Breast Implant Placement and Incision Options
This is a complex decision for which there is no easy answer. It depends on such factors as: age skin type race history of child birth intentions for nursing degree of ptosis (sag) implant type amount of asymmetry patient preference previous surgery/scars medical history size of the areola/nipple exercise/activity level etc.
Submuscular placement
The implants under the muscle have their shape defined by the muscle which can make them look smaller than they really are.
I prefer the sub-fascial plane so the implants define the shape of the breast and enhance the lift.
Saline vs Silicone
The choice for saline vs silicone is simply personal preference. The overall look is fairly similar, however the feel will be different. Teardrop shape is also a matter of preference. These kinds of issues would be better addressed during a consultation.
An implant can “bottom out” if the inframmary crease is damaged.
There are several reasons why an implant might appear to be “bottoming out”. Sometimes, the inframmary crease can be damaged.
Although it can be surgically fixed with the implant in place, most of the time I recommend removal of the implant and capsule for 6 weeks to three months and then reinsert the prosthesis when the crease has reestablished itself.
Often, the implant will be in front of the muscle as the activity of the pectoralis can create a vector of force against the crease when the implant is in submuscular position.

it appears that you would benefit most from a breast lift. You could do an augmentation with implants at the same time if you wish to add more volume to the breast. Adding implants alone will only cause the breast to sag even more.
Fat grafts
I would say that you may be perfect for some fat grafting to improve symmetry and shape. You will not close the gap much but it will be better. You may need to do 2 procedures to get the results you desire.

800 cc breast implants?
800 cc breast implants are very large implants. I would be interested in seeing how you came to this conclusion. In my practice, we do a combination of things to determine the size that you would be happy in.
Trying on implants in a bra and under clothing Dimensional analysis fitting the “right” dimension of implant to your frame 3D imaging so that you can “see” the changes in your body before surgery If you feel that you have accurately determined 800 cc is the correct implant size for you, you may be right (but I doubt it).
In my practice, this is a very large implant and would likely do best in stages with an initial implant in the 500 cc range. 800 cc saline implants WILL give you a rounded upper pole and very augmented appearance.
The feel of the breast will be very unnatural, which it sounds like is not what you are interested in.
Very small percentage
The incidence of silent rupture is very small. Most cases i have seen were related to trauma e.g car accidents etc.. However, the FDA advice on gel implants is MRI followups. The new generation of gel implants will not leak into different body areas even if ruptured.
If it is leaking, then you need to have the implant replaced.
If your implant is leaking, then it should be replaced. There really is no restriction on your activity, since if there is a leak, there is nothing short of replacing the implant that can be done.
You should go back to your surgeon for a consultation and set up surgery.
You may want to consider a saline implant
The implants do not effect your risk of getting breast cancer. A silicone gel implant can interfere with a mammogram, making it difficult to clearly see breast tissue, whereas, it is easier with a saline implant.
If there is any concern with not having good visibility of breast tissue then there are other options such as a MRI or ultrasound to help visualize the breast. So, one solution is to get a saline implant since it does not interfere significantly with the visibility of the breast tissue.
Now in regards to your breast shape, you say you are experiencing sagging. If someone is sagging, you need a breast lift. If the breast implant is placed into a sagging breast ,the weight of the implant is going to cause more sagging, so generally it is not advocated because the “sagging problem” will be compounded by the weight of the implant.
Breast implants are safe during air travel.
Breast implants are not affected by altitude or high pressure. So flying with breast implants is safe. Rest assured and have a fun trip!.
Breast Implants appear smaller after a year
Well, if your breast implants are saline-filled and getting smaller, they are deflating. It would be unusual for both to be doing so, however. After surgery, the skin and muscle may relax and the implants will tend to “drop” a bit, which may make them seem smaller.
On the other hand, changes in your actual breast tissue or fatty tissue may be responsible for your breasts appearing smaller. Your surgeon can tell you what is happening after he examines you.
Downsizing versus removing breast implants
You will likely notice a significant difference between the 350 cc implants and 250 cc implants. It is possible to remove the implants altogether, but keep in mind that having those implants has stretched your skin, and so does pregnancy.
Removing 350 cc implants altogether will result in a dramatically different breast shape and size, making you not just smaller, but possibly flatter and more droopy. This is especially true if you do not have much breast tissue and already have drooping.
There are other factors involved too, such as the size of your frame, style of implant, quality of skin, etc. Having 250 cc implants will definitely look different from having no implants. It is difficult to say without consulting with you in person what would be best, but what your surgeon has recommended sounds reasonable.
Regardless of how small you decide to go, you want to have an attractive breast shape overall. Basically the options are implant removal +/- breast lift, and implant exhange +/- breast lift. Make sure you understand the expected outcomes and limitations for each option.
Once your expecations are clear to your surgeon, he or she can explain how to best accomplish your goals. If you are uncomfortable with one surgeon’s recommendation, consider seeing another board certified plastic surgeon for a second opinion.
