Size of breast envelope
Breast sizing is a very individual and specific question. The answer should be given to you after a clinical exam. In general, breast implant sizes are chosen based on breast diameter, the amount of existing breast tissue in the patient’s desire in terms of a cup size.
How to choose a natural size breast implant.
In my opinion and in my practice 450cc is a large implant so I share your concern. A 350cc implant should increase your cup size by 2 in the same brand and style of bra that you currently wear.
Please see a board certified plastic surgeon for an exam and sizing evaluation. Specifically ask the doctor how he/she will address the asymmetry. If you want your breasts to match you may need a small lift on the larger lower breast and possibly a slightly larger implant on the smaller side.
Be aware that trying to correct the asymmetry with a larger implant on the smaller side only, without a lift on the larger lower side could make the asymmetry more noticeable. You need to find an experienced breast plastic surgeon who can address the asymmetry.

Implants and a lift
It looks like an implant may give you more volume and a small lift would be beneficial.
You need to do some homework for your answer
All saline implants come with a suggested range of fill volumes. Some surgeons regularly push the upper limit of the possible volumes while others stay at the lower end. The implant manufacturers will permit a little overfill but if too much is put in you can have your warranty voided.
The homework is checking with your surgeon. You need to know the range of volumes for your size implants and the actual volume put in at the time of surgery. You can probably go about 10% above the manufacturers maximum.
If this difference (actual volume to maximum potential volume) is not large, it wouldn’t justify the risk of potential complications. For regular sizes I would suggest that less than 50 cc would not be worth it.
In my practice I discourage any corrrection of size before a full 3 months has occured. Several well done psychological evaluations of breast patients support the idea that the minimum time to take “ownership” of your new breasts is at least 90 days.
Augmentation best when lactation stops
In order to get the best estimate of final breast size during augmentation it is best to allow the breast to return to a natural state as you describe. This will give a true picture of the natural breast concerning size, symmetry, nipple position, and shape and give the best prediction of the long term result anticipated from your augmentation.
Lactation of the breast is maintained by stimulation of the nipple and gland causing a let down reflex or cascade of homones from the brain that stimulate lactation. To stop the cycle the old remedy is a tight bra, ice as needed, and of course minimal breast stimulation.
When the cycle is broken and lactation stops, the glands then shrink and the breast assumes its natural state. You are almost there and perhaps one more month should suffice. Augmentation can be completed safely even with very modest expressable milk, however there is one unique complication which can occur.
The surgery will stimulate the breast and may start a short burst of lactation. In the pocket where the implant is placed the secretion or milk can collect. We call this a galactocoel. The discomfort, swelling, and time to resolve this issue can be frustrating.
Again, best to wait just a bit more.
Lift with implants
You failed to mention what your expectations/desires were. Allow me to be blunt and to the point. You need a lift. If you were to have implants alone (I recommend subpectoral-always) you would likely end up with what we call ‘double bubble’.
Not a pretty look. Please do not fall for “an implant will take up the slack” explanation. Implants DO NOT lift breasts. I’ve been doing this for many years and seen it time and time again.
Women who get large implants to ‘take up the slack’ are very disappointed and upset. Large implants will stretch skin and tissues making matters worse. What I would suggest, (if you desire additional volume) is to place a small sub-pectoral implant and reposition your existing breast tissue over the implant through a circumareolar incision so as to minimize scarring.
OR, if you are happy with your current volume, I would again reposition your existing breast tissue so that it is higher through a circumareolar incision. This will align your areolas at the center of your breast mound where they are supposed to be.
Can I avoid a breast lift?
Your breast are fairly low set. Ultimately a breast lift is the best option with implants for optimal positioning on the chest wall. Under some conditions, you can have a sub glandular breast augmentation that augments the breast.
But in this case your breasts still hang low. Lift is limited unless you over size your implants. It may be better to wait until you have your kids and then get a breast augmentation and lift to restore your bustline.
Breast augmentation and implant size
It’s hard to assess this accurately without an examination, but 400 cc implants in your body size are almost certainly going to be very obvious with some bulging at the top of the breast. If this is the look that you are going for, and you accept that your skin will stretch with time secondary to the weight of the implants, then you will probably be happy.
If you want the breasts to be filled but not bulging, then you will probably need smaller implants. I would suggest discussing this with your surgeon to ensure that you are both on the same page in terms of the look that you hope the achieve.

If, however, you are referring to the shaped, “gummy bear” implants, I would caution you on the necessity of going out of your way.
An implant with inherent shape can shift in orientation, leaving you with an un-natural appearing result. Many of those with access to these implants choose not to use them outside of the reconstructive cases.
Wait 6 months after breast feeding to do breast augmentation.
1) Unfortunately, you are not a candidate for just breast implants. You have too much sagging. You need a combination breast lift and breast augmentation.
2) No surgery for 6 months after you stop breast feeding. Your breasts will change, and your hormones need to settle down.
Implant profiles
Hard to determine which style is best for a patient without pictures or measurements. Generally I use Allergan Inspira implants and they come in moderate and full profile and an extra full profile. I use moderate for more natural look and the higher profiles to get more projection.
But it really depends how much breast tissue you have and the skin envelope and the chest wall diameter. Every woman is different. Look for a board certified plastic surgeon.
What type of breast surgery is the best option for an athletic 35 year old?
Your nipples and areolae are above the level of your inframammary fold, so you really don’t need a periareolar lift. Your breasts demonstrate atrophy and deflation that often occur after breast feeding children.
Restoration of your lost volume with implants alone should be able to provide you with natural-looking, more perky breasts that you desire.
Breast lift
Many women are in between needing a lift along w/ their augmentation vs. avoiding the additional scars, recovery, and costs. Yes, adding an implant almost always provides a small lift of the nipple position, just by filling out the skin envelope you have.
But, if it requires an overly large implant to accomplish that, you are better off opting for the lift too.
