Thanks for your question. Firstly, you need an MRI to make sure that your implants are intact if you are thinking of keeping the ones that you have. That said, you need a physical exam to evaluate where the implant is in relation to your breast. From your photos it seems as though you have an imbalance in both the implant size as well as some of the factors that sha;e the breast. I'm not sure what your surgeon was referring to with respect to "shaving down" the capsule, that is not common terminology in plastic surgery. Depending on the degree of contracture and other circumstances, most surgeons remove the capsule or incise the capsule in a checker-board type of configuration, and those of us who see a lot of other surgeon's capsules would likely treat you in one of those fashions. Worrying about capsular contracture is not really worth your time, and if you are happy with your result then that is what you should be enjoying. Signs of contracture include hardness and asymmetry, but if you are not having any issues then I don't recommend being concerned about it. You are correct in that many different surgeons will use many different manners of anesthesia for surgery. Lidocaine, or xylocaine which is the same thing, is most often used with some epinepherine which causes blood vessels to constrict so there is less bleeding. If more involved surgery is planned, Valium or other drugs may be used as well.

I think your question is more geared toward timing... you are physically an excellent candidate for anything you want to do with your breast enhancement, from doing nothing to adding volume to lifting.
