This is a great question and a lot of patients tell me that they won't plan on wearing bikinis in the future. My response to this is that I want them to feel comfortable wearing one if they want to. There are two key factors that can provide this confidence: a low transverse scar and a hidden belly button scar.

Preoperative deflation is a very useful tool prior to revision augmentation. It not only helps give a better idea if a lift is needed but also allows the tissue to rebound a little without the weight of the implant. Based on your photos a lift will be necessary. If the capsule is indeed "crunchy" or calcified it should be removed as well.

You can definitely do a second stage of fat grafting to fine tune your original result. Make sure to discuss the areas that bother you to ensure you get the result you're looking for. And see a board certified plastic surgeon.

Using a laser has few benefits from traditional labiaplasty techniques. It's just another way to cut the skin. It's used as a marketing technique. Ask to see before and afters and discuss your specific concerns. Many different fields offer labiaplasty - Ob/Gyn, Urology, Plastic Surgeons. The key is to find someone that has the proper training and experience. Cost of traditional labiaplasty will vary by location and experience. Average in the midwest is likely around 3500-5000.

You can likely proceed safely but I would put you on a prophylactic anticoagulant like Eliquis or Pradaxa for the first week post op. This has been shown to be safe at reducing DVT and minimizing bleeding complications. Make sure to discuss with your plastic surgeon. Abdominoplasties do have a higher risk of DVT compared to many other types of plastic surgery procedures.

Drains help remove excess fluid and prevent potential wound healing issues. They also help the tissues stick down to assist in the healing process. Many surgeons are going to "drainless" abdominoplasties and using sutures and glue to try to take their place. Both are reasonable options. Muscle repair is beneficial if the muscles are separated for if the fascia is extremely loose. Based on your photo you may not need the muscle repair. Based on this one photo I would recommend getting a tummy tuck with your BBL. You will definitely have loose skin afterwards and you would likely benefit from tightening your muscles. 
Consult with a plastic surgeon to have a full exam to make sure you're making the right choice.

Submuscular implant position is limited by the pectoralis muscle insertion. I always point this out to patient preop and discuss their anatomical limitations. Your options include:Breast revision and additional release of medial pec muscle. This would likely make a small improvement because you can't only release it so much. Fat grafting to fill in the void. Overall I think you have a nice result. Fat grafting could help with minimal downtime/recovery.

I would choose your surgery based on your final goals, not the final scars. Don't go bigger than you really want to be to avoid the lift. You'd likely have the best outcome in terms of shape, size and nipple positions with a lollipop lift and implant. An implant alone will likely leave your nipple slightly low but it would fill your upper pole. Talk this through with your surgeon and consider a second opinion.
