Breast implants are not poisonous or dangerous
There are two types of breast implants currently in the market: saline and silicone. The outer covering of ALL breast implants will eventually fail, causing a break in the implant. If you have chosen saline (salt water) implants, the body will simply absorb the saline, causing a change in the shape and size of the implant.
With silicone implants, the exposed silicone gel will not be absorbed and will remain there until removed by surgery. Therefore, some patients may have a failed silicone implant and not be aware of it (silent leak).
Silicone, by itself, is NOT dangerous or poisonous. Silicone is present in many other things, and we are constantly exposed to it. Previously, before any studies were performed, the assumption was made that silicone gel will cause auto-immune disorders (arthritis, lupus, etc.).
However, numerous large studies, comparing tens of thousands of patients have failed to show any association between silicone and auto-immune disorders.
You appear to be candidate for breast augmentation with or without breast lift
You seem to be someone who would benefit from having breast implants. You also appear to have some degree of looseness, or ptosis, in the breasts, so you may need/want to consider some form of lifting procedure too, depending upon what your goals are.
In many instances, especially if we are looking for a more “natural” appearance, lifting isn’t necessary as the volume from the implants will create enough lift to meet your goals.
Remember too, that there are really a few different reasons why we might use implants for people, and even though they may seem similar, they are different conceptually.
In your case, two of the more important things we would get out of using implants would be a sense of “fullness,” or projection in the breasts, and volume or size. These concepts take your personal goals into consideration, and they will dictate the size and type of implants you will want to consider.
I understand that you aren’t necessarily looking to be really large breasted, so the volume goal isn’t your most important consideration, but it seems like you would like more fullness, or shapeliness, to the breasts.
With those thoughts in mind, you’ll need to meet with some board certified plastic surgeons who have experience with both augmentation and lifting of the breasts, so that they can do a proper evaluation on you in person.
You will want to get detailed measurements of your chest and breasts, perhaps undergo a sizing exercise to try on implant sizers, and review pictures of your goal, or “dream,” breasts with the surgeon so that everyone understands just what you want your breasts to look like.
Then they can make a realistic recommendation for you regarding type of procedure and size and type of implant needed to fulfill that goal. to you!.
Breast Feeding with Implants
I generally tell my patients that if you were able to breast feed without implants then you should be able to breast feed with implants. Breast augmentation generally does not affect breast feeding, however not every who undergoes breast augmentation is able to breast feed.
If you have never breast fed before and your undergo breast augmentation and later decide to have kids and breast feed you may not be able to. This is usually because your body for some reason or another does not have the ability to allow breast feeding.
Generally 60-70% of the population who tries to breast feed is able to. In summary, yes if a person is able to breast feed before breast augmentation surgery, she should be able to breast feed after.
Breast augementation and feeding
if you intend to perform a breast augmentation, will probably wait 3 months , to allow all milk to fade and areola and swalling to settle. if you plan a breast lift, will probably wait minimum 6 months, as the quality of breast tissue is changing all over this period .
Implants may stimulate galactorrhea
Squeezing the breasts may stimulate ongoing lactation but you should have stopped by now. You may want to have your doctor check a prolactin level to make sure this is not high and causing the ongoing milk production.
An implant may cause your breasts to lactate, called galactorrhea. I would advise that you use the crease incision to avoid cutting across milk ducts while actively producing milk. This could potentially cause a higher chance of contamination, possible infection, and/or biofilm with subsequent increased risk for capsular contracture.
Large breast implants
As stated by others, each woman’s body differs and it’s always been my feeling that the surgeon is doing the patient an injustice if an implant is chosen prior to the procedure.
The tissue distensability and the aesthetic result should be assessed with “sizers” and with the patient in a semi-upright position while under anesthesia.
One thing that few people seem to think about is the effect of larger implants on skin and breast tissue over several years. Implants in the size range that you are considering will both thin your breast tissue and stretch your skin such that in a few years you may find emptiness in your upper chest area (from skin stretching and implant descent) and mistakenly think that you need an even larger implant.
This leads to a “slippery slope” causing the eventual need for a skin reduction (full uplift) and the necessary surgical incisions.
Just a little advice to be cautious based on 25+ years experience!.
Breast Augmentation Does Not Prevent Or Interfere With Breast Feeding
Regardless of whether one has saline or silicone breast implants or whether the implant is above or below the muscle, there is no problem with breast feeding. The only potential interference, as this is not very common, is if the implants are placed through a nipple incision where some of the ducts might get damaged.
7 months
Giving yourself 7 months to settle and heal should be plenty of time. I would suggest holding off for your dress fitting for at least 4 months, 5 if possible, to give your implants time to settle and all the swelling subside.

I don’t use transaxillary incisions
I love circumaerolar incisions, they give a more natural breast and they fade and are almost invisible. The transaxillary approach can very often give you an implant that is too high and asymmetrical – which is why I will not use it.
I agree too that they should go over the muscle, but remeber, the bigger you go the more you will sag. Nothing prevents sagging.
Bra choice after breast implants
It is perfectly safe to wear under-wire bra after breast augmentation, especially after the first 2-3 weeks. It is uncommon to experience pain with under-wire bra. On occasion when patients loose significant a=mount of weight the tissues get thin and one can experience some discomfort when the wire pushes against the implants.
For vast majority of our patients there is no restriction on the bra type. It is important for the surgeon to assess the type of breast tissue that the patient has and perform an appropriate type of breast augmentation to assure that there is maximum tissue over the implant.
Under muscle implants are best, particularly for thinner women
Your photo shows a thin chest with visible ribs; the most obvious need to place an implant under the muscle. Do not be confused with partial sub-muscular vs sub-muscular. Technically, all implants will be partially sub muscular because you do not have muscle coverage over the outer aspect of the implant.
For long term considerations as well as short term appearance you will do best to use an under muscle implant.
What can I do to correct my breasts?
The diagnosis of your breast shape is correct, but we see these problems frequently. Find an artistic and experienced surgeon and you can have a dramatic improvement with a lift, areola reduction and augmentation.
Silicone Implants feel like the real thing
Silicone implants, by far, feel much more natural than saline implants, in my opinion. However, I have all my patients feel both implants during their consultation so that each woman can make up her mind of what feels best to her.
The majority easily choose silicone gel implants because of their natural feel. Visit a boarded PS to feel both types of implants and decide what is right for you!.
Asymmetric Breasts
You are bound to get differing opinions when your breasts are not symmetric in size or shape, there is an element of constriction in the bases of each, and your nipple position is low. What you need to know is that you are not a ‘simple breast augmentation’, and in many doctor’s hands you will have a less than ideal result.
You really need an ABPS certified/ASAPS member surgeon who specializes in all aspects of cosmetic breast surgery, not just implants. This type of surgeon represents a small fraction of available surgeons in LA, so you should really take your time before moving forward.
