Breast Lift with implants
A breast lift (mastopexy) is a great procedure to help create a more “youthful” breast after breast feeding. Adding an implant at the time of a mastopexy can help restore some volume to the breast.
When you are ready to schedule a procedure, meet with board certified plastic surgeons in your area who can help you sort through your options to find the best surgical plan for you.
In a thin A cup female placiing the implant below the muscle is the best for a natural, longer lasting result with better future mamograms.
I prefer smooth round saline implants and the Mentor high profile works very well in narrow chested body types. Please see a few board certified plastic surgeons for their opinions.
Ironman competitor considering implants
In general the submuscular plane has the lowest risk of complications such as capsular contracture. However, Implants will likely decrease your power to some degree if placed submuscular. Your options are to postpone an augmentation until you are no longer competing or risk a higher rate of capsular contracture.
You already have a well shaped breast and you have to decide if the increased size is worth the possible decrease in strength or risk of contracture. If you do choose to go forward with augmentation in a sub glandular plane then a newer form stable implant would be the better choice.
The form stable implants show a lower rate of capsular contracture. in sorting out the information and making decisions.
Natural Augmentation with 550cc HP Implants
I’m not sure what you mean by natural. At 5’5″ and 130lbs, a D cup breast size is not found in too many women who do not have breast implants. So in that sense, you are not going to look very natural.
However, if you want large breasts where the implant is not visible, then you may be able to achieve that. This will depend on the amount of breast tissue you have. It is impossible for anyone to give you a good answer without having examined you.
From my experience, your description along with the fact that you’ve had two children and already have implants, I would guess that your breast tissue is more likely to be thin than full. As such, going to a 550cc HP implant will likely result in a less than natural look.
Also, if you have loose skin and sagging, don’t count on a larger implant to give you a “lift”. You will be disappointed. You may need a breast lift or mastopexy. I suggest you speak with more than one plastic surgeon to get some more ideas.
Does the price of the surgery include the post op visits?
Yes, the costs of surgery includes postoperative visits; careful postoperative care is an important part of the success of any procedure.
Breast asymmetry
Without an exam it it difficult to tell. With breast asymmetry of a substantial amount I usually cannot correct this with different size implants. If there is little tissue usually the different size implants are easily seen and it doesn’t look even.
In this type of case I would augment the breast with the same size implants and then do a Mastopexy on the larger breast to help it match the other side.

I think to get as close to the desired result that you are showing us, that a lift and a large implant will be required. I do not think you will have the round shape the desired picture shows without a lift of some type.
Clarify with your surgeon
I’m not sure why you thought you were getting smooth implants, but ended up with textured. You’ll need to ask your surgeon specifically about this. Textured implants tend to stay in place more than smooth; they also tend to be slightly more firm (not much) and MAY reduce the incidence of capsular contracture.
They’re not a bad implant choice. The bottom line is that you look good and like your breasts. If that’s the case, it may not matter which implant you have.
Saline or Silicone
Ah, the million dollar question. In general, silicone feels more natural, but the right one for you is really going to come down to a personal choice. Talk to your surgeon and then weigh the pros and cons of each.
Breast implants can appear to change as your body changes
Unquestionably, you should see your plastic surgeon for an evaluation. If you had saline implants placed, it is possible that you are experiencing a slow leak. This would be very unusual, however, to be happening on both sides.
More likely, your body has changed in some fashion, and your implants are appearing different even though they are not changing. Breast implants change breast size by altering the proportion of volume in your breast with respect to the actual tissue of your body.
Dual plane breast augmentation and possible lift needed
you appear to have mild ptosis with a greater element of glandular ptosis. Whether you would be a good candidate for a dual plane approach or a formal breast lift to get the shape you desire is based on your examination and goals.
As for implant placement, going under the muscle has many benefits such as decreasing your chances for rippling, decreasing your chances for capsular contracture, increasing your chances for greater longevity, and also a more natural look.
