Breast Augmentation
The size of the implant has the greatest impact on the final appearance; large implants give the greatest upper pole fulness. Stop thinking in cup sizes; no one can guarantee a cup size based on an implant volume.
Although round and shaped implants can achieve a similar result, shaped implants have a more long lasting good result and have the LOWEST complication profile.
Go visit a few ABPS certified/ASAPS member surgeons that specialize in breast surgery.
Position of incisions for breast augmentation.
There are several different incisions that can be used to insert a breast implant. Inframammary fold beneath the breast) periareolar, axillary and umbilical. The umbilical (belly button) route is not commonly used because it does Not improve the results and is only used for saline implants.
The most commonly used incision is in the fold beneath the breast. It is without question the most accurate and versatile incision. All types and sizes of implants can be inserted through this incision and it heals very well, being hidden in a natural skin crease.
The are no disadvantages to this approach. The next most commonly used incision is the periareolar which is around the lower half of the areola fropm the 3 to 9 oclock position.
It too can be used for most types of implants however there is a limit to the size if silicone implant that can be inserted through this incision depeding on the size of the areola.
It is slightly less accurate in that the entire pocket may not be able to be visualized and there is a slightly higher chance of a visible scar or distortion of the breast tissue below the scar.
The axillary incision is also much less often used because in most cases it is a blind procedure where the surgeon does not see inside the pocket and watches the outside of the breast as long instruments are used to make the pocket.
It offers no advantage nor does it improve the results when compared to the two more commonly used incisions and limits the size silicone implant that can be used and because of the axillary hair may have a more visible scar.
All 3 incisions when used carefully and sutured with care will give good results. Your surgeon will choose the incision he or she feels will give you the best resul tand that they are most comfortable using.
Old breast implants
If your ammmogram was ok then you can leave them alone. The gold standard exam would be an MRI if you really want to be sure.
Breast Augmentation Size and Shape
My responses are the following:
SIZE
1) size of breasts is a personal choice.
2) using the formula 1oz. = 30cc ….take 8 and 1/3 oz of rice = 250 cc or 10 oz. = 300 cc and place in a nylon stocking to create your own “sizers”.
Then place in a full bodied non padded bra and try on some form fitting clothes.
SHAPE
1) the reason your breast hang/sag is that your current “skin bra”is larger than your breast tissue.
2) there are three main types of mastopexy (breast lifting procedures).
- periareolar (doughnut scar)
- vertical (lollipop scar) a combination of both 1 & 2.
- inverted t (anchor scar) a combination of 1-3. Your breasts show some asymmetry (not unusual).
3) if you don’t have a lift and augmentation only …..your breast tissue will droop over the implant (like a waterfall) and the nipples will point downward resulting in a odd appearance . The model photo you like appears to show option number 2-c aboveBased on the limited photos of you most likely will need option 2-c above.

Avoid mastopexy with larger implants
Sometimes, but in your case no! That is the short answer. The long answer is that you are a special case. As a weight loss patient you have lost a lot of tissue support for one.
Adding extra weight to the already weakened breast tissue will doom it to fail- eventually. Also, your goal will be to add size until the nipples rise up to some acceptable position without regard to the aesthetics of the breast, and this will not likely create a good outcome.
Really, without exam/measurements, and just eye-balling it, your nipples are so low, the other consideration is that the implants would have to be placed in a very low position in order to raise up the nipples- not good.
All of these things taken together mean that you are doing the wrong operation to correct the problem, and that cannot and will not fix your problem- in will create new problems, and maybe, the new problems can’t be fixed.
The mastopexy leaves scars, and that is undoubtedly why you want to avoid it. However, the scars fade over time, and are usually not at all bad.
Can I get breast implants without a lift?
Your nipples will still appear low after a straight augmentation due to the implant sitting higher up on your chest, especially the right nipple.

How can I have natural looking implants?
Certainly you can have natural looking implants after an augmentation. When we size patients for implants we like to look at your whole body – your height, weight, chest size and shape waist, hips, etc. – along with the most important thing – your input, to create the surgical outcome that you desire.
Pain/discomfort after Breast Augmentation
It is important to remember that every patients pain tolerance is different. People recover differently than others. The best way to have an easy recovery is to get lots of rest and listen to your plastic surgeons post care instructions.
Also, you will be prescribed a pain medication to help with any pain!
Implant choices
I think that saline and silicone and gummy implants are all good choices. I will say that most of my patients select silicone implants.
Too big
5 ft tall and 112lbs with 450cc implants sounds too big for a natural look to me. If this is what you want, you might reconsider. I am seeing more and more patients in midlife who regret such decisions.
Is it worth it to get a breast augmentation to go up only one cup size?
Increasing the breasts by one cup size is a frequent request and achievable. The best cosmetic result in any particular breast augmentation patient depends on a variety of factors, including: Your individual anatomy, realistic expectations, a thorough discussion with the plastic surgeon about the options, and an understanding of the pros and cons of any particular implant choice.
Proper sizing is not just about the number of cc’s. The thickness of your tissue, breast dimensions which include the width, height, and projection, as well as chest wall width all need to be considered when choosing an implant.
Trying on implant “sizers” of various shapes and volumes while wearing a tight t-shirt, bra, or bathing sit at a preoperative visit will help you and your surgeon choose the optimal implant. There are no manufacturers’ standards for cup sizing in the bra industry.
The cups of a 32 C- D and a 38 C-D are significantly different. Cup size varies from manufacturer to manufacturer and even within styles from any particular manufacturer. There is also no direct correlation between an implant size or shape and resultant cup size.
Keep in mind that following the advice from a surgeon on this or any other website who proposes to tell you exactly what to do without examining you, physically feeling the tissue, assessing your desired outcome, taking a full medical history, and discussing the pros and cons of each operative procedure would not be in your best interest.
I would suggest you find a plastic surgeon certified by the American Board of Plastic Surgery and ideally a member of the American Society for Aesthetic Plastic Surgery (ASAPS) that you trust and are comfortable with.
You should discuss your concerns with that surgeon in person.
Lift or not? Am I making the right choice
I think a breast lift along with the implant would give a superior end result as it would allow some shaping of your native breast tissue on top of the implant.
Large breast implants
you need to get surgery to make your wish your bigger breast come tru but you need to know that if you get implants too big that can make you look fake and a non aesthetic result.
you need to consult a surgeon in person and to choose whit him/her what breast implant is best for you.
Breast asymmetry can be improved with different size implants
and for providing the photographs. I find that implants actually magnify the asymmetry. From the photograph you provided with you leaning over, you can see that the breasts have different projections. Often I will use implants with very different projections to try and make the result more symmetrical.
If the asymmetry is more volume but projection the same, different size implants with similar projections can help. Another option is to remove tissue from the larger breast and use the same size implants. It looks like you might also need a lift.
Be sure to discuss these issues with your plastic surgeon and do not hesitate to get a second opinion.
