Scars after breast implants

The inframammary incision tends to be become more apparent with maturation of the breast implant shape and final positioning. There are many alternatives to scar revision if this is the case.

Subpectoral vs Subglandular/Subfascial

I would expect that you would lose some degree of function with a subpectoral placement. This is not an issue for most women. The subglandular or subfascial placement will lead to a faster recovery and would give an excellent aesthetic result based on your pictures.

However, it is not a good idea if your breast tissue is rather thin. You are more likely to feel the edge of the implant and you need to be careful about the selected implant type.

There are other increased risks with subglandular placement, such as decreased visualization of breast tissue on mammography and capsular contracture. Unfortunately we do not have a perfect augmentation procedure for athletes. If possible, reach out to other female swimmers to see what they have experienced with breast augmentation.

Breast augmentation in asymmetric breasts

I will assume you are seeing a board certified plastic surgeon. In your consultation your surgeon assessed your asymmetry by physical examination and likely took measurements of your breast and chest as well as photos.

Based on this information as well as her/his experience made a recommendation regarding the size, shape and style of the implants. All of this information is likely to be more reliable than the use of the external devices by the lady “fitting” you.
Another supplemental tool that surgeons may use when operating on patients with significant asymmetry is the use of intra-operative “sizers”. In some patients with asymmetry it may not be completely clear based on their physical assessment what size, shape and style implant is best.

Sizers are temporary implants that may be used in the operating room. In surgery the space or “pockets” for the implants can be created and different sizers can be placed in the pocket and the patient assessed to see which ones look the best.

In the operating room the OR table back is raised up and the surgeon can look at the breasts in the upright position and make a determination which implants are most appropriate. The temporary implants or “sizers” are removed and the final implants placed.

Breast implants and chest wall width

I pay very close attention to chest wall dimensions as the basis for choosing an implant. I can tell from your photo that you do not have a 16-18 cm width with respect to the proper measurement of your chest.

Your’s will be less than that for sure. The best “look” will come from a moderate profile implant that has a good match to your true chest wall width. I’d be pleased to help you figure this out if you would like to visit.

Breast Lift versus Implant

You will not be happy with the appearance of your breasts with breast augmentation if you don’t also have a breast lift performed. A common misconception is that breast augmentation will just make your breasts look bigger, and perhaps even lift them, but in your situation it will just make your breasts appear unattractive.
A breast lift will reposition your nipple and bottom of the breast so that a breast implant will make your breasts look normal and natural. The cost is some scarring, and in many that is minor.

You should visit well respected surgeons that perform all forms of cosmetic breast surgery and look at photos of patients who started out looking like you, and see how the results look.

Yes and No

You can add “some” saline to the implant, but it would require gaining access to the fill valve which likely is right behind your nipple/areola. With a peri-areolar incision, I have done it in the past.

However, if you want to go up more than about 50cc, it may be preferable to step up to a new, larger implant. Check with your surgeon to see if there is room to add to the existing implant, but think twice before proceeding with another operation as it may simply not be worth it.
3 weeks is still early for implant position (even if one has dropped already).

At three weeks from breast augmentation surgery, implant position is frequently higher than ideal, which can take anywhere from days to months to finally achieve. This can happen on one side or both, but even when we strive to perform exactly identical procedures on each side, it is common to note different swelling, bruising, discomfort, scar formation, nerve regeneration, and sensation between sides.

In rare cases, if things aren’t right after 3-6 months, additional surgery is necessary to either raise a too-low crease, or to open the pocket more and drop a too-high implant. But neither would be advised until enough time has passed to see what final implant position is, and just what needs to be done.
It’s easy to see why dropping a persistently-high implant position is easier to perform, achieve proper position, and maintain long-term, than trying to raise a too-low pocket (and keep it there despite gravity and scar softening)! Most surgeons believe (and we see this in our patients) that implants initially placed slightly higher than aesthetically “ideal” will drop, soften, and settle over time.

However, there are some surgeons who try to position the implants in exactly the location they feel is best at the time of surgery, and then place their patients in surgical brassieres or elastic wraps to try to “maintain” the implants in that position.

In my humble opinion, that fails all too often, as evidenced by either a population of unhappy patients with too-low implants, or too many re-do surgeries to raise pockets. Neither is good. In your case, you have one implant (your left side) that looks pretty good in terms of position right now, though it may actually end up too low as time goes by IF your implant continues to drop.
The presently “too-high” right implant may be just fine over time as it settles and drops as the capsule (scar around the implant) matures and softens. IF it is too high compared to the left side because of bleeding, early capsular contracture, and a thicker, tighter capsule scar that has inhibited dropping, this will not get better and will require re-operation.

