First, you should see your private physician to see why you are anemic. If low Iron, then either iron pills with Vitamin C will help or even Iron injections work very well.
I personally do not have issues with a healthy individual having a mommy makeover with a starting hemoglobin of 10.4. But I always start my patients on iron and vitamin C pre-op when anemic.

Advice for achieving the best anchor breast lift scar possible?

I start by using the shortest scar possible. I try not to extend it laterally as this is the most common area for hypertrophic or keloid scars to form. Next, I use intradermal sutures (this avoids the needle holes that go on the outside of the skin on either side of the incision. Lastly, I use paper tape for a few weeks till totally closed and then switch to 3M silicone tape which I continue for several months.

While 405 is a good size, it really depends on what you want to look like. I have patients bring me ‘wish pics’ of naked breasts. Then I order several sizes and use a sizer during the procedure to make sure I use the correct size. Many times, the rib anatomy is different and a larger implant was required to obtain the correct results. You may be able to increase by 30-50cc’s.

You look great now, but if larger breasts are desired, a breast augmentation would solve this issue.
You belly also looks good, but you have some extra skin and a little fat. I have to offer an abdominoplasty in someone with your abdomen, but it appears you have loose skin above AND below the belly button which makes you a candidate for the tummy tuck (abdominoplasty)

Do I need just a lift, or a lift with implants, or just implants?

You have ample breast tissue, just downward a bit. I feel a periareolar mastopexy (breast lift with scar located only around the areola) would make your breasts a bit rounder and perkier.
A lollipop mastopexy (breast lift with a scar around the areola, and one going from the bottom of the areola to the crease) would give you a much tighter lift and rounder breasts-with more upper pole fullness, but the vertical scar is the tradeoff.

At one year post augmentation, your breasts have fully settled.
it would be fantastic to see your pre-op photos to compare, but based on what I see now:
The scars are fin. If the one side still bothers you, a small amount of Kenalog 10 (weak steroid injections) may help it.
The one breast is higher but it appears that might be the correct height and the other has descended too low?

Unfortunately, I would need to perform a physical exam to accurately answer your question. An educated guess, I feel you did require a plication (very common to realize the rectus is loose while under anesthesia and open) and when you pull the center muscles closer, it may cause a bunching of the skin and fat in the central region. If your ‘lump’ is soft, it is excess fat which may be liposuctioned.
I would go for another consultation from a board certified plastic surgeon for a second…

Easy answer. IF the abdomen bothers you and your Plastic Surgeon feels you are a good candidate (which I feel you may be), then go for it. You will be happy with your flatter belly. When you get pregnant, your abdomen will not get as large (less fat cells to fill).

Fat transfer to the breasts appears to be a very safe (and somewhat effective) procedure. The mortality rate is extremely small and I have not heard of a death following a fat transfer to the breasts.
Normal complication exist:
Bleeding, infection, scar tissue formation, fat necrosis, asymmetry, fluid accumulation (hematoma or seroma). All of these complications are very rare.

Your photos are very difficult to judge due to the pixilated areas.
It is much cheaper doing multiple areas of liposuction at the same time so I would say to save up and do the upper lateral back and arms at the same time (they also use the same incision -point of entry).
If you have to decide, then choose the area that bothers you most.

Your breasts look very nice now (18 years post augmentation). I do not feel that your nipples are high, just a bit lateral which is how you are built. Don’t consider a lift if you like them now, just replace the implants as they are well past the 10 year mark (not all impalats must be changed, however, at 18 years old, they are past the normal age of replacement)
