Implants will enlarge your breasts and may address some of your concerns. They cannot change the normal orientation of your breasts by themselves and there are always left to right differences before and after any surgery.
Changing the shape usually also requires a breast lift. An exam and consultation with a plastic surgeon is recommended to discuss your options and expectations.

Silicone & Joint Pain
There have been a great many studies performed since silicone implants were mistakenly removed from the market in the early 90’s. None of them have indicated any relationship between the implants and arthritis or other causes of joint pain.
It appears that this was “junk science” introduced by trial lawyers.
Bed rest after breast augmentation?
After breast augmentation down time is pretty minimal. Many women who have surgery on Thursday or Friday are back to work on Monday if necessary, provided their job does not require lifting or pushing or pulling.
An administrative job, such as yours would have little down time required. Implants under the muscle are typically a little more sore than above the muscle.
I like to describe it as similar to the soreness after starting a boot camp after not having done pushups in a long time.
With a good supportive sport type bra, you should be able to return to work soon. Of course, no driving if you are on any narcotic pain medication, but transition to Tylenol or motrin within a day or two is typical.
Implant Size
I would wait a full three months for the majority of the swelling to resolve. It’s not uncommon for patients to desire larger implants. You should discuss this with your surgeon to decide on whats best for you.
An implant exchange for a larger size is less invasive than the original procedure, but does require another trip to the operating room as well as purchasing new implants. Each surgeon handles the “surgeon’s fee” associated with an upsize differently and you will need to discuss this with your surgeon.

Options for Unequal Breasts
Well first of all most women’s breast are asymmetrical. The differences you have would create difficulties when fitting bras and bathing suits. Others have mentioned reductions and lifts. These are great options that come with scars.
Consider another option such as liposuction to both breasts (because you wanted to be a B), with more volume being taken from the bigger breast.
Allow the the skin to recover and readjust, if the left breast still hangs significantly lower, a smaller lift such as a periareoloar lift can be be done.
Natural look with breast implants
It would be best to meet with an experienced surgeon in your area to determine your chest width dimensions and skin envelope prior to selecting an approrpitate implant. Generally the larger the implant,especially on a thin frame the less natural they will appear.
Too soon
It takes over 3 months for implants to settle. All patients want this process to move faster and it may for 1 implant but not for both. It takes time. Have faith, I’m sure your doctor did a good job.
Breast implants are not a substitution for a breast lift
If you need a breast lift and want the results of a breast lift, then you’ll have to have a breast lift. Using an implant will not remove the need for a lift. The only reason to use an implant is if you want your breasts to be larger.
In that case, I would place your implants under the muscle. A close family history of post-menopausal breast cancer would not preclude your having implants. You would need to have screening exams as usual.
Should I Get my Breast Aug Done by an Unsupervised Fellow at a Teaching Hospital?
You must be using the fellow for a very REDUCED fee!!! If you wish a boarded PS in attendance than either use a private practice boarded PS and pay for the expertise and/or do not insult the fellow in PS by requesting an attending PS who most likely will leave the operating room after the surgery is started.
Wanting Breast Implants- Do I Need a Lift Too??
You have good looking breasts ,it would be a pity to distort them with additional scarring. I would do a breast augmentation in the sub-fascial position ,and then re-assess in a few months .
Breast Implant Location and Exercise
The best plane for implant placement in someone who exercises a lot (especially someone who is thin with limited soft tissue coverage) is in the partial submuscular plane. There can be some movement of the implant with exercise when the implant is in the partial submuscular plane, which can be of concern in a body builder.
However, this is a small issue in most patients.

