Based on your pictures you have a wide chest and need an implant only to get an excellent natural result. I agree with your concern about the lift and think you should seek out other experienced board certified plastic surgeons to get other opinions. Remember cup sizes are not standardized and I recommend trying on sizers at the surgeon's office to get the "look" you want.

Great job on the weight loss and maintaining the weight for several years! Your BMI is still high (42) which puts you over the limit for many out patient surgery centers. This is why some offices will not even see you for safety reasons. I would search for plastic surgeons in hospital based plastic surgery practices who deal with large weight loss patients. Good luck

Usually older men with type 2 diabetes suffer from internal fat, meaning fat around the organs below the muscle layer. When you pinch your abdomen if there are several inches of fat between your fingers then this would represent external fat and yes, liposuction may help in the hands of a careful well trained surgeon. If there is only a thin fat layer then you have internal fat and liposuction would not help. Make an appointment with a local board certified plastic surgeon for an opinion.

It is predicable that with a lift after removal of an implant there would be little upper pole fullness. Patients who have lifts without implants are always counseled in this fact. Unfortunately the only way to return this upper pole fullness is with another implant. Fat grafting could be of benefit but is less reliable and you cannot get the volume that an implant would provide.

Generally as long as you are in good health you should be fine to undergo abdominoplasty with a history of gastroschisis. The belly button will likely have some issues with blood supply and your surgeon should plan on reconstructing it at the time of surgery. Seek out a board certified, experienced plastic surgeon with lots of experience in your area

You look to be an excellent candidate for a full abdominoplasty with flank lipo and muscle tightening. The tattoos can likely be spared, You can help your surgeon design your excision exactly as you want.
Realize that an excellent tattoo artist can "repair" areas that are not perfect after surgery and your tattoos, even if spared, will definitely look different after a tummy tuck. Visit an experienced board certified plastic surgeon in your area for an in person consult.

Sounds like you had a tummy tuck in the distant past and now have loose skin in the upper abdomen. The operation that would be most helpful would be a reverse abdominoplasty which has the scar across the upper abdomen (in the fold just under the breasts). Generally in patients with a reasonable BMI (under 30) I prefer patients to be at their normal weight prior to liposuction and not lose a lot of weight before the procedure. Remember liposuction is not a weight loss procedure it is a contouring procedure. It's better to diet after liposuction to enhance your result rather than to crash diet before and then gain the weight back after liposuction which will not help your result.

It upsets me when I hear stories like yours. A consultation looking for help turns out to hurt and a fixable issue goes unfixed. Find an experienced board certified plastic surgeon who has a good reputation in your area, interview 2 or 3 of them if the first one is not confident that they can solve your issues. If you are in good health surgery should give you a great improvement.