Dual Plane is where the muscle is separated from the gland inferiorly and allows the top of the implant to be more under the muscle where inferiorly towards your feet it mainly under the gland and there is no muscle.
The benefits of dual plane are basically the same as subpectoral, but allow for better lower pole expansion and slight nipple elevation for cases of truly mild ptosis. When I discuss with my patients about breast lifting techniques and also use of implants, I break down the different topics into parts.
The easy way to decipher what each procedure produces is that implants increase breast volume and add upper pole fullness. Implants are placed in either a subglandular or subpectoral pocket centered over the left and right chest breast footprint.
In fact, in patients with such sagging who undergo an augmentation without a lift, their breast tissue hangs off of the implant that is sitting on their chest, creating a shape like Snoopy’s nose, hence called a “snoopy” deformity.
Implants can not give a therapeutic lift and taking shortcuts will only lead to suboptimal results. In some borderline cases of ptosis (like yours), breast augmentation can be performed and if there is unsatisfactory sagging, then a lift can be performed later at a second stage.
An in person examination with a board certified plastic surgeon will be most instrumental in going over your lifting options in conjunction with implants. With respect to implant sizing, I want to start off by telling you what I tell many of my patients: Do not focus on cc’s or bra sizes as many women see their friend with “x” number of cc implants etc and think that they have an idea of what they want.
There is a lot that goes into implant selection — your breast volume, the amount of cleavage you want to achieve, your base width, your rib shape/projection, etc. Also, when it comes to bra sizes, most women are not accurately sized for their bras.
Please keep in mind that a breast implant is a round object and has a certain diameter. Your base width on each side of your chest will determine “how big of a ball” or what diameter implant you can reliably go up to with a socially acceptable cosmetic outcome.
The implant will “augment” or increase the size of your breasts to an amount acceptable to you and your surgeon. There is no correlation between height and weight and base width; therefore you need an in person examination to be accurately measured.
The most important thing to consider is really the size and look that is appealing to YOU with the implants you choose. Many surgeons have sizing models whether using bras with implants or computer simulation.
I encourage you to discuss with your surgeon many of these considerations when deciding which implant and size to use during your surgery. In case you need a mastopexy, two lifting techniques that are very good ones in the right patient and I use are based on the following:
1) if it is mainly the nipple position that needs lifting, I like the circumareolar approach (also called a donut lift) vs
2) if a large portion of the breast tissue/weight needs to be lifted, a circumvertical and sometimes a full anchor incision is needed as the circumareolar approach in my mind is not strong enough to hold the weight of the breast and results in stretching of the areola and drooping over time.
Make sure to seek consultation with a board certified plastic surgeon with expertise in aesthetic breast surgery. I hope this helps answer your question and if you are uncertain about the recommendations made by the surgeon you met with recently, I encourage you to get further consultations.
Choosing your incision for breast augmentation
If you have surgery, you will have scars. There is no magic way to perform scarless surgery. But you can have nearly invisible scars with good technique and a good plan. Breast implants can be put in through four different incisions: at the edge of the areolae, under the breasts, in the under arm, and in the belly button.
I don’t recommend the bely button incision since it leaves a visible scar at the belly button, the surgery is done blind through a long tunnel, and you may void the warranty on the implants when they are placed that way.
The other three are all good choices that depend upon your preferences and your body. If you a bit of breast tissue already you might like the incision under the breasts. The natural hang of the breasts will hide the scars.
If you have smaller breasts that you want to increase a lot in size this may not be the best choice for you, so you might want to consider the scars at the edge of your areolae.
These scars usually blend very nicely and are nearly invisible in most cases. The underarm approach can be great if you heal well with no detectable scars in some cases. If you don’t heal well, however, you will be showing the world your scars every time you wear a sleeveless top.
My personal preference for most cases is at the edge of the areolae. You should discuss the options with your surgeon and figure out what is right for you.
There is a difference between 250 and 350 cc breast implants
There is a substantial difference between 250 and 350 cc implants. Much more than between say 450 and 550. Downsizing you with a little lift sounds like a good idea. Trust your doctor.
