Largest Breast Implant size
Breast Implant size depends on many factors:
1. Your skin elasticity
2. The dimensions of your chest and breast
3. The breast tissue you have to cover the implant
4. How fake or natural you want to look
These are some of the factors we take in cosideration when we recommend an implant size. The larger the implant to your measurements the more the complications are, and the more frequent you will need further surgery.
YOU ARE FULLY RESPONSIBLE FOR ALL FURTHER SURGERIES. Going from AA to DD the breast implant will be at your collar bone and under your arm pit.
Is that what you want? If so you will require two surgeries: One to put a tissue expander, to stretch the skin then another surgery to put the supersized implant (800cc). But you will destroy your body image and beauty.
You may find somebody to do that. On this thread there are the best Board Certified PS who are telling you it is wrong.
Do you still want it? Even if you are a strip dancer (I have many) you probably do not want your breast destroyed.
Breast Implant Sizing
I would recommend that you “try on” the implants. I routinely have a sizing session with my patients at which time they can see how different implant sizes change their profile, etc.
This has led to a very incidence of changing the size of implants during the several years I have been in practice.
Breast surgery
You need a full breastlift as well as an augmentation I would encourage you to see a board certified plastic surgeon to find out what your options are.
Best breast implants
Unfortunately Syla, there are no best breast implants for saggy breasts. The implant is the device that gives volume to the breast, and while I am a firm believer in silicone I am also a clinical investigator for a new type of implant called the Ideal implant.
It appears to me that you will need a combination surgey of a lift plus implants or an Augmentation Mastopexy. Very often I do these with just a scar around the areolar and sometimes using a vertical scar as well.
When I do a lift, most of the time I use silicone unless a patient specifically asks for saline. I find the saline implants tend to bottom out too much. I hope this helps you.
Breast implants OVER the muscle will correct slight sagging.
1) Breast implants will make your breasts significantly larger. If you are already a D cup, that is not a good idea. Very large breasts age poorly. So I don’t recommend implants in any position for you.
2) Breast aumentation over the muscle, or with a dual plane approach, will correct mild sagging, and is a good idea for women with smaller breasts. Breast implants behind the muscle do not give women with sagging a good shape.
3) I know you are afraid of a breast lift, but it is probably what you need (either with a scar around the nipples or with a lollipop scar). Women accept breast lift scars just fine if they love the new shape of their breasts.
4) One of the most common mistakes that I see in New York City is the woman who had implants to correct sagging, and now has very large, deformed, still sagging breasts.
Saline implant removal
I am surprised that an MD would suggest admission to a hospital to what amounts to a typical outpatient procedure unless there is something in your history that you have neglected to say
Dual Plane or Breast Lift
It is clear that you have grade 2 ptosis and need a lift. An implant will only fall into the excess skin no matter what dual plane technique is used. Without a lift you will look ok for the first 2 months – which is when most surgeons will show their results.
Make a surgeon show you similar patients 1 year from surgery with an acceptable outcome. You should see a board certified plastic surgeon in your area to get a formal opinion through an in-person consultation. ,
Even Small Saline Implants More Likely to Ripple
All saline implants are more likely to ripple than silicone implants are. The fact that you are having your implants placed below the muscle makes the rippling less likely to be seen or felt, but the chance is still greater for rippling with saline.
Implant size too small?
It depends! What is your desired cup size? Generally count on about 200 cc for each cup size increase. That doesn’t necessarily mean that you can have 800 cc (four cup sizes larger) implants if you don’t possess that anatomy to accommodate such an immediate increase in size.

650 cc is too big for you.
You have saggy breasts and the right one is worse than the left. You also do not have very much breast tissue to handle 650 cc. You have constricted folds which means unless you do a lift you will have to lower the folds on both breasts.
Lowering the fold has to be done perfectly so the old fold disappears otherwise you will have a permanent double bubble. Also when you lower the fold the area of the old fold will be quite thin.
The larger you go the thinner this area becomes and you will start to feel the implant more in the lower pole. This can be an issue with some patients. Also saline implants generally ripple more than their silicone counterparts.
so if the lower and lateral portions of the tissue are thin you may experience rippling when you go 650 cc. Usually the HP saline implants ripple much less than the MP or MP+ type. Finally I see your abdomen has fresh stretch marks.
This means you have the type of skin that would develop new stretch marks if you go 650 cc. I would advise you go smaller the first time.
Is it possible that I can get an augmentation without a breast lift?
It is likely that a combination breast lift with implants will give you the best result. For more information on breast implants with lift please read below .
Lift needed with augmentation?
I believe you will be best served by a lift with an augmentation. Like you mentioned nothing beats a face to face consult, but you sent good photos that show a fair amount of skin laxity and without a lift your implants will be too low and likely to lateral.
A lift will permit not only raising the position on your areolas it will allow them to be moved more medially, and the lift will remove the lax skin that will relax too much more with just an implant alone.
Breast implants over the muscle better for body builders.
1) I don’t know exactly how much you exercise, or how overdeveloped your pectoral muscles are, but if you have a very muscular chest, over the muscle breast augmentation is safer.
Get a plastic surgeon you trust to make the right decision.
Will an implant replaced my lost fullness and not increase my sag?
You are an excellent candidate for a breast augmentation alone which will restore the volume lost with pregnancy and breast feeding. By the way there is no sagging! Visit with two or three plastic surgeons in your area before making a decision about how to proceed.
Buyer’s remorse?
Please be patient, follow your PS’s post-op protocol and realize that once the overlying breast flap (skin & tissue) + muscle relax, your result will be softer, mobile and prettier. I usually forecast approximately 7-10% shrinkage, but remember that silicone gels require slightly longer than saline implants to settle.
Your photo shows implants, which fill your chest, provide a soft round “cleavage” and do not protrude excessively off your rib cage. I would guess that you had relatively tight skin beforehand, (i.e not prestretched by pregnancy/lactation or weight fluctuations).
What size did you discuss with your PS, pre-op? In the event that you decided to downsize, wait at least 3-6 months before proceeding, in order to minimize wound healing issues.
