Breast implants in Hawaii – when to do them.
If you are considering breast augmentation and pregnancy in the same sentence then I would advise you to have your children first and then augmentation.
I like to refer to augmentation as a pseudo pregnancy because the breast enlarge with pregnancy.
The skin of the breast can stretch considerably and this can result in skin stretching and breast distortion. If you were to get pregnant with implants already in place, then your skin would stretch more and the stretching and stretch marks would be exaggerated.
The likely-hood of needing a breast lift following a pregnancy is that much greater. In my experience, most woman lose breast tissue following pregnancy.
This is due to the breast being engorged with milk and the extra pressure cause the breast tissue to atrophy (become thinner).
Now if you are going to wait a few years before having children, then I would take my chances and have the surgery in Hawaii. There is no need to waste a few good years of looking great.
I would recommend a Breast Lift with your implants because it will give an overall better result. Yes, you can just do the implant, but they may sag. It also depends on what you are looking for.
If you just want an implant and the lift isn’t your concern, just do the implant. However, I recommend a lift with the implants.
It is hard to say exactly how your breast will look once it drops, but it will take on a more natural contour, with a fullness in the lower portion.
LOOK VS. FEEL
Saline or silicone implants in Hawaii are intrinsically not more or less natural-looking. It is more the feel of the implanted breast that is more natural with silicone when it functions as designed. The look is more related to your natural breast shape and size and the choice of size, dimension, and style, as well as position of the implant(above or below the muscle) rather than the choice of filler material.
If you have a good fit between your breasts and the implant, you can look very natural, or not unnatural, using saline implants. However, the feel might be less natural than the same silicone implants.
It then becomes a trade-off between your comfort with saline or silicone, and the ned to feel more or less natural. And of course, you might not know what they feel like until they are actually in your body.
Implants and asymmetry
If there is not a large difference between the two breasts, and one is having large implants placed, using the same size implants sounds reasonable. Not two breasts are exactly the same.
Breast Augmentation and Lift
For the best result, you would benefit from both a breast augmentation with implants in Hawaii to increase the fullness at the top of your breast, as well as a lift to help bring your nipples to a higher position and tighten the breast skin for a more youthful look.
If you feel that your breasts are already a good size, you can get a breast lift alone. Having a breast augmentation alone may make your breasts appear heavier and is not recommended. It is best to have an in-office consultation to take measurements and evaluate your tissues so determine the best size implant for your body.
For Breast Augmentation Use a More Locally Placed Incision Than The Tummy Tuck
The key to a good breast augmentation is control of the inframammary fold and pocket dissection. While it may be possible to place breast implants through the tummy tuck incision, this is not any easy way to achieve those requirements.
Using a high-lateral tension abdominoplasty approach, dissection in the upper abdomen is confined primarily to the central one third, and so the area inferior to each breast is not dissected.
And from an accessibility standpoint, the breast is farther away from the level of dissection in the upper abdomen than you might think.
The inframammary fold incision allows the surgeon to control the level of the fold, provides good exposure for either a subglandular or subpectoral pocket, avoids the contaminated area of the nipple-areolar complex, heals well, is reusable for future operations, and is well hidden in the inframammary crease.
A properly positioned implant, and subsequent happy patient, will more than compensate for the small amount of extra scarring.
Breast lift?
I have little question that you absolutely need a breast lift. Unfortunately, you have a bit too much drooping I’m loss of volume for augmentation alone. Unfortunately in situations like this simply doing augmentation Will give unacceptable results with the breast in breast tissue spilling over the implants.
Depends on the Size Change
if you want to be reasonably larger, adding volume will most likely NOT be beneficial. Adding volume makes the implant “larger” by making them project more, but narrows the base width. usually changing the devices to another size is better.
If your implants are not filled to the maximum recommended volume, more can be added, but agin usually less than 50cc which may not make a substantial difference. Think of 50cc out of 350cc, not a big addition
Size change and breast revision
the volume changes with chest size. see my previous posts. a 32 usually requires about 250=-275 to get to a c cup 34 300-350 36 350-390 it may be possible to change the size and fix the pocket internally.
I am a petite 5’3″, 118 lbs with chest size 32A. My surgeon recommended 304cc. I am afraid they are too big for me
I tend to be conservative in my size recommendations but must say that an implant of 304 mL is pretty close to the average recommendation.
