“Natural” looking breast augmentation

The word “natural” is so much overused and misunderstood by patients with respect to breast augmentation. It should be replaced by describing honestly how much “fullness” you want in the uppr pole.

That is because a “natural” result technically describes “what could occur in nature” while often what a patient means by it is just something that doesn’t look like Pamela Anderson.

In no way, could nature give you the look that a 400 cc Moderate Plus implant will give. Consequently, it won’t be “natural.”

You will have some excess upper pole fullness as is intended by this implant profile.

While you may achieve the cup size you want, at 105 pounds, you likely have extremely little breast tissue and about 90+% of the result will be implant.

As such, it won’t look “natural,” it will look like an implant.

So, go for the “look” you want and ask to be shown photos of patients like yourself and see if they got what you want. An experienced breast augmentation surgeon will surely have photos of people like you to evaluate.

Why not use tiny Breast Implants

The choice of implant size in my practice is dependent on the chest wall size, other physical characteristics of the chest, and the desires of the patient. Basically, implants less than 200 cc’s would be used in a very tiny patient (as a guess, ~ 26′ band size?).

The most common bra size in my practice is is 34, so very little need for such a small implant — it won’t look good or do the job. And yes it is true that we like things bigger here in Texas, but, when it comes to implants, we prefer harmony, balance, and a good aesthetic sense.
Silicon implants have been in use well over 40 years for breast augmentation and there has been absolutely no evidence that they lead to any increased risk of breast cancer or any other type of cancer.

In fact, there is some evidence of a reduced risk of breast cancer in some age groups in women who have breast implants. Nor do implants inhibit the diagnosis of breast cancer provided the patient follows the suggested guidelines for routine breast screening.

Implants vs Breast lift

Your goals are the most important when considering what is best. I see numerous patients that have been to other doctors and have many different opinions on what to do. It always comes down to size and shape.

Shaping for you is lifting, size is implants, or both either staged or at the same time. Scars on the breast vary with the patient and they fade but don’t disappear. This is why I spend a long time talking with patient ahead of time to get their goals and what is their best treatment.
It is not unreasonable to put in implants without a lift with the idea a lift could be done later.

Breast Asymmetry

You are right and I’m can easily see your asymmetry. Swelling is part of recovery, perhaps not enough to account for that discrepancy. You can be corrected and you can feel good about yourself again.

As with all cosmetic surgery you need some time between first surgery and the correction. It is far too dangerous to correct your asymmetry at this time. Generally I will wait three months. This will allow more time for the soft tissues and skin to relax and hence make the correction easier on your tissues.

Could I get away with breast implants without a lift?

When a saline implant loses its saline it usually happens pretty quickly and the saline is absorbed. There is no urgency to remove the implant; however, if replacement is considered the sooner the implant is removed and replaced the better the condition that the previous pocket will be.

After loss of volume the pocket begins to scar down and requires extra time to reopen the pocket for a replacement implant.

Recovery after BA

Every patient handles this recovery a bit differently. However, as long as you are not requiring narcotic pain medication, and you felt relatively comfortable, I would probably let you go back to work after a long-weekend recovery (3-4 days).

It’s better to plan on needing more time though, so you won’t be disappointed.

How much is a cup size?

It is not easy to answer this concisely. The cup size system is not ideal, scientific or consistent and I wouldn’t get too hung up on it at this stage. The size difference between cups depends on your back size but there is also huge variation in how you are measured and how that translates to different manufacturers.

I would rely on sizers, bags of rice or, ideally, 3-D imaging which is rapidly becoming the gold standard. I have attached a link with some more information.

Implant Profile and Body Shape

It is impossible to recommend implant without seeing a patients body form or shape. Profiles increase projection and look different on different bodies depending on shoulder width, muscularity, base diameter of the breast and desired cup size.
I use a sizing system to let patients see what the implants look like on them and show moderate and high profile on their body frame. You can look at other patients results based on height, weight and athletic build similar to yours on line and print the pictures you like with implant information they used as a tool for your plastic surgeon to help you decide.

I understand where you’re coming from – it seems every time I get a new gizmo, a better one is announced (sometimes even for less money!). You need not worry in the case of breast implants, though.

I say this because the implant designs and manufacturing techniques are constantly being improved by the major manufacturers in the US- and the most recent, cohesive silicone gel designs are the most natural feeling implants we have ever had access to- with the most strongly proven safety profile as well.

So while the implants may get better over time, I do not think the improvements made over current implants will be so dramatic as to render them obsolete any time soon.

Placement of Implants Above or Below Muscle

I very rarely place breast implants above the muscle even though I routinely placed them there several years ago. I definitely feel that you will be happier if your implants are placed beneath the muscle, but there will likely be a bit more discomfort for the first few days.

Perfect fit

There are many perfect fit breast implants for your body but some will help you achieve your desired size and shape. In general, the larger the implant, the wider the diameter so most of us would increase the profile to keep the diameter within your breast dimensions.
This is done to balance the risk of uniboob and lateralization of your breast implants. Obviously this is an overly simplified version of what I do when selecting your breast implant because I have to consider what your goals are.

What type of breast surgery is the best option for an athletic 35 year old?

Whenever I hear- the word “perky”- I discuss primarily a lift. It appears that your sternal notch to nipple distance may put you in the lift category. This – in conjunction with your 285 cc silicone gel recommendation in my opinion would give you your best result.

Which cup size should I go with?

The best cosmetic result in any particular breast augmentation patient depends on a variety of factors, including: your individual anatomy, realistic expectations, a thorough discussion with the plastic surgeon about the options, and an understanding of the pros and cons of any particular implant choice.
Proper sizing is not just about the number of cc’s. The thickness of your tissue, breast dimensions which include the width, height, and projection, as well as chest wall width all need to be considered when choosing an implant.

Trying on implant “sizers” of various shapes and volumes while wearing a tight t-shirt, bra, or bathing suit at a preoperative visit will help you and your surgeon choose the optimal implant. There is very little difference in the ultimate result between the 2 sizes that you are contemplating.

If the implants are going to be under the muscle, they will look slightly smaller than the sizer will look that you can try on in just a bra or t-shirt. Keep in mind that following the advice from a surgeon on this or any other website who proposes to tell you what to do without examining you, physically feeling the tissue, assessing your desired outcome, taking a full medical history, and discussing the pros and cons of each operative procedure would not be in your best interest.


I would suggest that your plastic surgeon be certified by the American Board of Plastic Surgery and ideally a member of the American Society for Aesthetic Plastic Surgery (ASAPS) that you trust and are comfortable with.
