Always hard to tell with photos in these cases- you have a thin frame that makes any little discrepancy very visible. it could be a recurrent hernia, it could be a small pocket of seroma fluid or a bit of fat. The fact that you can feel a hole however says you likely have a recurrent hernia.

you have a lovely result from your abdominoplasty otherwise, i would recommend having a consult with a general surgeon who does laparoscopic hernia repair- in most cases this issue can be alleviated...

you do have a somewhat indistinct interbreast definition. implant shape and size are critical to be sure that does not worsen. "less is more" is the mantra you need to keep in your head to avoid a potential disaster- a conservative implant size and narrower shape is necessary to give you a nice result.

because of your thin frame and somewhat wide interbreast distance, I would recommend a lift to be able to properly accommodate an implant that would be proportionate to your body. Some doctors may recommend a larger implant to "fill up your skin"- and that works- but an implant that would suit your body would be best served to be smaller with a lift to give you a perky, well shaped look with improved cleavage and interbreast distance.

sorry for your disappointment- we don't undergo cosmetic surgery to be underwhelmed wit the results!

looking at your photo- i do agree your breasts are too low- the distance from your nipple to your fold seems too long for your breast envelope, cheat wall height, and implant size. that being said- there can be other factors such as skin elasticity that can contribute to this issue.
