If you are capsulated, then you will need a capulectomy or neo-pectoral placement of the implants. Also I would recommend wider and flatter implants to give you more clevage (closer together) and to get rid of the fake look and make them more natural looking. I would definitely stick with silicone implants, as you are very thin and silicone will help minimize any implant palpability/rippling.

I would want to know why your initial implants didnt "drop". Was it because the muscle fibers where not release adequately, did you develop capsular contracture? With a larger implant, the tissue will stretch to a greater extent because of the weight. So, yes your tissues will stretch more, however, the implant may sit higher as that is likely what you are shooting for.

I would discuss your revision surgery further with your surgeon and make certain you are clear of the expectations...

It is difficult to predict bra size as there are many factors that determine bra sizing. Every company has their own sizing as you may size one way in a particular brand and then another size with another company.
In general most would agree that 180 cc or somewhere in that vicinity will affect the bra size by one size. Now that also depends on the patient, their body and so forth.

The treatment would be to redo the areola and size it down. I generally put a permanent suture around the areola and close it in a way to prevent widening over time. I would do this first and see how things look. If the discoloration is still an issue, medical tattooing can help resolve or improve the issue.

Sorry to read about your issue. Judging by your anatomy, I would have told you that it is going to be difficult to get you a lot of cleavage but by maximizing the base diameter of the implant, being aggressive with the dissection but not overly aggressive, we would achieve as much cleavage as your body allows. It is difficult to give you much advice here as your case would really benefit from an experienced surgeon assessing what was done during an exam and giving you realistic expectations. Judging by your photos, you would need a lift in addition to implants. Implants do not provide a lift and do not bring the nipple/areola up to a higher position. Breast implants will give you the added volume and fill out the skin envelope.
