If indeed you have tried the treatments listed isotretinoin (aka accutane) may be a good choice for you. It is the closest thing there is to a cure for acne with about 80% of patients having no further acne after a 5-6 month course if dosed appropriately. The trend is to get away from long term antibiotics. And, topical retinoids (Retin-A, tretinoin, Tazorac, etc) have their limitations in some people. There are two possibilities here. The first is that this is another spitting stitch. Depending on the deep sutures (the ones under the skin) your doctor used they can last up to 6 months or rarely longer. This can resolve without intervention. The second possibility is that that cyst was not completely removed and it is beginning to recur. This would require another surgery if this were the case. I think the first scenario is more likely. Judging by your photo those sound like the great repair options and you will likely not need anything more involved. The repair will likely depend on whether the defect after removal of you cancer extends through the vermillion border into the pink part of the lip.
