Treating lines on arms
Sep 19, 2019
Treating coarse lines on the arms and legs is tough. Skin there tends to be more resistant to treatment than the skin on our faces. However, I have seen people get improvement with ablative lasers. Lines can get finer and photo aging and overall skin tone/quality improves with several treatments. That’s where I would start- a reputable dermatologist or plastic surgeon that has a good ablative laser. Hope this helps!
Can scars after 4 years after a neck lift be improved safely?
Necklift scars
Sep 17, 2019
Unfortunately the scars behind the ear can occasionally get a bit wide with the tension that is placed on that area.  The good things I see about your scars are that they remained flat and faded well.  Scars can be excised (cut out) and then repaired- often under local anesthesia.  We can’t always promise perfect scars that way but perhaps the tension will be different and the ultimate scar could be somewhat improved.  As far as improvement in discomfort/itching, that will be an unknown.  Wi…
Cohesive gel breast augmentation over 800cc?
Large implants
Sep 5, 2019
In the US, the largest silicone implant on market is 800cc. Saline implants can be overfilled for more volume however, if the volume exceeds the upper end of the manufacturer’s recommendation, the implant warranty is voided. So, you have no protection in the event of early rupture. In the past, occasionally implants were stacked on top of one another often times with bizarre looking results and hefty costs for revision surgery.Long term, there is only so much our skin can take. Heavy i…
If I have surgery and my weight continues to change are my breasts going to expand again?
Result and weight changes
Sep 3, 2019
I do admit to patients that their surgical results (of any nature) will be more predictable and longer lasting with weight stability. Most adults do fluctuate a bit and I don’t think small changes in weight make much difference because of how weight is distributed. However, the larger the fluctuations, the larger the effects on the skin with loss of skin tone inevitable over time. As far as breast size, some women note changes in their bra size with weight gains or losses as little as 1…
Can you advise on a mole on side of my face that is getting pretty big?
Changing lesion on face
Sep 2, 2019
Hello there! It is very important to recognize when a lesion on the skin is changing and that almost always warrants a visit. That looks like a SK, a seborrheic keratosis. Sometimes they grow in response to injury, irritation, or with time. But, seeing a dermatologist can better assess in person and determine if excision is best. Offices frequently have reasonable self pay rates for such mole/lesion excisions and things like this can be taken care of under local anesthesia in office much…
Upsizing after lift and augmentation in 2017/2018. Is it possible to get my “goal” look? (
Lift revision
Jul 20, 2019
It looks like you have the volume and essentially the shape of your goal look.  And yes, your surgeon can tighten the skin at the base of the breast to make the nipple position appear to come down and bit and shorten the lower pole of your breast.  Best of luck to you!

Mommy makeover after weight loss
May 13, 2019
Hello and congrats on your weight loss and health journey!  I agree that your planned weight loss will get you to a very reasonable state to undergo the surgery you desire.  At 190 lbs, you will have a BMI of just under 30.  Your overall health status sounds great- just don’t take up smoking of course!Your weight loss will change your skin and photos so, we may not be able to give you a complete breakdown of what you will need based on today.  But, a tummy tuck with some limited lip…
How common is suture migration?
Suture spitting
Jan 10, 2019
Unfortunately we all see this.  Those buried sutures that are supposed to dissolve underneath your skin without an issue will sometimes poke up through the incision line.  When they do that, they can cause further inflammation at the incision line and sometimes create a small wound around it.  Its a good idea to have your surgeon clip those out for you so that your incision can continue to heal.
Is getting a breast augmentation on accutane okay to do?
Accutane and surgery
Dec 29, 2018
The test answer on our plastic surgery exam was always to wait 6 months after stopping Accutane for elective surgery. Most of the concern rests with issues related to wound healing especially large areas of skin resurfacing. That’s a bit different with a straight forward breast augmentation. Many dermatologists are offering different opinions on this question and have said that it’s likely safe to do such an operation within 1-3 months of stopping the drug. I’d suggest talking to your derma…
Are my breast implants saggy at 2 weeks post-op?
Postop time after breast augmentation
Dec 10, 2018
Thank you for the question!  Unfortunately, your postop photo is very dark on my screen and I can’t see much.  But, I reassure patients that you and your breasts need some time for adjustment following breast augmentation surgery.  It may be two-three months before you can decide exactly how you feel about your outcome.  Some patients may not be able to achieve as round and high an appearance as they were hoping (sometimes due to length of chest, skin quality, existing shape of breast tis…
Does it seem as if I have enough breast tissue to do above muscle?
Breast augmentation with minimal ptosis
Dec 10, 2018
Hello and thank you for the photo.  You appear to have minimal true ptosis.  Your breast just appears a bit deflated.  And, you appear thin.  For several reasons already discussed, in my practice, I would recommend a submuscular approach.  The saline vs silicone option is completely up to you of course.  But, with thinner tissue, you will have greater risk of seeing ripples/wrinkles and having a less natural feel to the implant with saline.
Is a breathing tube down the throat done with breast augmentations?
Anesthesia with breast augmentation
Jul 19, 2018
Thanks for your question.  In short, it depends on how the anesthesia is conducted.  Breast augmentation can be done under sedation without a tube used since you are breathing for yourself with monitoring.  It may also be done under full general anesthesia using an airway (LMA frequently versus an endotracheal tube).  I often times do the surgery with sedation now and find it serves the patient well.  Occasionally, patients may not be safe for sedation due to airway concerns (like known sign…
Serous fluid coming out of incision 5 weeks after breast augmentation. Is this of concern?
Incision drainage after breast augmentation
Jun 22, 2018
A small bit of serous drainage in the first days after breast augmentation can be completely normal but drainage now after weeks of healing should definitely be investigated.  It may be nothing more than a buried suture working its way out but it can also indicate issues like a fluid collection around the breast implant especially if that side seems more swollen.  So, please check in with your plastic surgeon.
Unsure of uneven small breast augmentation. Will this eventually look better or am I going
Early postop result
Jun 21, 2018
You look terrific for 8 hours postop.  Please give the result some time to develop.  The skin and tissues need several weeks to relax and settle with the implant.  I’m sure you will hear similar feedback from your surgeon.
Will Breasts Sag if I Decide to Explant?
Change in breast after implant removal
Jun 20, 2018
The volume of the breast of course always diminishes after implant are removed and this can make someone feel more ‘saggy’ after such a surgery.  Everyone is a bit different and your skin elasticity depends on a few factors–  how large the implant size, aging, genetics, weight loss, how your own breast tissue has changed.  Women with great skin tone and small implants may look very much like their preop breast after implant removal.  Some of it is an unknown and many times I remove imp…
