Are some people more prone to capsular contracture, regardless of which type BA implant?
Are Some Patients More Prone to Capsular Contracture
Dec 5, 2020
Any foreign object and is introduced below the skin level will develop scar tissue around it. It doesn't matter if it's a breast implant, pacemaker or glass from a car accident, all will develop a capsule of scar tissue. The body doesn't like foreign objects and therefore wants to wall them off. A lot of study has gone into breast implants to determine rates of capsular contracture. What we know is that any implant placed above the pectoralis muscle has a relatively high rate of capsular...
Nipple pain and hypersensitivity 4 years post-periareolar lift with 375cc gel implants. (photos
Chronic Nipple Pain After Aug/Lift
Dec 5, 2020
Hypersensitivity after beast surgery is not an uncommon situation. If caught early on I have my patients massage the nipples for five minutes twice a day. This over stimulates the nerves and they essentially reset to their normal function. After four year I suspect this may not work. However, it is worth a try. At this point I would try a medication that is specific to nerve pain, namely Gabapentin. If this doesn't work you may need steroid injections into the nerves or a nerve...
Are there any hernia surgeons that can fix me once and for all?
Recurrent Abdominal Wall Hernia
Dec 3, 2020
Having a ventral hernia repaired three times does not bode well for a permanent fix. To be frank the issue isn't the surgical repair. The problem is too much internal fat and therefore too much pressure on the tissues. If you really want this fixed you'll have to lose the internal or visceral stomach fat. Not only will it help your hernia but visceral fat is the "bad" abdominal fat. Best of luck.
I've been getting conflicting recommendations from consults - what's going on?
Disagreeing Plastic Surgeons
Nov 12, 2020
So not sure why you want a breast augmentation as you have very beautiful natural breasts. However, let's discuss each option. First, round vs anatomic (teardrop). If you take a round implant and hold it by the edge what happens? Well, it turns into a teardrop. A smooth round implant is dynamic as it rolls around in the pocket and will always be in the anatomic shape when it's in the pocket. Anatomic implants are great for breast reconstruction after mastectomy. However, they give a...
Revision needed after breast lift?
Breast Ptosis or Sagging After Breast Lift
Nov 12, 2020
I wonder if you asked your plastic surgeon for a lollipop lift or he suggested it. Either way it was the wrong operation. He should have told you that a standard breast lift or anchor pattern was the only way to achieve the best result. In your case the lollipop or circumvertical lift improved the medial and lateral components of the breasts. However, the distance from the nipple areolar complex is still too long. This is why the nipple appears high and there is a lot of breast tissue...
Can this be corrected and should it? Did my surgery get botched?
Breast Change with Pectoralis Contraction
Nov 12, 2020
First of all there really isn't anyway you can "take care of" your implants. Once they are in it's fire and forget. What you are experiencing with pec contraction is an animation deformity. It's very very common. This happens because the implant capsules, that form, are attached to the pec muscles. Thus when you flex them it pulls on the capsules and distorts the breasts. There isn't a fix for this. As far as being too close together that's hard to say without seeing your preoperative...
How do I go about this difficult situation?
Right Breast Issues After Fat Grafting
Nov 12, 2020
I really wish that plastic surgeons would stop fat grafting breasts! It's a horrible procedure that rarely accomplishes the goal for which it was intended. The problem with fat grafting is that fat is very fragile and if it doesn't get a blood supply quickly it dies. When it dies a number of things can happen, 1. the body will reabsorb the dead cells and fat, 2. The fat becomes hard and calcified, called fat necrosis or 3. it turns into fat cysts. Unfortunately, usually 2 or 3 are the...
Does any doctor have experience with ulnar neuropathy or CRPS after Breast Implant Removal?
Implant Removal and Capsulectomy
Nov 12, 2020
I'm sorry to hear about your CRPS. As a hand surgeon for many years I appreciate how difficult it can be to treat and resolve this. However, breast implant removal should not affect the CRPS or cause it to "spread." I perform hundreds of implant removals. It is my personal opinion that the capsule should be removed. The raw surfaces created by the capsule removal allows the tissue to better adhere to each other and thus prevent the development of a seroma or fluid collection. In...
Wide-set breasts, very flat at top, small amount of beast tissue, not symmetrical. Are my breas
Breast Symmetry Correction
Aug 17, 2020
The short answer is no you don't have tubular breast deformity. You have have breast volume asymmetry, nipple height asymmetry and loss of upper pole volume (the chest above the breast tissue). My recommendation would be a breast augmentation with low or moderate profile implants. This allows for a greater base diameter which will help fill the upper poles and give you a very natural result. In addition you may need an implant one size larger on the smaller left breast to improve your...
Severe itching after Mentor BA. Allergic to the adhesive. Itching and redness has spread after
Redness, Oozing and Itching After Surgery
Apr 4, 2019
Rashes are VERY common after surgery.  They can be caused by a reaction to the surgical scrub prep that is used to clean the skin, adhesives in the dressings, glue if it is used and warmth of the post operative dressing.  There are two ways to treat the rashes: steroids or antifungal medications.  Initially the surgeon has to decide if the rash is a topical dermatitis or reaction to the adhesives.  In your case if the the rash didn't get better with the steroids and with the photos you sub...

Post Op Course After Breast Augmentation
Apr 4, 2019
There are three post operative periods, the immediate, the subacute and the chronic or late.  You are in the subacute period.  The pain you are experiencing is hypersensitivity caused by the nerves "awakening."  This is called hypersensitivity. I have my patients massage the nipple or the area of hypersensitivity for five minutes twice daily.  This will "reset" the nerves and make the hypersensitivity resolve.  The area of flatness is minor and still can improve because it takes at leas...

Nipple Sensitivitiy After Gynocomastia Surgery
Mar 19, 2019
When I perform gynecomastia reduction I almost uniformly do liposuction and excision or tissue removal.  This, however leaves a small amount of breast cells in the nipples.  Typically patients are numb after this surgery.  However, as with any breast surgery HYPERSENSITIVITY can happen.  This occurs when the nerves are recovering from surgery and are angry.  They being to fire out of control, hence hypersensitivity.  I have my patients massage the nipples with Vaseline for 5 minutes twice...

Long Time Drain After Tummy Tuck
Mar 19, 2019
Most surgeons use drains after tummy tucks.  It removes the fluid and prevents the seromas (fluid collection) that can occur if not used.  However, I believe that if the drain is still in at four weeks it can begin to cause more fluid or inflammation.  I will remove a drain by four weeks.  Fluid may accumulate and need to be drained but it's very rare a this point.  In addition the tummy is numb and typically you cannot feel the aspiration or removal of fluid.  I always instruct my patie...

Tummy Tuck with and without Drains
Mar 19, 2019
Tummy tucks without drains came into the surgical scene about 20 years ago. The thought was that if the tissues were intimately associated with the abdominal wall it would heal faster and without the issues of drains. The reality is that the body produces fibrin, which is a type of glue.  The abdominal flap or skin and fat is glued naturally to the abdominal wall within 24 hours vis this fibrin.  The post operative course does not have anything to do with tissue adherence but rather t...

Fillers for Tear Trough and Nasolabials
Mar 19, 2019
Any filler can be used anywhere it is needed.  I am of the mindset that it is easier to add more filler then it is to remove filler.  So typically I will use less than a half of a cc of filler for each tear trough.  the remaining can be used for the nasolabials or other area.
