Every surgeon has his protocols and restrictions. I personally accept my patient’s decision on wether or not to reconstruct the areolas and nipples. If you do not want Areolas and nipples, you simply sign the special informed consent that states that it is not “normal” but desired and your personal preference. grafts were necessary for the surgeon who performed them as part of his protocol, not for your personal preference. I would need to see photos in order to further advise.

Thank you for your question. In your particular case, you have too much loose skin that will sag after Gland and fat removal from the brest mounds.
Your Double incision mastectomy would be a wonderful procedure that will be very aesthetic and conform to masculine anatomy.

Hello and thank you for the question and photos. Since the Areolas and Nipples are considerably lower than the breast Crease (Medical Term is Ptosis Level III) you definitely need a Breast Lift, with or without implants. The difference is that a Breast Implant would provide upper half of the breast fullness. If you consider that your breasts are firm, then a breast lift alone might be the best choice. The type of Breast Lift would be the Inverted “T” scar or also called “Anchor” scar.

I agree that your facial features are not robust, but your nose is the MOST IMPORTANT aspect of your face that needs feminizing. A Feminizing Rhinoplasty will give you the added femininity that will always be the CENTER of attention of your face. I would narrow your nose, raise the tip and remove the slight hump.
I would not recommend jawline surgery for now.

The Double incision mastectomy always has the advantage of resulting in a completely flat chest. Nevertheless, enough skin has to be present before surgery in order to safely and aesthetically perform this technique. The small current size of your breasts is not the main issue to consider, but the amount of extra skin that can be removed. Pictures would surely help (front and side views).
