From my expertise in breast reduction and my publications on this field in the literature, I can say that breast reduction can be repeated without free nipple graft, however it will be wise to wait for a longer period before considering any form of breast surgery.

In my expert opinion, the decision of above or below the muscle is very individual to each patient. There are a number of factors that dictate the plane/position to be used, eg, thickness of the skin, current breast size, degree of breast droopiness, etc. This is best discussed and analysed during the consultation.
