Choosing Breast Implant Size
The decision on breast implant size should be a combined effort between you and your plastic surgeon. You can use the expertise of the plastic surgeon to let you know what range of sizes would be appropriate for your anatomy.
Breast implant bottoming out
You’ll need to be examined by a board-certified plastic surgeon to know for sure, but it sounds like you may have bottoming out. Bottoming out occurs when the implant slides below the natural breast crease.
That makes the nipple ride high and the upper part of the breast appears flatter.
Breast Lift
An implant alone will not give you a nice result in my opinion. You will need a lift. The implant will sit very low on your chest and not fill out your breast nicely. A 750cc implant is also an extremely large implant and will lead to further problems down the road.
I suggest you go with your gut instinct and seek out another opinion. It is best to be seen in person by a Board Certified Plastic Surgeon to finalize your plan, but based on your pictures and questions, you need will need a lift +/- an implant.
Breast implant & areola reduction without lift?
You can have a periareolar incision performed for placement of your implant, allowing for reduction of your areola in the same operation.
That said, nothing replaces an in-person examination, but in my opinion you would most benefit from a breast lift in conjunction with implant placement, but this ultimately will depend on your desired appearance and degree of breast enhancement.
Be sure to see an ASPS board certified plastic surgeon for consultation.
Removal of saline implants
In my practice, patients who have no underlying medical issues can have their implants removed in our accredited outpatient surgery facility and go home that same day. Breast implant removal surgery without a capsulectomy is scheduled for an hour and then time is needed for recovery from general anesthesia.
I recommend that patients undergo general anesthesia for this procedure so that they are the most comfortable during surgery, however, it can be done under IV Sedation and local anesthesia. If you are not comfortable with the information that was provided by the surgeon you originally consulted with, I would definitely get another opinion from a plastic surgeon who is certified by the American Board of Plastic Surgery.
Implant size change
Without seeing before and after pictures, it is somewhat difficult to answer this question. Cup size is not a good guideline to decide on size of implants because cups vary quite a bit between bra manufacturers.
Using pictures of breast profiles you like can be helpful. Downsizing implants does not always require a mastopexy. It sounds like you probably will not need a lift but an in-person exam would give the best information.
Natural shape with round implants?
I have been using smooth round implants under the muscle for over 25 years. Once the implant is in place the quality of the silicone gel allows the implant to settle with more material at the lower pole, unlike saltwater implants which maintain excessive fullness to keep their shape
From your description it sounds like the goals you have are very achievable with a smooth round implant.
Your best option is to consult a board-certified plastic surgeon with considerable experience in breast augmentation .
Breast augmentation usually done as an outpatient
I would not attempt a submuscular breast augmentation under local anesthesia. Most breast augmentations are performed under a general anesthetic and since the procedure is relatively short, 1 hour, most patients would go home the same day.
The only reason for an overnight stay may be a personal decision or medical complication such as bleeding.
Desire for a breast augmentation?
From your photograph you appear to be an viable candidate for a breast augmentation. A breast augmentation is a permanent operation with life long consequences. If you are really interested then it is time to get a consultation with a qualified board certified plastic surgeon.
It is only through education that you can make the correct decision for yourself.
2 different sized implants
It is difficult without an examination but it appears that your breasts are different in size by enough to consider 2 different sized implants. Discuss this with your surgeon. I would typically bring a few sizes and use tester implants to decide which gives you the most symmetrical result.
Sometimes you end up using the same sized implant, but you never know until you are in the operating room.
You can then try different sizers in the office or the rice test and then pick which size implant you would like.
Treatment of Tuberous Breasts
As my colleagues have stated, the treatment of tuberous breasts is quite challenging. Any surgical plan to address true tuberous breasts require attention to a constricted breast base (where the base of each breast is too narrow) and a widened areola.
An implant can be considered when the patient wants a fuller look. In true tuberous breasts, there has likely been an underdevelopment of the breasts; hences the need for an implant to augmentation volume/size.
In your case, a board certified plastic surgeon needs to performa physical exam to further evaluate the nature and severity of your tuberosity. Based on these findings, he/she will make their recommendations.
Bras after breast augmentation
After breast augmentation, I typically counsel patients not to wear any bra with an underwire for 6 weeks. This time allows for the implant to settle and the incision (if inframammary) to heal well. So at the one year mark, it is definitely okay to wear a bra with an underwire.
Now if you had an inframammary incision, it is possible that the scar may still be slightly tender as scars can take 6-12 months to mature. If your scar has thickened some, it can definitely be irritated by an underwire a year after surgery.
If you have some concerns with your scar, speak to your surgeon as I am sure he/she will be happy to answer them.
Breast implants and no lift
You can certainly choose to put implants in alone but with the starting position of your nipples, they won’t look great. A lift is needed to elevate the nipples and tighten the breasts.
Implant size choices
You have a small frame and not any true ptosis so I don’t think that it will take a very large implant to achieve a nice, natural result. I would advise putting the implant behind the muscle and using a small implant so as to minimize wrinkles and lateral fullness.
In order to accurately choose the exact size and shape implant you will need to be examined in person.
