The size of the implants put tension on the donut lift which is contributing to the dilation and scarring. You can correct this by converting to a vertical (lollipop) lift which will also improve the position of the nipple-areola. Downsizing the implants will help too. Cutting the ligaments is not always necessary for tubular breasts and is not the issue in your case.

This is not a commonly done approach but it works in the right patient. Implants can be done at the same time. It is important to understand that the upper scar is hidden in the crease along the bottom of the breast but crosses the midline where it can be visible.

There are two causes of double bubble: Lowering the inframammary fold resulting in the implant dropping, and detachment of the pectoral muscle which is done with the dual plane method. The cut muscle can adhere to the capsule resulting in pulling along a crease that looks very similar to the first cause. Have them evaluated with ultrasound to make sure there is no rupture. If so, you would qualify for replacement of the implants under the manufacturer's warranty, and your surgeon will want to develop the surgical plan accordingly.

My vote is for capsulorrhaphy (internal suturing) and a Gala internal bra. Your implants are on the large side, and they need support in order to maintain the upper pole fullness that you want.

The degree of lifting with a crescent technique is minimal, and usually used for cases of mils asymmetry in nipple-areola position. It depends to a large degree on where the nipple position is relative to the bottom of the breast, and to the size of the implants. My guess in your case is that at least a Benelli lift but more likely a lollipop lift is going to give you the best shape with implants.

Capsulectomy adds significantly to the risk of bleeding and other issues and will prolong recovery. It also makes it risky to do a breast lift at the same time. Considering also the fact that it removes some tissue, there is a definite possibility that it could compromise the eventual result especially if you are thin.