I personally choose to select the final implant during the surgical procedure using test implants called sizers, and for someone with your proportions I would probably be planning to use between 260 and 320 mL to achieve a C cup.
Partly this depends upon the distensability and amount of skin laxity that is present and the individual patient’s goals. It does not seem to me however that a 304 mL implant is unreasonable for your frame and I would point out to you that many patients who are naturally conservative later wonder if they should have selected a slightly larger implant.
Discuss your concerns in detail with your surgeon before the procedure and then let him/her do their best work and enjoy your results. Remember it is not just size but contour and shape that matter.
Breast deformity
This deformity has the appearance of a ring or tight tissue around the nipple that prevents the breast from expanding normally. The breast tissue protrudes through the areola as it forms and enlarges.
To fix it, you may need a surgery in HI that involves an incision around the areola and placing an implant to expand the skin of the breast.
Breast Ptosis and Implants
Regnault famously classified breasts Ptosis according to Nipple position and the lower breast pole. Classically when the Nipple is below the lower pole a mastopexy lift is needed for the best results albeit there are those that in mild circumstances such as yours offer very large implants to inflate the Breast enevelope.
However this is not a good option for long term aesthetic considerations.

I think you will look great with an augmentation. No lift will be necessary. Your board certified plastic surgeon will help guide you to achieve your desired look.
Moderate Plus Vs High Profile Silicone Gel Implants
There is definitely a difference in moderate plus compared to high profile implants, although sometimes it’s hard to tell just from the photos. High profile implants give a more dramatic result, with more fullness at the top part of the breast (upper pole).
They’re shaped differently from moderate plus implants, with a narrower base that gives the more pronounced forward projection. Younger women often prefer the high profile implants, as they look more like a natural youthful breast, while the moderate plus profile implants result in a more natural shape for older women.
The bottom line is that results look different on different bodies. You may love the look of high profile implants on other women, yet on yourself the moderate plus choice could be better. An excellent surgeon can help you make the right choice.
Breast reduction
Your left breast is larger and lower than your right, which is true for 80% of women. At the least, you will need a reduction of the left breast. Depending what a B cup means to you, you might need to have breast reduction for both breasts.
You need to establish this with your surgeon. This will dictate the fee, depending on whether you do one breast of both. I doubt that your insurance will cover this for you. This is an outpatient procedure.
There will be scars. The result may not be perfect. Please consult a board certified plastic surgeon to get all the information.
My doctor wants me to have 350 cc, but I want 450 cc. Am I allowed to refuse his recommendation and get the size I want?
You should certainly discuss your concerns and desires with your surgeon. With an elective aesthetic procedure such as a breast augmentation, or any procedure for that matter, a trust and understanding is certainly required from both parties.
Your surgeon should also serve to inform you of the risks and benefits of the procedure as well as help YOU to decide which is the right decision for YOU after discussion of everything that s/he can offer.
It is your decision to decide who you will enter that agreement with, and vice versa. If you do not feel comfortable, consult with others and choose the one you feel most comfortable with who will meet your expectations.
Ruptured implants should be removed and/or replaced.
Remove the implants with the capsules and consider replacement with saline versus silicone-filled implants if you wish. As long as the skin/breast tissue integrity is OK, this should be possible. You need to be properly evaluated by a qualified board certified plastic surgeon to get this sorted out.
Uneven breasts
The short answer to your question is that in most cases silicone will give you a better result that saline. However, you have a more complex situation. Your asymmetry is not the slight unevenness that can be corrected by slightly different size implants.
You will likely need a breast lift on your larger breast in conjunction with placement of the larger implant on the small side. You need to see a surgeon with experience with these type of issues.
Risks Associated with Breast Augmentation
As with any type of surgical procedure, certain risks accompany breast augmentation surgery. It is important to know the risks and discuss them with your doctor so you are prepared in the event a complication occurs.
Side effects such as tenderness, swelling and bruising are normal, but excessive swelling, reddness, vomiting or fever over 101 degrees is of concern and requires immediate medical attention. These are possible signs of infection. To prevent this from occuring, you’re doctor takes extra precautions during surgery and also prescribes antibiotics to take after breast implants surgery in Hawaii.
