Breast augmentation for mild breast lift

It is possible to fill out a slightly droopy breast with an implant in order to give the appearance of lifting the breast. This is just an illusion however. Augmentation just makes bigger what you naturally have.

The only real way to lift a breast is to remove skin. The fullness one get with an implant tightens the skin, and appears to lift the nipple areolar complex. In your situation, a moderate sized implant in the 300 cc range would do the job .

Lift or not

You have a good amount of laxity and much of the breast tissue is below the fold. I think you need a lift. You will only need implants if you want to be larger and have more fullness to the top of the breast.

Patients with skin laxity and good natural volume will always have some degree of droop. Natural tissue responds to gravity even with a lift.

Lift with implants at the same stage is a surgery that is more prone to recurrent relaxation especially in the weight loss patient where skin tone is looser.

Be sure you are well informed prior to surgery.

Breast Augmentation.

You need a lift. Simply put if the nipple is below the inframammary crease you will require a mastopexy. Your nipples are definitely below the inframary crease.

For Implant Sizing, Dialog with your Plastic Surgeon is Best

First, there really isn’t much difference between 325 and 275. It’s at most a 15% difference. Second, it’s hard to tell which would size would be best. All would probably be fine. Third, photographs are helpful, but I also use many other factors when choosing what is the best implant: chest width, skin thickness, skin stretchability, etc.

Breast enlargement cream?

Unfortunately there is no such “magic” breast enlargement cream that will achieve the kind of results that you’re desiring. There are of course multiple non-surgical breast enlargement alternatives for consideration on the market, but you’ll need to be realistic to what is actually on offer vs the kind of outcome that you desire.

This explains why breast enlargement is the most popular cosmetic procedure in the world!

Implants alone or implant and breast lift

you do look like you would benefit from a lift, but the nice thing about elective surgery is that you never HAVE to do anything. If you are looking for more upper pole fullness and increased breast volume but nipple location and overall breast shape are secondary concerns to you, then it is a reasonable thing to consider performing a breast augmentation with an implant and seeing if you are happy with the results.
A dual plane breast augmentation could give the illusion of your nipples having been lifted by lowering the position of the implant relative to your natural breast mound and allowing the nipple to ride up over the most projecting part of the implant.

The benefit to doing this is that you potentially avoid scars and extra surgery if your needs are met with that one operation. The risk is having a “waterfall” deformity where the breast tissue sags down over the implant mound.

Without a physical exam and measurements, it’s impossible to say for sure if this would happen to you.

Make sure you see a plastic surgeon who is certified by the American Board of Plastic Surgery if you consider moving forward to increase you chances of having a safe and successful outcome.

Do you need a breast lift

Only a good examination will be able to determine whether you require a breast lift. It seems like you may be right on the border, and you could have a reasonable result with just an implant, but perhaps a much perkier result with a combined procedure, including lift.

Sometimes, in this situation, it is best to have a breast augmentation, expecting that you may need a lift someday.

Less pain after breast implant surgery

Let your doctor decide during surgery which one looks the best. The sizes are so close I don’t think you can tell the difference.

Lift or implants

I agree with several colleagues above in that you would be fine with implants alone. However, I think it also depends on the look you are going for. I have many patients who want a better shape but do not necessarily want to go “bigger” and it sounds like you are leaning toward this.

If you desire a higher, tighter and more athletic looking small breast then you should have a small vertical lift (lollipop scar) so that some of that skin excess is taken away and the whole breast sits higher on the chest.

If you want a more “natural” look then you can get away with a breast implant that sits at the lower part of the chest. This will not lift the breast higher on your chest wall.
Implants alone will often make “what you already have bigger” whereby some skin manipulation (lift) will allow for more reshaping. If you did go for a vertical lift then I would perform it at the same time as your augmentation.

High Breast Implants In First Few Months After Surgery

It is very common to have that look in some cases after breast augmentation for the first few weeks or even months. This can be particularly evident in very small breasts with tight skin and larger implants.

If it persists much beyond 3 to 4 months, however, then the implants may need to be lowered in a second surgery if you are still unhappy with the look.

Breast Implants over the muscle for active arm use

Because of your activities with significant arm use and mild droop, over the muscle implants are definitely the right choice. You will also need some form of mastopexy (lift).

Breast implant size is a choice for your body

My personal belief is – it’s your body, you are the one that will be carrying them around and have to live with them and it is you that is paying for the implants and surgery, therefore my goal is to match the size that you want.

That is not to say that I would do something unreasonable, but I will make every attempt to know in advance before surgery, using pictures as well as trying implant sizers in your bra to know what you want.

Then I will try to get as close to this as possible using the many different implant options, including the different profile implants, to match my understanding of what you as the patient wants.
Stop breast feeding before surgery.

I usually recommend patients stop breast feeding 3-6 months before surgery. This time should allow your breasts to settle into the post breast-feeding phase and you will be ready for surgery.

Most importantly, consult with a board certified plastic surgeon who is experienced in breast augmentation.

The best options for natural looking breasts are usually silicone implants

There are a number of factors to consider when speaking about “natural” look and feel. Starting with the images that you have posted, the one thing that can be said about your breasts and chest wall tissues, is that there are very thin tissues to cover an implant, and therefore, any implant that is placed will have a high probability of being seen and felt.
Thus, in my opinion, saline implants have no role in a young lady such as yourself, if you are trying to achieve the most natural looking and feeling breasts. Now, having said that, I need to dispense with one small issue before I go on further.

When silicone gel implants were reintroduced to the market for all primary breast augmentations, they came with a recommendation by the FDA that they not be used on patients under the age of 22.

There are many surgeons who will interpret this guideline as an absolute rule and refuse to offer you silicone gel filled implants.

In reality, that guideline is just that, and the way these things work is that we physicians in this country have the right to use our medical judgment on a case-by-case basis to provide the best medical care to our patients.

Thus, assuming that my physical exam of your breasts would corroborate what I see in these images (and I would expect it to), I would find that in a young lady such as yourself, with thin skin and tissues to adequately camouflage a saline implant, and tight, constricted lower pole tissues, a cohesive gel implant would provide the best results for you and be least likely to result in re-operation for dissatisfaction.

I would offer you silicone implants despite the general FDA guideline based upon medical necessity. Having said that, I would strongly encourage you to consider the use of a shaped, cohesive silicone gel implant, and there are a few choices in manufacturers and many different profile types and dimensions available to choose from.
The one thing that they all have in common though, and which makes a shaped implant ideal for a lady like yourself, is that they have less projection at the upper part of the breast and more at the bottom and behind the nipple, so they will provide a much more natural appearance and less obvious roundness and over-projection of the breast.

They all produce great results, which are every bit as soft and natural in feel as natural breasts, and there are subtle differences between the different types of shaped implants that can be used to our advantage.

That is why, if you are considering this as an option, it is important to find a board certified plastic surgeon who has experience with all types of shaped implants so that you will get complete information about all options available and the best option for you individually can be selected.

Concerns about rotation and other similar problems associated with shaped implants are usually expressed by those surgeons who have little or no experience in using them, as I have not had any problems like that in my practice.
