Each particular doctor may have an anesthesia preference to treat gynecomastia. It can be done with either general or local/sedation. I prefer local/sedation and patients tolerate it extremely well. It reduces cost and there is usually less post-op fogginess, nausea and sore throat.

From you photo, you do not need an extended tummy tuck. The incision should go just across your lower abdomen. It should take out and be a little longer than your C-section scar. You could also consider a mini-tummy tuck. The incision would be a little smaller and probably a littler lower. To treat the skin above the belly button, if you did a mini-TT you would need to "float" the belly button which would put it in a slightly lower position.

Thank you for your photos. Yes...this is gynecomastia and can be nicely treated. This can be done through direct excision using an incision around the nipple. It can also be removed using minimally invasive techniques though a small puncture in your armpit area. Any surrounding gynecomastia of the breast outside of the nipple area can also be removed at the same time.

This is a great question as many others may have concerns with "chubby" fingers. Unfortunately, there is no surgical correction due to the small confined space of the finger which contains nerves, tendons, etc. Diet/excersice/ weight loss is the only treatment.

You are a great candidate for a tummy tuck procedure. Obviously, if you can wait to do this after your last pregnancy would be ideal. In my experience, though, many patients do get pregnant after a tummy tuck and most often do not have to have the tummy tuck repeated. Discuss this option with your plastic surgeon. You may get correction now and not have issues requiring correction after your last child.

You are a great candidate for a tummy tuck which will remove excess skin and tighten your muscles which have been stretched out. You are not a good candidate for abdominal liposuction as you will likely be left with loose skin. Liposuction over your back and flanks will, however, give you a nice improved result.

Yes, you can indeed do a tummy tuck after a mini-tummy tuck. However, from your photos, you don't appear to have a lot of loose skin and may only need muscle tightening with possibly some liposuction. This can be performed through your existing incision and should flatten your abdomen.

I agree with your doctor. You have loose skin after pregnancies and you will get a much better result with a tummy tuck rather than liposuction of the abdomen. However, as you point out, a tummy tuck is a much bigger procedure. If you are willing to accept loose skin and just get rid of the fatty bulk, liposuction may be very helpful.

Indeed, after a 100 pound weight loss, you are probably a very good candidate for a tummy tuck. They can be expensive. If you are having skin problems from the loose skin sometimes medical insurance/Medicare/Medicaid will cover a panniculectomy, which is removal of the lower abdominal skin and fat. You can ask your doctor for a letter of medical necessity and submit to your insurance provider to see if they will approve.

Visceral fat may prevent your tummy from being completely flat, but your surgeon should be able to give you an opinion after an examination. From your photos, it does not look like you have a large amount of visercal fat. You do seem to have an umbilical hernia that can be fixed at the same time as your tummy tuck.

Thank you for posting your photos. Indeed, if you are not concerned about the bridge of your nose (the boney portion) then you do not need to do a full rhinoplasty. A tip-plasty which would include alar refinement should be just fine. This avoids having to "break" the nose and a faster, less severe, recovery is expected.
