Yes, we believe bacteria in the breast is the main culprit that causes CC. Nipple piercing increases the bacterial colonization. Surgeon must take extra precautions to avoid that area. Inframammary or transaxillary approach have lower rates of CC.Dr.Ary Krau

It is not possible to achieve long lasting upper pole fullness without the use of implants. To convert large droopy breasts to a more perky look, one needs to remove that part of the breast that sags below the crease and convert them to smaller perky breasts. Behind these newly formed perky breasts an implant can be placed of essentially any size. Generally speaking, when undergoing breast lift with implants, the larger breast is reduced and lifted more so as to achieve better symmetry. Once this is done same sized implants should be used. The exception is when chest wall asymmetry exists.Good luck,Ary Krau MD

There are essentially 3 groups of breasts. Those that absolutely do not need a lift. those that absolutely need a lift, and those, as in your case, can undergo either procedure. In your case I believe you will get quite an acceptable result whether you have a lift or not. With a lift, however, your breast will be higher but at the cost of more scarring. It is unlikely that your implants are ruptured. you have visible ripples or "knuckles" of your implants. This is seen most often with either saline or textured implants, and when implants are above the muscle. Sometimes capsular contracture can cause this but I doubt that is the case with you as you show a similar knuckle on your left where the breast looks looser.

If your breast crease is low, we consider that a "low foot print". There is no reliable way to raise the fold when implants are used. Although, your photo has a limited view, it does appear that all your breast tissue is essentially above the crease. In this case breast augmentation alone would work.Ary Krau MD FACS

Perfect symmetry never exists. No woman has breasts that match perfectly. There's always some asymmetry, even with perfectly executed surgery. Having that said, an attempt at better symmetry should be considered. Your best symmetry will be achieved with a lollipop type of lift. A donut lift can be used, but symmetry will not be as good. Also keep in mind that even though the lollipop lift has one more incision that the purely circular donut, the quality of the scars are superior...

Your best result will be achieved from augmentation mastopexy (lift with implants).The Vertical Mammaplasty is an excellent technique popularized by Dr Madeline Lejour where a breast is either lifted or reduced. No horizontal incision is required, just a lollipop pattern. This technique however does not work well when implants are used. Clearly, it would depend on what your goal is. However, I think implants alone would work quite well for you. As far as your asymmetry, keep in mind that all breasts are somewhat asymmetric. No woman has perfect symmetry. Placing different sized implants would make things worse, as the smaller breast usually sits higher, and a larger implant on that side would accentuate the uneven upper pole.Good luck,Ary Krau MD FACS

There are, essentially, three categories in which breasts fall into. in group one, no lift is needed. In group three, a lift is mandatory as the breasts would look worse if only implants are placed. Finally, there is that middle group, which you fall into. In this group you have the option of either undergoing pure breast augmentation or lift with implants.
