The risks of any surgery increase with a high BMI.  Risks such as wound healing abnormalities, infection, seromas, DVTs and many other complications are significantly increased with a high BMI.   In addition to that, you have nerve damage, multiple hernias, and mesh which all add to the increased risks.  Reduction of your weight may reduce the incidence of complications and would certainly also improve the aesthetics of this type of procedure.

Patients should wear bras immediately after surgery.   A sports bra may be more comfortable initially because underwires may irritate the base of the breasts.   After 2-3 weeks, an underwire bra is well tolerated.   Sometimes an ace wrap maybe used to reduce localized swelling in addition to a sports bra.  Patients should sleep in a bra during the first several weeks as well.

Sientra implants are excellent implants.   The company has one of the best warranties and the implants are soft and natural feeling.  That said, all three of the major companies, Mentor, Allergan and Sientra make excellent products, have good warranties, have gummy bear (solid gel) formulations, and are well respected by board certified plastic surgeons.

The patient in this photograph does not look like she has tuberous breasts.   Tuberous breasts typically have a long, narrowed shape with an enlarged areola and a very high inframammary crease.  Patients with this condition frequently have asymmetry and the breast tissue seems to protrude through the nipples.   Patients with a tuberous breast deformity sometimes need a mastopexy to decrease the size of the areola and reshape the breast at the time of a breast augmentation.

Breasts are certainly changed after giving birth and breast feeding.  Sometimes the changes to a woman’s breasts are minor but sometimes the changes result in significant sagging, flattening, and stretch mark development.  Because of the potential of skin laxity as well as the possible difficulty with breast feeding, it is usually best to wait until after children to have cosmetic surgery to your breasts.   To answer your question, I would “sit tight and wait”.
