Statement: Dr. Mark Deutsch is a double board-certified plastic surgeon and the founder of Perimeter Plastic Surgery in Atlanta, Georgia. He performs an extensive array of surgical cosmetic and reconstructive procedures ranging from tummy tucks to breast augmentation. He is known for his dedication to educating and guiding patients to ensure the best possible treatment and most natural results.

After receiving his medical degree from the University of South Florida Morsani College of Medicine, he went on to Louisiana State University Medical Center/Charity Hospital in New Orleans, Louisiana for his residency in general surgery. Following that, he completed fellowships in plastic surgery at Albert Einstein College of Medicine/Montefiore Medical Center in Bronx, NY; microvascular Surgery at M.D. Anderson Cancer Center in Houston, TX; and breast, head, and neck cancer reconstruction at M.D. Anderson Cancer Center.

The postop restrictions are based on the procedure you are having; for most procedures we like patients up and walking soon after surgery and patients can drive as soon as they are off narcotics. For more extensive procedures we recommend no heavy lifting or exercise for up to 6 weeks after surgery. It is best to review these with your surgeon

Thank you for sending your question and pictures, you appear to be a good candidate for several procedures including breast augmentation with a breast lift, full abdominoplasty and liposuction of your flanks and lower back. You did not mention your BMI, keep in mind your results will be improved as you get closer to your ideal/goal body weight. Best to find a plastic surgeon who you are comfortable with and discuss your goals during the consultation. Good luck!

Thank you for sending your question and picture, you do appear to have enough breast tissue to "hide" an implant placed above the muscle. Keep in mind that your chances of implant visibility and/or capsular contracture may be increased with this approach but you also will avoid the "animation deformity" caused by placing the implant under the muscle. Best to discuss in detail with your plastic surgeon, good luck!

Thank you for your question and sending your pictures. You did not mention the cup size you are trying to achieve, I am concerned that unless you choose a very large implant you will not be happy with your results. Your breasts are ptotic and wide and you appear to have a wide sternum. I would recommend a small breast lift with your breast augmentation and the cosmetic results should outweigh any possible decrease in nipple sensation

I feel you are a good candidate to proceed with surgery at your current BMI. Keep in mind you can expect an even better result as you get closer to your ideal/goal body weight, it is best that you discuss the specifics of your surgery with the board certified plastic surgeon that you choose to do your surgery. I would recommend a breast lift (with implants if you desire a larger cup size or more upper pole fullness) with an abdominoplasty and thigh lift. Bowel perforation is a very unlikely complication with these procedures. It is best that you go on as many consults as you need until you find the surgeon you feel most comfortable with. Good luck!

You do appear to be a good candidate for a breast revision. The procedure could involve scar revision with possible fat transfer and Kenalog injection if your scar is wide and thick. It is best that you meet with a plastic surgeon in your area who can meet with you and develop a treatment plan with you. 
Is it normal for my implants to still look like this 6 weeks out? What can I do?

I understand your concerns and it is important to be patient. It can take 3 months for implants to drop in their pockets and you are only 1 month postop. I would recommend you follow your surgeon's advice and recommendations for postop care. I would give it another 2-3 months before considering a revision.

I feel that a breast lift with a small implant would solve both issues, if you are not interested in a larger cup size then perhaps removing some breast tissue with implant/lift would be ideal for you. It is best that you see a board certified plastic surgeon who can examine you and discuss your surgical options in detail

Thank you for your question and pictures and congratulations on your weight loss. I would be concerned with inserting such a large (600cc) implant and performing a lift at the same time. I feel you are at lower risk for wound healing problems and "bottoming out" if you have this done in two stages. Make sure that the surgeons you are seeing are proficient in performing cosmetic breast surgery on patients who have had massive weight loss and you are comfortable with their plan. Good luck ...

Yes, scar massage is vital to achieving a good result postop. There are various scar creams and gels and tapes you can place over your scars and I like to start this 3 wks. postop. Please consult with your plastic surgeon to help guide you with the best treatment for your scars
I have seen 2 surgeons. One has recommend a tummy tuck and one has recommend a lower body lift.

Thank you for your question and pictures, a side picture or picture from behind would be helpful. From the views seen you are a good candidate for a tummy tuck and I would extend the incision as far as possible to remove the excess skin over your flanks. This should give you a nice result in the front of your abdomen and if you desire a tighter outer thigh/upper buttock area you can always have a later procedure.
