This is a good question, and quite honestly it may be a matter of choice to be discussed between the Plastic Surgeon and the patient. Be aware that there is a limit of approximately 1000cc to the total of the Liposuctioning if performed with a tummy tuck( at least this is the existing recommendation in Florida). This is a good question, but it is very unlikely that you would have another rupture because the new implants don’t have a tendency to rupture at all. The answer is that they will probably look the same under the muscle, but in the lower pole of the breast where there is no muscle there may be an appearance difference if the saline implants have any rippling or fold differences than the silicone. Silicone implants weigh less than saline implants do, have less rippling in appearance, feel smoother much of the time, and are chosen the majority of the time when patients consider having a breast augmentation.Good luck to you!Frank Rieger M.D….

This a good question and several approaches can be considered. A non surgical technique utilizing a chemical called Kybella under the skin can be considered, but multiple sessions may be required over several months. You are correct in that you already have loose skin and liposuctioning will certainly make the condition worse. The scar following a tummy tuck would be well hidden along the lower abdomen along your hairline, and you certainly would get a much better belly button in the process! You still appear to have central neck platysmal muscle looseness. A limited neck revision to either surgically remove the central bands of muscle, or to tighten them up with revisional suturing may be necessary. Talk with your Plastic Surgeon about this.

You show some weakness to the right alar nostril and will need to have some supportive cartilage added to the structure of the nostril to support it properly.
