There are a number of choices.  If your goal is to simply reduce the size of the areola, and provide a modest lift, then a periareolar lift ("pursestrings lift") might be a good choice for you.  Seek out a board certified plastic surgeon with a lot of experience preforming this procedure.

Scar revisions can be very beneficial is some cases.  However, as your breasts continue to stretch, loosen and drop in the coming years, that may be a  better time to consider revision surgery.

Without photos it is difficult to offer you much advice.  However, in most cases, these things can be improved with revision surgery.  Some of these procedures are quite straight forward while some would require more extensive surgery. Schedule a consult with a board certified plastic surgeon with a lot of experience performing breast surgery revisions.

You may be a candidate for a lower blepharoplasty but this is not an easy thing to fully correct.  Make sure you seek out a very experienced board certified plastic surgeon and get several opinions.  Best of luck.

Fillers can work well in this area but the best filler is your own fat which can be injected in this area.  This can be done under local anesthesia in the office.

The tear trough is actually the area just above the "tube Like" structure you are describing.  Imagine a tear rolling down your cheek in the depressed area just above that line.  That is the tear trough.  Fat grafting can be very useful in this area.

Fat grafting to your cheeks can be very useful in many cases such as yours.  It doesn't appear that you need fat removed from your lower lids.  Arrange a consult with a board certified plastic surgeon with a lot of experience in facial fat grafting and eyelid surgery.  Best of luck with this.

Implants as old as yours should always be exchanged at the time of a breast lift.  The vast majority of implants are ruptured by the time they are 29 years old. Exchanging the implants during the lift is usually easy and very cost effective compared to going back later for an implant exchange.

In your situation it is always best to get a second opinion.  The fact that the asymmetry doesn't bother others doesn't mean that it shouldn't concern you.  Another surgeon may be able to help you through the decision making process.  Best of luck with this.

If your goal is to have smaller perkier breasts this might be an option.  Make sure that your surgeon feels that you have enough breast tissue to create a breast size and shape that will be pleasing to you.Best of luck.

It is safest to wait 3-6 months after you stop breast feeding before undergoing breast surgery.  If you are still lactating at that point you might need to have your OB prescribe medication to stop lactation. Keep this is mind as you plan your surgery and pregnancies.Best of luck.

Without photos it is impossible to give  you a solid answer.  If your  breast sagged prior to your augmentation they will do the same after removal.  It also makes a difference if your implants are above or below the muscle.Your surgeon should be able to guide you through the decision making process. It may take 12 months or more before your scars have fully matured.  Give it time, avoid tanning the scars and keep in touch with your surgeon.  Best of luck with this.

In some cases this can be improved with a purse-string (periareolar) breast lift.  This involves making a circular incision around the areola which will make the areola smaller as well lift the breast and reduce the tuberous qualities of your breast.  Find an experienced board certified surgeon in your area for a  detailed consultation.  Best of luck.
