If you are not satisfied with the results it does appear from the photo that more tissue could certainly be reduced from the labia minora, the prepuce/clitoral hood and possibly the perineum and labia majora. Hi and thank you for your inquiry. From you picture it appears your concerns are that of normal hymen tissue. I do not think this needs surgery as it is normal tissue. The 'hole' is probably just a fold of tissue. If it is an actual hole, that can be elucidated at an exam by your gyn MD so do see one just to be sure. A hole may need to be fixed if truly there.
