General anesthesia is always required for a patient undergoing a mommy makeover (abdominoplasty & breast procedures- most commonly). There is a lot of manipulation to the tissue and muscle which would not be tolerated if you remain awake. There is a limit to the amount of numbing medication that can be used during your surgery based on your body weight. The procedure you will need to remove the excess skin from the upper abdomen is a reverse abdominoplasty. The excess skin and tissue will be pulled upward towards the breast. Often the incisions are completely hidden under the breast crease. Depending on the amount laxity in the skin/tissue sometimes the incision does have to extend to the midline (in between the breasts). A physical exam from your surgeon will give you a better idea on the extend of the incision.

Yes, this can be corrected. Typically we recommend Galatea Mesh - an absorbable mesh that will help support and secure the implant, it acts like an internal bra. Due to the size of your breasts however you will still have pretty minimal cleavage even with the added mesh, if upper pole fullness is what you are looking for a slightly larger implant or at least a high profile (high projection) implant will get you closer to the desired look.

In order to get your optimum results a crescent lift would be recommended. You are borderline however I feel most women in this position regret not going through with the lift if they don't. It will ensure that the nipple/areola complex is centered on the fullest part of the breast. The benefit of the crescent lift is that there is minimal scarring. The incision at the top of the areola heals very nicely.