This should not be considered for 3-6 months or even longer, to determine exactly what will happen over time. So, as you can see, you may have a left breast that continues to drop and could end up too low, a right breast that continues to drop and may end up just fine, a left breast that is doing what is intended and a right breast exhibiting signs of early capsular contracture and malposition, or both breasts doing “fine” but at different rates of healing! Whew! Hard to tell exactly what is going on without knowing what happened during surgery, what you surgeon saw immediately post-op or in your early recheck(s), and what your present examination shows.
This takes an in-person follow-up visit. Your surgeon can assess and evaluate your breasts, determine what seems to be occurring, and whether anything can or should be done to aid your healing process. I frequently recommend an elastic bandeau below a good-position breast to help hold implant position from further dropping (like your left side), or above the breast to aid the still-too-high implant in dropping (like your right side).

Ask your surgeon! The short answer to “Is something wrong?” is that a photo (thanks for including one!) and brief description cannot provide enough information, but physical examination by your surgeon will allow him or her to provide you with advice and instructions.

Breast symmetry long after correction

Long term breast symmetry is within reach for most after correction of uneven breasts with breast augmentation, or in breast reduction procedures. Perhaps the most reliable and stable results come from the correction of breast size through reduction surgery.
The reason is that the surgeon is directly able to adjust the breast volume, and adjust and tailor the breast envelope to create symmetry which will hold up over time. Brest augmentation will also work out well if the breast shape and envelope are fairly similar, and the volume difference is within say 100cc.

We begin to lose the predictability of how an asymmetric breast will ‘age’ when the envelope is very dissimilar, such as the fuller breast on one side, and the more tubular breast on the opposite side.

Skin differences will stretch and age under their own plan, and the best bet at long term symmetry would be through reduction of the larger breast, and mastopexy to shape the tubular breast, again equalise the skin, and the volume.

The volume reduction may not please you however, and the alternative ‘plan B’ would be and implant on each side asymmetrically so, and a lift or mastopexy to equalise the skin envelope. The pressure of the implants on the skin will cause relaxation and thinning (aging) of the breast skin and a second mastopexy may be in your future.

To keep this in perspective, after ten years or so, new implants will be in your future as well because they do wear out as well. Implants do require maintenance, but if you would like the volume you should not be discouraged.

Am I a Breast Augmentation Candidate

I agree with the other plastic surgeons. it appears that you are a good candidate. I do suggest you take their advice. I recommend a Board Certified Plastic Surgeon and a thorough consultation and evaluation.

I don’t want a lift, can I get away with just implants?

Due to the degree of ptosis, or sagging, that you have, a lift with implants will give you the best result. A lift will remove excess skin, decrease the size of your areaolae, and restore your breasts to a perky position on your chest, while an implant will ass volume.

I understand not wanting the scarring associated with a lift, but as I tell my patients in consultation, a beautiful breast shape with a thin, faded scar is still a beautiful breast.

If I have breast implants that are under the muscle, can they be taken out and be placed over muscle?

Speaking from a technical standpoint, yes, it is possible to convert a sub-pec implant to a subglandular implant. The real question is to ask if this is the right thing for you and why or why not? This discussion cannot take place without a physical exam and a discussion of your concerns and goals you hope to achieve.

I would suggest scheduling a consultation with a local board certified plastic surgeon to discuss your concerns. This may be the right answer for you or it may not and there may be other options to address your issues.

Over or under the muscle breast implants

I personally do not agree that your best bet is above the muscle. I would advise below the muscle with a lift for the best result. Long term, above the muscle implants, especially if they have any size to them, tend to drop down into the breast like a ball in sock and can look matronly.
Mammograms are also better below the muscle. Your activity level shouldn’t change this either for me.

Breast enhancement options

I think I have a very nice solution for you. There are multiple ways to do a breast augmentation, however I think the best type of augmentation for a very active person is the sub fascial breast augmentation.

The implant is placed in a connective tissue layer called fascia. Instead of placing implants under the muscle which causes pain six weeks of no exercise and sometimes animation deformities which makes it obvious that you have implants, placing the implant under thIs layer requires minimal downtime, no damage to the pectoralis muscle, and no animation deformities with a very natural result.

I frequently operate on mothers that I’ve had deflation in their breasts and do a lot of mommy makeovers. I would love to see you and consultation and give you my advice in person.

Scars from breast augmentation

Regardless of the incision made to place breast implants, the incision site results in a scar. The main locations for breast implant scars include: Under the breast (inframammary) Around or within the areola (periareolar) In the armpit (axillary) In the belly button (TUBA – only available with saline implants) The choice of incision is based on the size of implant, the type (saline or silicone), your own unique anatomy (size of areolas, etc.), and placement of implants (above or under the muscle). When discussing breast augmentation with your plastic surgeon, you will be given pros and cons of each of these incision sites and which one they feel would work best for you.

Your input as to where you would like incisions to be placed is very important. Be sure to ask to see examples of the final scars in each position prior to making your choice.
Would saline or silicone over or under the muscle work best for me?

My recommendation is this: In your case, silicon implants (300-350 cc), sub glandular placement, for i can see in your pictures you have enough tissue breast to be placed there. Round implants. !
