I would not recommend undergoing any sort of cosmetic surgery on your abdomen if you have have an ostomy on both sides. The risks outweigh the benefits in your situation.

It should not affect the tt to have an umbilical hernia repaired afterwards. Ask that the surgeon use the already existing incision around the umbilicus so you dont have any new scarring.

The only truly effective treatment for loose abdominal skin is an abdominoplasty. RF and lasers may improve the skin somewhat but will not be as dramatic as surgery.

The tightness you are feeling is likely due to some scar tissue developing. Everyone develops differing amounts of scar tissue and you may develop more than the usual person. Having said that the way to combat this is deep massage of the area and stretching/exercise to soften the scar. The more you massage and use the muscles the more collagen will be converted to softer more organized fibrils of collagen and will make the scar more flexible.

I would consider it safe to proceed with abdominoplasty with a small hiatal hernia that your gastroenterologist is comfortable watching. You should take the pantoprazole prophylactically for a week before and several weeks after the procedure to prevent any worsening of reflux due to increased pressure in your abdomen after the TT. There is usually only a need for mesh if the muscle cannot be brought together in the middle during the diastasis repair. This situation is rare, especially in a very fit patient like yourself. I would expect to be able to repair the diastasis which should correct the midline hernia as well. You will have to have a strict lifting limit and avoid any core straining for 6-8 weeks afterward to allow the muscle repair to heal.

It is fine to take it up to the day of surgery and afterwards but you should be sure to stay very well hydrated postoperatively as spironolactone can cause you to be dehydrated.

Ultherapy uses ultrasound energy to tighten the skin. This should not cause issues with the plates and screws that are in place. You should be fine to have this treatment.

The only filler for the breasts is fat. Sometimes it takes several rounds of grafting to get the desired result in breast reconstruction (and augmentation), esp if the patient has very little fat elsewhere that is available for transfer. Continue to see your surgeon and follow up after the procedure.

If you have a deep plane neck lift the muscle layer of the neck is tightened along with the skin. The muscle tightening will not be affected by weight loss as there is not a lot of fat below the muscle. Skin can loosen if there is subcutaneous fat (fat between the skin and muscle) that reduces after weight loss.

Yes it can is the bottom line. Numbing cream helps but it does not eliminate the pain but does help. You should be able to tolerate it esp if it is done at a lower setting.

Based on your pics I think a traditional TT would have a great result. Your surgeon just needs to be sure to get all the loose skin laterally removed and tightened.

I dont think a lift will improve your result much. You have a very natural result and your nipple position is good over your implants.