Other risks include: Rippling – Rippling typically occurs with saline breast implants, particularly when placed over the muscle. This condition is when the edges of the implant are seen under the skin. To reduce this risk, Hawaii implants are placed under the muscle or half under/half over the muscle.
Capsular Contracture – a condition where the protective tissue that is formed around the implant tightens around the implant causing distortion and pain. To help prevent this, your surgeon will create a pocket slightly larger than the implant to allow it extra space.
This also provides extra space incase excess tissue forms. Breast augmentation massage is used to move the implant around in the pocket to prevent this excess tissue from forming, which also keeps breasts soft and natural looking.
Rupture/ Leaking Implants – A very common concern among women is “what happens if my implant leaks?” With saline implants, the affects are quite noticable because the implant will deflate and the saline, which is salt water, will be purged from the body.
Detecting a rupture with silicone implants in Hawaii is a bit more tricky. Because the implant is made in a similar fashion to a gummy bear the contents of the implant do not leak out into they body, however it is important to have it removed and replaced as soon as possible because it may lead to infection.
When an implant ruptures, some patients have reported breast pain, swelling, fever, joint pain and fatigue. If you experience any of these symptoms or notice that your saline implant has deflated, you will need to call your surgeon right away.
The feel is real!
Augmented breasts should look and feel nearly similar to real breasts. Especially if that is the goal of your augmentation. Your surgeon should be able to council you on the best options for implant type (silicone), size range (moderate) and placement (under the muscle) to give you a very natural enhancement.
Keep in mind, this is not every patient’s goal and this is why you will sometimes see unnatural looking augmentations. It’s not as common now, as the “tear drop shape” natural look is trending, but I still have some patients request a very high round “Barbie” breasts! Make sure and look through your surgeon’s before and after’s to see a range of outcomes on different size and shaped patients that show he gets consistent, but different looks for his patients.
This shows they are dedicated to achieving what YOU want and are not focused on a certain aesthetic. If breasts are ever hard after an augmentation, this is a sign of capsular contracture and not common.
It indicates the scar tissue capsule around the implant is squeezing it. This would be a reason a patient should be having a revision replacement surgery.
Breast Shape
A breast reduction is indicated to make the breasts smaller, lift the areola and make them smaller as well. The tough part is providing long term fullness in the upper pole.
I have found that leaving breast tissue on the chest wall and shifting it upwards can give long term improvement without an implant.
Large gaps after breast implants in Hawaii
There are two main reasons for gaps between the breasts after implant placement. The first is the patient’s anatomy. If the breasts are wide apart to start with, then after the implants are placed, the width may appear larger.
Second, is the implant placement. If the implant is placed to laterally (far apart), then the breasts may appear to have a large gap between them.
High Profile vs Moderate+ Implants for “Natural Looking” Breasts
Although I use Moderate+ or Moderate profile prosthesis in the vast majority of cases, given your base width dimension of 13.0cm and your wanting a 500cc implant, a high profile implant would probably be appropriate.
That said, without any photos of yourself and the look you desire, it’s impossible to say for certain. While the size of the prosthesis does play a role in whether or not you achieve a ‘natural’ or ‘unnatural’ look, the surgical technique used in relation to the pre-operative breast appearance is also closely related.
That said, there are many women who like the look of round, projecting breasts with upper pole fullness (high cleavage) – neither look is better than the other, just different. Every surgeon has a certain aesthetic they prefer, but at the end of the day, if the patient wants a certain look, it’s my job to achieve it (within reason of course).
And while I now almost universally use moderate or moderate plus prostheses (~10% and ~80% respectively), given your base width dimension of 13.0cm and your wanting 500cc of added volume, high profile implants in HI (which I use ~10% of the time) would be appropriate.
If correctly performed, you should be able to achieve your desired result.
If the original scar tissue capsule remains the recovery from second surgery can be relatively painless.
Replacing implants in the same plane, such as under the muscle, after they have been removed for some time may be quite easy if the original scar tissue capsule remains. There is a natural production of scar tissue around all implants that completely encircles the implant.
If this capsule is still there when your surgeon goes to reinsert your implants the procedure can go quite easily and the recovery much less painful then the first time. This is because the surgeon does not have to dissect a new space since it is still present from your original operation maintained by this scar tissue capsule.
If however the space has instead collapsed and scarred down then more dissection is needed to establish a new space and this will lead to a more uncomfortable recovery. You will have to see how this plays out.
