Based on your photos, you seem to be in good shape. Not sure how much weight you are still trying to lose, but I would suggest being stable at the weight you want to be prior to consulting for surgery. Reason being, although implants are certainly one choice for improving the appearance of your chest, a lift and/or fat transfer OR even a combination of the before mentioned are also some options.

I do not think you will get your desired look with implants alone. Your areolas sit low on your breasts and unfortunately, an implant will not change that. The best recommendation, based on your photos alone, would be for at least a vertical lift with an implant. You just really need to be prepared for scars in front of your breasts, which tend to fade nicely with time on someone with your skin tone.

Unless you are having certain symptoms or capsular contracture, I would not recommend getting the capsulectomy. However, I do advise my patients that once I'm in there, if something looks suspicious I will remove it.

Due to the rupture and your implants being subglandular, I would recommend removal of the implants first and allowing everything to heal prior to the lift. The fact that your implants were placed over the muscle might have impaired the blood supply to the nipple/areola area and it would just be a safer way to go to stage the process.
