Breast implants above or under the muscle

Where each surgeon places breast implants depends on his/her training, experience and location. On the most recent survey of which I am aware, about 55% of American surgeons placed the implant below the muscle. In Europe, the percentage placing the implant there was only about 25%.

There is also a difference in the implant used as Europeans use textured implants a majority of the time (they do well above the muscle) whereas American surgeons prefer smooth shelled implants (which tend to form scar capsules less when below the muscle).

I personally prefer textured implants placed over the muscle. However, for someone with minimal tissue superiorly I will place them under the muscle. The only way to tell this is with an exam. If you use your arms a lot or are very athletic, you should have them above the muscle if you are a candidate.

Pick your surgeon based on how comfortable you are with him/her. Decide what is important to you and discuss this at length with whichever surgeon you pick.

Older implants

I have removed implants older than 30 years old only when there was a problem. The older generation implants have a thinner envelope and runny silicone so if they are broken it should easily show on imaging.

Howeve if they do not pose a problem for you and they are intact I see no reason to subject yourself to surgery unnecessarily.

Natural look with implants

Think low profile, think small, think gel and think submuscular. Also, consider fat transfer if you have a little extra padding on your lower body and would accept a one cup size increase.

Discuss implant options with a plastic surgeon

It sounds like you have decided upon the form-stable implants without a consultation. There are some advantages for some women in specific cases, but the vast majority will still get a better result with the round cohesive implants available now.

The experts who used the “gummy bear” implants in the clinical trials estimate that only about 15-20% of women will be good candidates for them. So you need to be seen in consultation in order to determine whether they are actually the right choice for you.

And of course no one knows when and if the FDA will approve them for use in the U. S.

Ok to proceed with breast implants now

9 months is plenty of time. As long as you breasts are no longer engorged and soft, proceeding forward with surgery would be fine. I would recommend not to stimulate your breasts by squeezing your nipples.

Based on athletic nature, I’m not sure if a submuscular implant would be the best choice as this would possibly leave you with an animation deformity which is where visible irregularities occur when your muscle contracts, flattening the implant, although this would provide better implant coverage.

An option would be to place the implant above the muscle and add fat grafts to proved additional coverage while also improving cleave and upper pole fullnea. Fat is technique dependant so it’s important to have a plastic surgeon who is experienced in these techniques.

Incision Placement

A periareolar incision is a reasonable choice for you, assuming your areolar size is not unusually small. Another important element in determining incision size is based on implant material. -Saline implants are inserted unfilled and then filled when in position.

My incisions for saline breast augmentation are typically less than 3cm. -Silicone implants are filled at the time of manufacturing, and the incision then must be bigger. I use the Keller Funnel when I place silicone implants, and therefore can keep my incision lengths shorter than those surgeons who do not. Possible to just get implants?

I can appreciate that you don’t want the scars from a breast lift but i think you would be very disappointed with just a breast augmentation.

To truly get a lifted and volumized breast, you need implants as well as a breast lift. I suggest meeting with a Board Certified Plastic Surgeon for an evaluation and to discuss your options.

Lift vs implants

I think you would do well to have an augmentation (without a lift). Your nipple position is good and well above the breast crease.

Be sure to consult with a board certified plastic surgeon who can help you to achieve your goal after a proper examination which would include chest and breast measurements.

A natural breast shape can absolutely be achieved with round implants. The shape of your current breasts will play a role in the outcome and the results you will get.

Plenty of time

7 months gives you double the standard 3 month healing phase so even should a complication arise you have time to correct it and heal before your big day. The biggest issue may be adjusting your dress if you already picked it out.

It’s Node Unusual

The presence of silicone in the lymph nodes is not unusual after breast augmentation.

Sub-microscopic particles of silicone may pass through the intact envelope of a silicone gel implant, and eventually these silicone molecules are filtered by the lymph nodes, which is their purpose.

If a silicone gel implant is ruptured, there may be a much greater amount of molecular silicone that is exposed to your body, since there is no longer the outer silicone rubber shell of the implant acting as a barrier or membrane to limit contact of the silicone gel with your tissues.

The body forms a “capsule” around the implant when it is intact, and generally, even when the outer shell of the implant breaks, the leaking gel is contained by this “capsule”. By itself, silicone in the lymph nodes is not a harmful condition.

However, if the implant is confirmed as ruptured (based on physical examination, mammogram, MRI, or other diagnostic test), it should be removed and can be replaced if the patient wishes.

Although it is not an emergency to do so, there is no advantage to waiting, either.

There is no need to biopsy or remove lymph nodes that may contain silicone as a consequence of implant rupture.

Breast Feeding and Breast Implants

Hormonal fluctuations during lactation can affect the size of the breast as well as the laxity of the skin. Patients considering breast augmentation should wait six months after stopping breast feeding before they undergo surgery.

Earlier surgery may risk mastitis and infection of the breast parenchyma.

Few technical points for settling implants

In general, the intra-operative goal is to have the implant in the correct location. However,in some patients specially with tight lower pole. An implant placed under the muscle could be a little high due to muscle contraction and because the implant will follow the plan of least resistance.

Massage,compression and time will make them settle well. Give it sometime.
