Exparel is a long acting (72 hour) local anesthetic. It’s commonly used in tummy tucks as well as other plastic surgery procedures. I find that the results are inconsistent- which is why I don’t use it routinely with all tummy tucks. You likely will not have enough fat to add that amount to both your breasts and your buttock. It will be easier for you to focus on using the fat for your buttock augmentation and then increasing the size of your breast implants in the future if you really want to go larger. The risks of blood clots in particular (DVTs- deep vein thrombosis) is most concerning with patients and elevated BMIs (body mass index). The higher the BMI the higher the risk. So try to keep your BMI as low as possible before having surgery, ideally under 30. (This is a guideline, not a rule)

Liposuction of the upper abdomen at the same time as a tummy tuck carries risk to the blood supply of the abdominal skin flap. This means that the liposuction can potentially cause the tummy tuck incision to heal poorly or become infected. The safest approach is to avoid liposuction of the upper abdomen with a tummy tuck and instead perform direct removal of the deep fatty layer- not the fat close to the skin.

Can you combine extended tummy tuck and medial thigh lift surgery safely?

Congratulations on your weight loss. If you are in good overall health, it is possible to combine a tummy tuck with a thigh lift if it can be done in a reasonable amount of time (less than 6 hrs). Some would say to stage the two procedures for reasons of blood supply as the skin in the groin area will be pulled in different directions due to the two procedures. Discuss this in detail with your plastic surgeon during your consultation.

If you are concerned about double bubble or implant malposition, you can inquire about using an internal support device. I prefer Galaflex which is dissolvable and adds strength to the repair- it will be reabsorbed in 18 months and replaced with collagen.

BMI for tummy tuck?

A BMI under 30 is recommended for plastic surgery, but this is just a guideline. Any BMI over 30 is considered obese and carries a higher risk of complications post surgery. At your height of 5’7” your weight should be closer to at least 190 before having a tummy tuck. I think you’ll have a much better cosmetic result as well at a lower body weight. I hope this helps. Safety is most important so take your time to lose weight prior to surgery.

Breast lift or not with implants

Based on these photos it appears you would do well with a breast augmentation (dual plane) without the need for a breast lift. During the surgery with sizers in place, it will become more clear, but it doesn’t appear the scar of a lift is worth the slight improvement in nipple position. Breast implants alone will give you a nice outcome. Later in life, if needed, you can always have a breast lift done separately.

I’m 4'11" and weigh 240 pounds. Should I be able to get the BBL surgery?

If you body weight is too high, the risk of complications is too great. I would recommend getting down to a healthy BMI (body mass index) before having plastic surgery or any kind. For your height, you should be closer to 150-160. Consult with your primary care physician or a dietician for assistance with a weight loss progress (diet and exercise).

You may consider Renuvion treatment to your abdomen if you have some skin laxity and don’t feel a tummy tuck is right for you. Bodytite won’t help much, compared to Renuvion which uses much higher temperature under the skin to contract the collagen attachments between skin and muscle. Consult with a few plastic surgeons in person for a more detailed exam.

Wait at least three months to allow recovery from your breast surgery. You may not be able to have 1000cc’s of fat injected per side, but that depends on your skin quality and donor fat. The combined BBL and tummy tuck is possible , but it may be difficult for you to lie down in a comfortable position during recovery.

Do I have to have TT or can I get good results with just BBL?

Liposuction with fat grafting to your buttock (BBL) will give you a very nice outcome and improve your waist to hip ratio further. If the skin of your abdomen has good elasticity and not many stretch marks, then liposuction alone may give a good result. If your lower abdomen has loose skin and many stretch marks, then a tummy tuck may be best. Consult with a few plastic surgeons in person for a more detailed exam.

Your body’s soft tissues (skin/muscle) will ultimately determine how much volume your breast can accommodate. Your surgeon will not know for sure until you’re in surgery for certain, if sizers are used which can help with volume estimation. 30cc either way wont’ make much difference in cup size.
