Implant size
This is always the dilemma. How to choose size. Generally surgeons use multiple methods to arrive at the best size to achieve your desired result but nothing is perfect.
I have found using sizers to be imperfect.
The sizer doesn’t usually fit your bra well, it is not in the anatomic position it will be after surgery ( subglandular or submuscular) and you have it on for a minute or two- not a lifetime.
I always find patients choose larger than I recommend. Likewise computer imaging is also not ideal. It does provide you with an image that gets close to the real result but the photograph can’t always reliably reproduce your own tissue characteristics.
The larger you go in implant size- the larger the diameter and if the diameter exceeds your breast width- there is a real risk of early and late implant malposition and overstretching of the pocket.
Best to bring photos of desired size and have a thorough discussion with a well qualified Board Certified plastic surgeon. I suspect 650 is probably a bit large but best to have a second consultation with your surgeon.
Breast Implants and Pregnancy
With regards to the breasts, it is a given that your breasts will change as a result of pregnancy. The breasts will enlarge due to a hormone surge and then after pregnancy, and possibly the cessation of breast feeding, the breast glands will atrophy and the breasts will therefore shrink in size.
The degree of enlargement and subsequent atrophy that one can expect to experience is highly variable and unfortunately, it is also unpredictable. It is very common for patients to undergo a breast augmentation before they are even thinking about having children.
I have not seen any scientific evidence suggesting that the presence of implants is associated with any more discomfort, additional swelling, and the potential for stretch marks that comes along with pregnancy. Depending upon the amount of growth and atrophy you experience, your breasts will either return to their pre-pregnancy augmented appearance or they may appear less “perky” or full.
Some patients will not require any future procedures post-pregnancy and others may require either larger breast implants with or without a breast lift to fill out any extra skin that may have developed from this rapid growth and then loss of volume.
Breast Implants to lift slightly sagging breasts
In your case a high profile implant 340-375cc should result in a slight lift and restoration of breast volume. I prefer to lower the infra-mammary fold by 1 cm. and insert the implant via a peri-areolar incision.
Silicone gel implants placed sub-muscularly would be my choice. My opinion is based upon viewing your photographs and would be more precise with an examination. in your decision.
Implants at Four Weeks
Your right breast is bigger and the implant is definitely lower. I would like to see your preop photos and i suspect some of your asymmetry was present before your surgery. It is still very early and you should wait another two to three months to see how your breasts look.
Not only can patients present with differences in breast size and shape, but they can also have asymmetry of the nipples and the position of the breast fold.
Breast Feeding
Regardless of approach, you will retain the ability of breast feeding, assuming you have the ability (it is not always effective in all women). However, the inframammary approach is the best incision for breast augmentation, leading to the least risk of the most common complication, capsular contracture, as well as other common problems of breast deformity associated scaring.
The inframammary incision allows the surgeon to avoid any breast tissue structures!
Removal of saline implants
Uncomplicated removal of saline implants can be done in the office setting under local anesthesia with very little downtime. in patients who have other medical problems, or if the situation is more complex from a surgical standpoint, general anesthesia might be required.
This can be the case if your surgeon deems that removal of the capsule is also necessary. Sometimes surgical drains are helpful to reduce the incidence of seroma after the implants are removed, however these are also easily placed under local anesthesia.
Sometimes I will purposely rupture saline implants with a needle in the office prior to removing them. This allows the skin to slowly retract has the implant deflates. A compressive wrap is typically placed after the procedure.
Did I make a wrong size choice?
Your size does look pretty good for your body frame. Hopefully your surgeon counseled you that the breast being human tissue will not stretch infinitely and accept any size implant.
There is an ideal size based upon the dimensions of your breasts and the tissue dynamics. That is the best way to select size, which may or may not be what you want, but neither you nor your surgeon can change what is ideal for you.
Select a larger size and the chance of creating potentially uncorrectable deformities, worse scarring, a more painful recovery, and the risk of requiring a revision goes up substantially.
It’s not risky
A small reduction in implant size like you’re considering is not risky. The recovery is very easy and the should be able to give you the look you desire. It’s important though that you understand the changes you trying to accomplish because changing from a 500 to a 425 in high profile only minimially changes the projection and base diameter.
Talk with your surgeon and ensure that if you go ahead with this change, you’ll get the results you desire.
Under the muscle
Yes it it possible to place a breast implant in the plane below the muscle via a periareolar approach depending upon the areolar diameter. This approach is NOT the choice for women with very small areolas (2 cm-2.
5cm) considering the new silicone implants.
There is no reason to wait if you currently desire breast augmentation.
The current generation of silicone gel implants are a cohesive gel implant. They, however, are not THE cohesive gel implants or gummy bear implants that you discuss. These gummy bear implants are more cohesive, firmer, and less pliable.
They are a form-stable type of implant, meaning the shape does not change. They are more anatomic, but they may not give you the “natural augmented look” that many women desire. They are not without problems or issues as well.
These gummy bear implants can have issues with rippling as well. They are not currently available in the U. S. except for a few offices that are enrolled in current FDA studies. They are availaable outside of the U.S.
Breast surgery post breastfeeding
Once you have finished lactating and nursing your child, you would be able to proceed with surgery. However, make sure you are completely sure you do not want to have any more children. Plastic surgery does not guarantee permanent results and another pregnancy, weight gain/loss, nursing may affect results.
The best advice I can give you is finding a board certified plastic surgeon to consult with regarding your goals and best possible options. You may benefit from a lift or breast augmentation alone. The use of post operative breast implant displacement exercises is controversial and not recommended by all practitioners. First discuss this with your plastic surgeon prior to embarking on this regimen .
Thin women need muscle coverage of the breast implants
Lee, you have been given a lot of good information and I would agree with the idea of putting the implants under the muscle. I would also suggest that you consider a silicone implant with your situation.
Women that are so thin with very little breast tissue generally have more opportunity to feel the consistency of the implant. As you consider all of the pros and cons you will be able to make the best choice for yourself.
High Versus Moderate Profile Implants
You can then compare with three-dimensional side-by-side photos of you with the different implants in place. On the whole, high profile implants have a more stark appearance when there is little breast tissue overlying them.
They can be useful in patients with narrow base widths who still want a lot of projection as well as in patients who have some droopy changes to the breasts that needs to be filled in.
