Are Sientra textured implants linked to ALCL?
Sientra Textured implants and BIA-ALCL
Aug 9, 2019
BIA-ALCL and SIentra Implants

On July 24, 2019 The FDA released a safety communication to update the current understanding of BIA-ALCL and called for the voluntary recall of the higher risk devices. This referred to the more aggressively textured Allergan BIOCELL implants. This recall did not include the textured devices from Sientra or Mentor

The FDA has been made aware of 573 cases worldwide of Breast implant associated anaplastic large cell...

Firmness around incision lines under breast is much more firm at 5 weeks. Surgeon says it's scar tiss
scar tissue after surgery
Jul 31, 2019
It is difficult to give you a personal opinion without a formal visit in the office. But Scars typically become thicker and redder for 6 weeks after surgery and then begin to fade and improve for 2 years or longe. . I have my patients use micropore tape for the first 6 weeks with aquaphor being used on any open areas, but once everything is healed I recommend that they start silicone applied topically twice a day, such as Pro-sil.( available online). Silicone strips or...
Choosing the right size IDEAL implant for BWD 14cm - is bigger better? Or will it increase complicati
Finding the ideal size of the IDEAL implant/ how important is the base width?
Apr 15, 2019
There are many different factors to consider when sizing an implant. You want to be full enough to meet your goals in and out of clothing. Many patients will wish they had gone larger after they get used to a particular size. But as the implant becomes larger and heavier there can be more "stretch" issues from the weight of the implant. Think back to any friends that had large breasts when they were younger- they have drooped by now. Some people say that a...
How common really is silent rupture?
Pros and cons of Ideal Implant vs silicone.
Apr 15, 2019
This is a great question and you have to weigh the tradeoffs for yourself. All implants will fail eventually. ALL IMPLANTS. When a saline filled implant fails, it gets visibly smaller. You don't see an external change with silicone implants so the rupture is "silent". Over many years that silicone gel from a ruptured implant can irritate the surrounding tissues. The risk of rupture in the 8 year data shows: Allergan gel 7.4 %, Mentor gel 13.6 % and...
Do most doctors re-pierce the ear after ear lobe repair from elongated piercings?
repiercing after earlobe repair
Feb 21, 2019
I normally offer to re-pierce the earlobes that I repair. I prefer to wait 3 months after the repair to allow the typical scars to begin to soften before piercing. But anyone that is qualified to pierce an ear can do this for you after the appropriate time has passed.
Can my ears look normal again before my wedding in October?
Repairing Gauges before wedding- how long to look normal?
Feb 21, 2019
Your earlobes can be repaired under local in the office in a procedure that will take approximately 1 hour. I prefer to use dissolvable sutures that fall out by themselves. The scars get redder and thicker for 6 weeks and then start to soften and improve. I typically recommend waiting 3 months to re-pierce the ears. After re-piercing I have my patients cleanse the new sites with hydrogen peroxide twice a day for 3-4 weeks before changing their earrings. I would...
Are Ideal implants too good to be true?
Ideal Implant: too good to be true?
Feb 16, 2019
The ideal implant is a great choice for many women. It is not quite as soft as a traditional silicone gel implant so some patients with thinner tissues may prioritize the softer feel of the silicone gel and decide that this benefit outweighs silent rupture, ie, not being able to tell when your gel implant fails without a special study. All implants, including silicone gel, will have some ripples at the edge and you are more likely to feel these when your tissues are thin. ...
Debating 455cc or 480cc? I am 5’4” and weigh 115lbs.
Is a nurse sufficient or should I meet the plastic surgeon when having breast augmentation?
Feb 14, 2019
Sizing a patient with breast implants is an art as much as a science and depends very much on the experience of the surgeon. Also, your wishes, tissues and expectations. Do you understand the trade-offs of larger implants and do those trade-offs make sense to you. I strongly recommend that you meet with your surgeon in person at least once and with enough time before surgery to feel comfortable with your choices.
As to sizing, Mentor has a sizing system of shells that you...
Is 530 considered a large implant?
Is 530 cc considered a large implant?
Feb 14, 2019
It is best to not get fixed on numbers, because what is large for one patient may be just right for another patient. The larger the implants, the more the tissue can stretch. The key in my opinion is to fill the breast but not overly stretch the breast. In my practice, a 530 would be considered a "large" implant because is is more than the average patient in my practice may need. But I have seen petite patients who have not yet had children where a 275 cc implant is...
How long does it take for implants to drop?
How long for Ideal Implants to "drop"
Feb 14, 2019
With any breast implant there is a subtle stretching of your breast tissues after augmentation that takes place after surgery. The time that it takes to get to a "final" look depends on how tight or loose your breast is to start and how elastic your tissues are. I have found that the Ideal implants can seem slightly stiffer early on than the silicone and my patients report that it takes 6 months to be able to comfortably push them together in a bra for cleavage. So "six...
Can ideal implants be placed on top of the muscle or ONLY sub-muscular?
Can Ideal implants be placed above the muscle
Feb 14, 2019
Ideal implants can be placed above or below the muscle, just like other breast implants. The benefit of below the muscle may be a lower risk of capsular contracture, more padding over the implant in the cleavage area, and it may be a little easier to do a mammogram. The disadvantage can be "animation" or motion of the implants when the muscles are tightened. In addition if the breasts are slightly droopy, the breasts can drop off the implant mound and appear...
What are possible reasons that Ideal implants claim a lower percentage of capsular contracture?
Ideal implants vs Highly cohesie "gummy" implants
Feb 13, 2019
This is a very good question. Yes, the 8 year data reported from the Ideal Implant core study is impressive. The rupture rate is 2.1 % vs a rupture rate from gel implants that is 3 -1/2 to 6 times higher depending on the manufacturer. And the capsular contracture rate of 6.6% is less than the capsular contracture rates reported in the FDA trials for gel implants.
It is not clear why the capsular contracture rate is so much better in the 8 year studies with...
