Natural Saline Implants

With a well-experienced board-certified plastic surgeon and with the proper implant selection in both volume and profile, patients can expect to obtain a very natural result following their breast augmentation procedure.

It is important to note however, that a patient’s cosmetic result from a breast implant surgery has a lot to do with what the patients expectations are (i.e. desired size) and how much breast tissue a patient naturally has to cover their implants. In my practice, most patients opt for the new cohesive silicone gel implants since they tend to feel and look more natural than the saline implants do.

Saline implants are definitely an option as well and I have used these on many patients who have had outstanding and very natural results, but I believe that the decision as to which type of implant to get should be made after a good and thorough dialogue with your surgeon.

I prefer to place all breast implants below the chest muscle as well and this will also lend itself to a much more natural look and feel.

I would recommend discussing the pros and cons of both saline and the new silicone gel implants with your board-certified plastic surgeon.

A well-experienced plastic surgeon should speak to you in depth about the differences between the two options, as well as the advancements in the silicone gel implant technology. Remember that a natural result is dependent upon patient expectations and the pre-operative conversations that you have with your surgeon, so communication is key.
It looks to me like you may need a breast lift in addition to an augmentation. Obviously, an in person exam is going to be the best way to help determine your ideal operation. It is imperative that you seek the advice of a plastic surgeon who is certified by the American Board of Plastic Surgery and has extensive experience in breast augmentation.

How to choose between high profile and moderate profile breast implants

It seems we are all in agreement that high profile implants usually work best on a narrow chest wall. As many of my colleagues have pointed out, the high profile gives you more projection and it is not as wide at the base.

Thus, it will not look like the implant is encroaching under your armpit. However, there are exceptions to this if you have a convex chest wall (outward). To learn more about how to choose between high profile and moderate profile implants please see my video and blog post below.
Rippling with breast implants

I almost always place saline implants under the muscle but I now primarily use silicone which results in a more natural breast. The manufacturers have altered the implants so the chances of capsular contracture are now about the same as with saline but the ripples are much, much less.

Implant choice

I think that in order to size you properly for breast implants, you would need to be seen in person. Looking at a photo alone is not enough.

Over or under?

I always prefer in petite patients with smaller amounts of breast tissue (a-b cups) to place under the muscle because less risk for implant show, less wrinkling or rippling, and less capsular contracture. An in person exam is the only way to tell which option is best for you
Talk to your doctor

Without examining you, nobody else can tell you what is best for you. I believe that your doctor should help you make the choice based on you breast base diameter measurement.

Mother of 3, Looking into Saline breast implants but really want to forgo a lift. Is that possible?

There is no implant size, no matter how large, that will prevent you from needing a lift based on your photos. Large implants do not elevate the breast and they will not fill out enough of your sagging skin to give you an acceptable breast shape.

You will only end up with excessively large breasts that will sag even more with your nipples pointing downward. The placement of the implant above or below the muscle will not have much impact on the way your breast will look without a lift.
Long term, you will have a nicer and more stable result with subpectoral implants and a breast lift.

No single best answer for everyone

You could have 2 or 3 more consultations and get as many different opinions. In gneral, going under improves the contour along the upper portion of the breast and may help it to look more natural. Some believe that implants are less prone to capsular contracture under the muscle.

Disadvantages are that the breast may distort when you are being physically active and the muscle flexes, and that the muscle may tend to push the implants too far apart or down. So, it is a matter of the surgeon’s judgment and experience.

Just to make it more complicated for you, there is an option called “subfascial” which provides support like going under the muscle but no risk of flexion distortion (called “animation deformity.”) A variation of this is to split the muscle instead of cutting it and use just the upper portion for implant coverage.

This is the technique I use in women who are thin and/or athletic, so that they get a natural contour but without the risk of animation deformity.

Best Implant Placement for ‘Exerciser’

I feel the most important issue is to determine a good implant size for you. Many athletic/fit individuals will benefit from a more conservatively sized implant to balance their body better and to allow them to engage in their preferred activities without having overly large breasts.

Next in importance is the implant type. The less breast tissue a patient has the more advantageous having a silicone implant is as it feels more natural and is less likely to be visually noticeable as rippling under the breast skin.

Lastly is the implant pocket. Good results can be achieved ‘in the correctly chosen patient’ with either over or under the muscle approaches. However, my feeling is that sub-muscular placement is more likely to look more natural and stay softer than the sub-glandular approach.
