There are two 2 inches incision in each breast instead of one. Is it normal?

Like other surgeons it is difficult to give a definitive answer without seeing the photographs but it certainly something I have never heard of before and you should return to your surgeon.
Is it normal for there to be a spot of blood at incision site after 5 weeks?

Hi.
It would not be normal to have a yellow discharge so long after surgery. From the photographs the wound looks a little red. You should go and see your surgeon ASAP as you may need antibiotics
