I would recommend you start by having a consultation with a Board Certified Plastic Surgeon who is performing the procedure well in your area. You are doing so many of the right things in choosing your plastic surgeon. After your consultations, I hope you will have a feeling based on all of the those factors you mentioned, of who would be the best choice for you. This is how most of these decision are made. Sometimes other factors weigh in (cost, distance from your home etc.). There is no perfect way for you to make the final determination, but know you are asking the right questions and doing good research. Good luck!

I vote inframammary. This is the most direct approach and there are advantages. The precision of your sub-muscular pocket (the space that will hold your implant) is much more exactly dissected using the inframammary approach. Liposuction is the best way to remove fat cells permanently from the area. Experience matters. Schedule a consultation with a Board Certified Plastic Surgeon who can show you Before and After photos of his or her own patients. Making sure your surgery is performed in an accredited operating room with the use of a board certified anesthesiologist is also important. In this way you can feel confident moving forward with your choice.

Yes, in many cases chin liposuction can be performed under local anesthesia. The field block is not typically painful and once it has set up the surgery can proceed without discomfort. You would do well to consult in person with a board certified plastic surgery with experience in aesthetic surgery.

Your youth may be an advantage here as a younger person’s skin has a better change to contract following liposuction. The cycles of coolsculpting and weight loss and gain make results less predictable. An exam by a Board Certified Plastic Surgeon could help determine these choices/options.

Yes, and they can be very enhancing. For someone of modest proportions, these smaller implants can be beautiful also. I believe you should try on sizers during your consultation and bring photos (before and after) of patients with similar proportions to your own to review with your plastic surgeon. Together you can find a look, and implants, for the figure you are hoping to achieve.

It is difficult from your photo to see the areas of your concern, but if in jeans you see an accumulation of fat at the hip rolls (“muffin top”) you can very successfully correct this with liposuction under local anesthesia with light oral sedation. If the liposculpting involves multiple areas and requires blending of these areas, I often use a general anesthetic (administered by a board certified anesthesiologist). Your height and frame, in addition to your already well shaped breasts, allow much more flexibility in choosing a size. This ultimately is a very personal decision and one you can feel comfortable with it if your plastic surgeon reviews with you their before and after photographs of patients who are comparable. In essence, a larger implant will make you appear more voluptuous, and a more modest augmentation more athletic and feminine. The choice is yours.

You will get your best results with liposuction by losing some weight pre-operatively. That allows the removal of more fat cells per unit volume when you are at a lower weight – so it is worth it. Kybella may help… but will require multiple treatments and a longer interval to achieve your goals.
