have loose skin below the belly button. They sacrifice the best results for a smaller scar. Additionally since they don't relocate your belly button, your belly button gets pulled down which can make you look older and less in shape.

When an area heals over and then closes back up only to open up again there is usually an underlying cause of chronic inflammation in the area, such as a fluid collection, non-dissolved suture, or permanent suture which is colonized with bacteria. I have seen this a couple times and the only way to deal with it after you are this far out from surgery is to explore the area and try to remove the cause of the chronic inflammation.

You likely have a chronic seroma cavity. This means scar tissue formed around the seroma area and it will stay that way indefinitely. There is little to no risk in leaving it alone. Theoretically it could get infected but that is HIGHLY unlikely. Your risk would be a lot higher trying to remove it because you could have wound problems, infection, anesthesia complications, a whole slew of things. If your surgeon recommended against removing it and it doesn't bother you that much I would agree...

Although you are very thin and in shape, you have excess skin and a scar from your children. The only way to effectively address the excess skin and fix the scar/shelf at the same time is a tummy tuck. Although the scar would be longer than the c-section scar, a tummy tuck isn't that invasive of a procedure, most patients at our practice are back to work within the week and just have to be careful with how much and how heavy of lifting they do for a while.

It sounds like you already did this, but I need to mention that you need to be explicit with your surgeon. You need to state the things that bother you and what the specific areas are and why. Then once he knows exactly what you are thinking you can both discuss options. If after your surgeon is on the same page as you regarding what bothers you, they cannot come up with an option which you feel will meet your goals, then seek a second opinion.

You likely won't have enough tissue to have what most insurance companies would consider a breast reduction, so to address your rashes in the fold under your breast, a breast lift or "mastopexy" would be a great option. Having a lift could raise the mound of your breast on the chest wall and reduce the amount of "fold" you have under your breast to help decrease the amount of rashes and irritation you are getting.

I would recommend getting a few opinions from different plastic surgeons for how you could best tackle this problem. You could definitely have the areolas re-sized and have the area which was necrotic addressed, and it may be possible to adjust the height of the nipples on the breast mound. Having a breast lift to change the shape of the breast mound, and therefore where the nipple sits on the mound may be your best option.