I think it would be very difficult to answer your question without knowing more (including some things that could only be determined on examination, like the quality of your skin and your tissues), so please understand that for the best information your should visit an excellent surgeon with a lot of breast experience for a full consultation.
Based on what you write and the photos you posted only, I would be very concerned that even if you were able to achieve your goal for appearance as a result of breast augmentation only, it would be a very short lived satisfaction indeed.
Here’s why- you say that your decline in breast volume and shape were the result of weight loss and aging… let’s begin by understanding that the reason weight loss and aging cause these changes in most women is because of their effect on the elasticity of the tissues.
As people gain weight, tissue elasticity is taxed and lost (how much is lost and how much weight gain is necessary for this to happen will depend on skin and tissue qualities of the individual, which are both inherited and lifestyle dependent) and while we are getting closer to being able to camouflage this, lost elasticity is generally lost forever.
This means that trying to create the appearance of a firm, perky, youthful breast through augmentation alone is more than likely to actually cause you unhappiness once the lost tissue and skin elasticity, gravity, and the weight of the implant take effect on your appearance.
I think the better question- the one you should be asking- is… “If I am going to need a mastopexy (breast lift) in order to achieve my goal, how can I maximize the chances of getting what I want and not regretting this course of action?”More than most other procedures, the outcome and your satisfaction with mastopexy will depend on who you choose to do it for you.
Again, without seeing you in person I think it would be a bit irresponsible to try to be more definitive in my answer to your question, but my impression is that you will need a breast lift to achieve the outcome you describe.
Accepting this now will help you avoid a second operation, additional anguish as you come to realize this after your first, and considerable added expense, but it makes it more important that you choose your surgeon carefully.
For deflated (drooping) breasts, high profile implants are usually NOT best.
But, more importantly, with ptosis (droop or sagging) such as yours, your surgeon must take into account the degree of skin laxity and the amount of breast implant volume needed to “fill” your exact degree of skin excess.
If, for example, you only want to be “a little bigger” but have more youthful “perky” breasts, you don’t require large implants, and must have a breast lift to reduce your skin envelope to correspond to your size wishes (or else you get the “rock in the sock” appearance).
375cc implants, IMHO, will NOT adequately fill your skin brassiere. Furthermore, high profile saline implants will have too narrow a base (10.7cm for a 310cc implant filled to maximum of 375cc) for your breast base diameter (probably 15 or 16 cm for a woman of your height).
This will give you an unnatural projecting, wide-cleavage look. If you like the 1 1/2 cup size increase that 375cc gives, then you would be better served by moderate profile 375cc (silicone gel) implants that have a 14.4cm diameter.
I absolutely recommend silicone gel implants (the latest generation of cohesive implants that cannot leak or rupture) for a more natural feel, better width to base diameter ratio (closer cleavage), least likelihood of visible rippling and no chance of eventual deflation requiring another operation to replace).
For more information read my article “Are Silicone Breast Implants Safe?” elsewhere on this site. If you want more size and projection, high profile (or moderate plus profile) implants of a somewhat larger size (and corresponding base diameters) may be best for you.
The chief comment here is that (higher) profile does NOT automatically correct droop; volume to “fill” the loose skin brassiere is the parameter to select. Your surgeon should have an idea of what size implants are needed to deal with your degree of laxity.
If this fits with your desire for a certain final breast size, great! If it is too big for you, then you need a lift and smaller implants. Once desired breast implant size is determined (and then whether or not you need a lift), then your breast dimensions are used to determine the implant base diameter/profile combination that best fits your anatomy.
It truly sounds as if your surgeon does not use this “patient request-anatomy based” method of determining choice of implants–“I use high profile 90% of the time” is the same as saying “I don’t care what the patient wants, or what their anatomy requires, here’s what I do.”
Choice of saline vs. silicone is a bit more of a discussion, but with the newest generation of silicone gel implants now available, and the wealth of scientific information showing the safety of these implants, I must say that I agree with the over 98% of my patients who choose silicone after exhaustively studying the pros and cons! Please consider additional consultation with one or more experienced ABPS-certified plastic surgeons who perform lots of breast surgery and at least have choice methodology based on patient request and anatomy
